STATEMENT OF WORK
CARPET REPLACEMENT FOR BUILDINGS 4534 AND 5015
SEYMOUR JOHNSON AIR FORCE BASE
27 August 2024
I. SCOPE. Carpet removal and installation service at Building 4534 and Building 5015, Seymour Johnson Air Force Base (SJAFB), North Carolina. This requirement includes the furnishing all labor, tools, equipment, materials, transportation, and performance of work necessary for the removal and disposal of broadloom carpet and replacement/installation of carpet tile in accordance with the Air Force Carpet Acquisition Program, drawings, contract specifications, as well as commercially accepted standards, processes, and procedures.
II. AIR FORCE CARPET ACQUISITION PROGRAM
A. The Air Force Carpet Acquisition Program is a two-tiered mandatory-use program. Tier I refers to the carpet manufacturers, and Tier II refers to the carpet installers. The contract associated with this statement of work is a Tier II contract in accordance with the Mandatory Use Policy for Air Force Carpet Acquisitions.
B. ORDERING FROM THE TIER I CONTRACT. There are seven (7) Tier I carpet manufacturers/contracts that are directed for use. All carpet and carpet adhesive materials associated with this contract shall be acquired from one of the seven (7) Tier I vendors by the awardee of this contract, herein referred to as the Contractor. The customer provides one (1) selection from the approved list of each Tier I vendor. The Contractor shall contact Tier I vendors to obtain/negotiate pricing. When contacting the Tier I vendors, the Contractor shall state they are seeking pricing in accordance with the Air Force Carpet Acquisition program and reference the associated Tier I contract identified below in Section II.C. After obtaining pricing, the Contractor shall determine which selection to incorporate in their quote. The Contractor shall not add any cost over or above the pricing of materials acquired from the Tier I vendor. Further, Tier I materials shall be separately priced in the Contractor’s quote.
C. TIER I VENDORS AND CUSTOMER SELECTIONS. Carpet selections for this requirement are “Geometric Large Category.” The following list identifies the Tier I vendors, contact information, and customer selections.

III. ORDER OF WORK.
A. BUILDING 4534. The Contractor shall remove existing carpet and replace approximately 250 square yards (SY) of broadloom carpet with carpet tile. Reference Attachment 3 of the Solicitation Document.

1. The existing VCT floor tile under the carpet is not to be disturbed. Reference Attachment 4, Facility Inspection/Assessment Letter for Building 4534.
2. The Contractor shall repair, level, and prepare existing floor as required for installation of the new carpet tile.
3. The existing wooden base mounding shall remain and/or be reused if removal is required during carpet removal and/or installation.
4. The Contractor shall provide a minimum quantity of two (2) boxes of carpet tiles as excess upon completion of the work for customer use.
B. BUILDING 5015. The Contractor shall remove existing carpet and replace approximately 360 square yards (SY) of broadloom carpet with carpet tile. Reference Attachment 3 of he Solicitation Document. The Contractor shall remove existing cove base and replace with new cove base.

C. SERVICE DESCRIPTIONS:
1. Carpet Removal. Includes all labor, equipment, materials (with exception of any materials purchased through Teir I vendors, which shall be separately priced IAW the Air Force Carpet Acquisition Program), and performance of all work necessary for the demolition of pre-existing carpet. Carpet removal includes the transportation of applicable materials and/or equipment, and the removal and disposal of all demolished material in accordance with industry standards and local regulations.
2. Cove Base Removal. Includes all labor, equipment, materials (with exception of any materials purchased through Teir I vendors, which shall be separately priced IAW the Air Force Carpet Acquisition Program), and performance of all work necessary for the demolition of cove base. Cove base removal includes the transportation of applicable materials and/or equipment, and the removal and disposal of all demolished material in accordance with industry standards and local regulation.
3. Floor Preparation. Includes all labor, equipment, material (with exception of any materials purchased through Teir I vendors, which shall be separately priced IAW the Air Force Carpet Acquisition Program), and performance of all work necessary for the preparation of floors prior to installation of carpet tiles. Floor preparation includes the transportation of applicable materials and/ or equipment, and routine leveling, cracker repair, and encapsulating of existing adhesive, sealant, etc., as required and in accordance with material application/installation requirements.

Floor preparation assumes routine floor preparation. Reference also Section VI.F. entitled Differing Site Conditions and Changes.
4. Carpet Installation. Includes all labor, equipment, materials (with exception of any materials purchased through Teir I vendors, which shall be separately priced IAW the Air Force Carpet Acquisition Program), and performance of all work necessary for the installation of carpet tiles in accordance with manufacturer’s instructions and warranty requirements. The Contractor shall remove, modify and reinstall existing doors, floors, thresholds, and shoe molding as necessary when installing the new carpet. Carpet installation includes the transportation of applicable materials and/or equipment.
5. Cove Base Installation. Includes all labor, equipment, material to include cove base and cove base adhesive, and performance of all work necessary for the installation of carpet tiles. Cove base installation includes the transportation of applicable materials and/or equipment. Cove base shall be vinyl, four (4) inches in height, one-eighth (1/8) inch thick, and of a complimentary color to the carpet selection. Cove base adhesive shall be per manufacturers’ recommendation. The Contractor shall provide to the Government manufacturer and product information of the proposed cove base and cove base adhesive with the Contractor’s quote. Within five (5) work days of contract award, the Contractor shall provide to the Government a key ring of cove base sample for Government selection and approval.
IV. SITE VISIT. Physical visual inspection and site survey is strongly encouraged, but not required, for prospective offerors for scope of work verification and measurements prior to submitting quotes/proposals. Floor Plan drawings are available. Reference the section entitled Instructions, Conditions, & Notices to Offerors or Quoters of the solicitation document for site visit dates and coordination requirements.
V. COMMENCEMENT, EXECUTION, AND COMPLETION OF WORK
A. AUTHORITY. Only the Contracting Officer has the authority to enter into, administer, or terminate contracts and to make related determinations and findings. No other individual or organization has authority to make any commitments or changes that will affect price, quality, quantity, delivery, other terms and conditions of the contract, or in any way direct the Contractor or its subcontractors to operate in conflict with the contract terms and conditions.
B. SCHEDULE AND COORDINATION.
1. Schedule of Onsite Work. Unless otherwise directed, hours of operation shall be between 7:30 a.m. and 4:30 p.m., Monday through Friday, with the exception of federal holidays. Any deviations (i.e. requests by the Contractor to work outside of the stated hours) must be approved and authorized in advance by the Contracting Officer. Observed federal holidays include:

2. Base Access and Authorization. The Contractor shall coordinate base access with the designated point of contact (POC) not later than two (2) weeks prior to the anticipated commencement of onsite work. Base access requirements shall include, but may not be limited to, government-issued identification (i.e. driver’s license) information for each onsite worker and vehicle registration and insurance information.
3. Inspection/Acceptance of Work. The Contractor shall notify the designated POC upon completion of floor preparation for interim inspection prior to proceeding to carpet installation. The Contractor shall also notify the POC upon completion of work for final inspection and acceptance.
C. GENERAL CLEANUP. The Contractor shall keep the worksite cleaned daily of all installation/removal trash, trash generated by employees, debris, and/or demolished or excess materials. Prior to completion of the work each day, the Contractor shall remove from the worksite all waste materials, rubbish, and debris. Upon completion of the work, the Contractor shall leave the work premises in a clean, neat and workmanlike condition satisfactory to the Contracting Officer. This shall be the required condition at the time of acceptance of all work under this contract.
D. PARKING. The Contractor or Contractor’s employees, including subcontractors, shall not park any personally owned vehicles (POVs) or company and/or delivery vehicles on grassy areas without prior approval from the Contracting Officer. When permitted, parking on grassy areas will only be for short-term delivery purposes (to include heavy tools, equipment, construction materials, etc.). The Contractor shall use designated hard surfaces or existing parking lots near or within the project site for parking of POVs or company and/or delivery vehicles. Permission to park on grassy areas does not relieve the Contractor of the responsibility to restore affected areas to original condition. Any/all tire and track impressions (ruts) created on wet or soft soil by vehicles and/or equipment used in the Contractor’s operation shall be repaired. The Contractor shall restore the affected area(s) to its original condition within a timely manner.
E. EMPLOYEE STANDARDS. The Contractor shall follow all rules and regulations in place at SJAFB. All work under this contract shall be performed in a skillful, workmanlike, and professional manner. The access of Contractor and subcontractor personnel on SJAFB and project sites shall be limited to official business only. Persons in violation of this shall be apprehended and appropriately disciplined.
F. DIFFERING SITE CONDITIONS AND CHANGES. The Contractor is responsible for reasonably assessing the site prior to proposal/quote submission. During the course of performance, should conditions unforeseen conditions be identified that present a hazard and/or alternate or excess service requirements that were not, and feasibly could not, have been identified prior to the initiation of service, the Contractor shall immediately notify the Government.
VI. WARRANTY. The Contractor shall guarantee all work for a period of one (1) year from completion date. This covers defects to materials and the installation itself. The Contractor shall be liable for any property damage if caused by negligence or willful act of employees. If damages are incurred, the Contractor shall be responsible for any repairs at no additional cost to the Government.
