Instructions, Conditions, & Notices to Offerors or Quoters

Miscellaneous text in this section has been modified to:

SITE VISIT. Offerors are encouraged and expected to inspect the site where work is to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award.

(a) . Contractors should be at the Seymour Johnson Visitors A site visit has been scheduled for Tuesday, 10 September 2024 at 2:00 PM EDT Center, located at 1050 Wright Brothers Ave, Goldsboro, NC, 27534 no later than to allow time for obtaining 13:45 AM EDT 10 September 2024 vehicle passes. All visitors must have a valid photo ID, vehicle registration, and proof of insurance.

(b) . In order to RSVP, submit the name of all RSVP for the site visit is requested not later than Monday, 9 September 2024 at 4:30 PM EDT attendees via e-mail to both angela.gibson.4@us.af. mil and aaron.vanvynckt.1@us.af.mil.

(c) Only one site visit will be held.

QUOTE SUBMISSION. Addendum to FAR 52.212-1, Instructions to Offerors - Commercial Items The following is inserted as an additional paragraph of the provision:

(m) Quote Preparation Instructions.

(1) Provide the contemplated material information as follows:

(A). - In accordance with the Air Force Carpet Acquisitions Program, the Offeror shall provide the Carpet and Carpet Adhesive following information associated with the carpet selection (i.e. one of the seven Government-provided selection options provided in the Statement of Work): manufacturer name, material description (product number, style, color, etc.), unit price, quantity, and total price of Tier I materials.

(B). - at minimum, manufacturer/brand, product identifier/description, and recommended colorCove Base and Cove Base Adhesive (s).

(2) Provide schedule identifying the following:

(A) Lead time for materials

(B) Performance period for all work at Building 4534, to include staging/preparatory requirements, performance of work, cleanup, etc.

(C) Performance period of all work at Building 5015, to include staging/preparatory requirements, performance of work, cleanup, etc.

(3) Standard Form (SF) 1449. Complete blocks 12, 17a, 30a, 30b, and30c on page 1 and the unit prices and total amounts on page 3. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation unless the offeror affirmatively states any exceptions, deviations or waivers to the contract terms and conditions. An incomplete quote may be considered to indicate lack of understanding of the requirement and may result in the entire quote being determined "unacceptable" and eliminated from further consideration. Pricing found elsewhere will NOT be considered.

INSURANCE. As prescribed in FAR Part 28, Bonds and Insurance, rates required for this purchase order. FAR 28.307-2, Liability:

(a) Workers' compensation and employer's liability. Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers. (See 28.305(c)for treatment of contracts subject to the Defense Base Act.)

(b) General liability.

(1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence.

(2) Property damage liability insurance shall be required only in special circumstances as determined by the agency.

(c) Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims.
