About this file

This is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania under Executive Order 2021-06, applicable to contractors and grantees conducting work for the Pennsylvania Game Commission on the 23WHM SGL 270 New Crew HQ MECH project. The form mandates that all contractors certify compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and applicable state labor and workforce safety laws prior to contract award. Contractors must affirm adherence to thirteen specific labor statutes and regulations, including the Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, and the Construction Industry Employee Verification Act, among others. A duly authorized official of the contractor or grantee must execute the certification under penalty of law.

The form establishes significant consequences for false certification, including contract termination and three-year ineligibility to bid on Commonwealth contracts under 62 Pa.C.S. 531 (Debarment or Suspension). Contractors are required to report any change in compliance status to the purchasing agency immediately upon occurrence. The certification is subject to penalties under 18 Pa.C.S. 4904 (Unsworn Falsification to Authorities), and the executing official must attest that they are authorized to bind the contractor or grantee to the stated compliance certifications.
