About this file

The document is a Worker Protection and Investment Certification Form issued by the Commonwealth of Pennsylvania, mandated under Executive Order 2021-06. The form requires contractors and grantees to certify compliance with various state labor and workforce safety laws, including regulations around unemployment compensation, workers' compensation, workplace misclassification, child labor, minimum wage, prevailing wage, equal pay, and other employment-related statutes.

The certification carries significant legal implications, with potential penalties for false declarations including contract termination and a three-year debarment from bidding on state contracts. Contractors must affirm their compliance with the listed labor laws and commit to immediately reporting any changes in their compliance status to the purchasing agency. The form requires an authorized official's signature, printed name, title, and contractor/grantee name, with understanding that providing false information is subject to legal consequences under Pennsylvania law.
