About this file
This is a Worker Protection and Investment Certification Form (BOP-2201) issued by the Commonwealth of Pennsylvania in conjunction with a solicitation for SR117 Thermoform PVC Pipe Lining services in Lebanon County. The certification form requires all contractors and grantees bidding on Commonwealth contracts to attest compliance with Pennsylvania's labor and workforce safety laws, including the Unemployment Compensation Law, Workers' Compensation Law, Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, and multiple additional statutes governing employment practices and safety standards. Contractors must submit bids electronically through the PA Supplier Portal by the bid opening scheduled for November 4, 2026, at 1:00 p.m. Eastern Standard Time via Microsoft Teams. The awarded Purchase Order will be valid through June 30, 2027, with no renewal options, though the Commonwealth reserves the right to extend any single contract term for up to three months under identical terms and conditions.
This solicitation is set aside for small business bidders only, and all line items must be bid in full or the submission will be deemed void. Contractors must certify their compliance with the designated labor and safety laws by submitting the completed Worker Protection and Investment Certification Form; providing false certification subjects the contractor to penalties including contract termination and three-year bid ineligibility under Pennsylvania's debarment and suspension provisions, as well as potential criminal charges for unsworn falsification to authorities. Interested bidders experiencing technical difficulties with the PA Supplier Portal should contact the Supplier Service Center at (877) 435-7363, and project-specific inquiries should be directed to Brian Horning at brihorning@pa.gov.
