About this file
This is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for contractors and grantees bidding on state contracts, specifically for mowing and miscellaneous services in Philadelphia County through the Pennsylvania Department of Transportation (PennDOT). Contractors must certify compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and applicable state labor and workforce safety laws, including provisions related to workplace misclassification, child labor, minimum wage, prevailing wage, equal pay, employment medical examinations, farm labor, wage payment, industrial homework, employee verification, healthcare overtime restrictions, apprenticeship and training, and employment record inspections. The certification requires an authorized official to sign and date the form, confirming the contractor's compliance status and obligating them to report any changes in compliance immediately to the purchasing agency.
Pennsylvania law establishes significant penalties for providing false certifications, including contract termination and three-year ineligibility to bid on state contracts under 62 Pa. C.S. § 531 (Debarment or Suspension). The certification is subject to the provisions and penalties of 18 Pa. C.S. § 4904 (Unsworn Falsification to Authorities). This bid solicitation for mowing, string trimming, and trash pickup and removal services has been reserved for Small Business Contracting, requiring all bidders to complete and submit this worker protection certification as part of their proposal.
