SUBJECT: STATEMENT OF WORK
PROJECT NO.: VDYD 162003
PROJECT TITLE: Repair Building 5002 PROJECT LOCATION: 126th Air Refueling Wing, Scott AFB, Illinois, 62225
JURISDICTIONS
The Project Work Site, Scott Air Force Base, 126th Air National Guard (Scott AFB IL ANG), is a U.S. military installation and as such, all aspects of the work on the site shall be subject to the requirements herein. The
Contractor shall coordinate all activities for compliance with these requirements through the Contracting
Officer’s Representative (COR).
DESCRIPTION OF WORK:
The work consists of furnishing all labor; materials and equipment necessary to complete the following items. (See Final Plans, dated 22 February 2017 for details):
Area #1: Rooms 100-104 (Command Suite)
1. DEMOLITION
a. Remove all hollow metal frames, sidelights, blinds, and Door 102a. Store Door 102a, all other materials will be turned over to the 126th ARW
b. Remove and store carpet tiles along hollow metal frames both sides of wall, and along the inside of the work counter area.
c. Remove a portion of the existing drywall soffit and metal stud work above the work counter to points noted on drawing.
d. Remove and store all parts of the caesarstone material countertop in the work area, including all glass partitions and chrome piping.
e. Remove and store a portion of the existing acoustical ceiling and components on each side of the soffit above the work counter as noted on drawing.
f. Remove a portion of the existing floor tile and tile base along all new walls as needed to continue the herringbone design using full tiles to the edge of the new wall. Store tile base.
g. The Contractor is responsible for all clean up and disposal of all demolished materials.

2. INTERIOR FRAMING
a. Frame the new walls at the work counter as shown on drawings to complete the new layout.
b. Frame new wall between Room 101 and 102 to fill in the opening up to the new door hollow metal frame.
c. All metal studs will conform to ASTM C 645, 25 gauge.
3. INTERIOR FINISHES
a. Furnish and install new 5/8 inch thick type “X” fire code drywall over the metal stud walls in Rooms 101 & 102.
b. Apply joint compound, and tape all joints and screw heads until smooth.
c. Apply paint to all new or partial new walls in Room 101 up to the nearest corner. Color Sherwin
Williams No. 6227 “Meditative Blue”, Eggshell finish.
d. Apply paint to all walls in Room 102. Color Sherwin Williams No. 7506 “Loggia”, eggshell finish.
e. Reinstall Door 102a, including hardware. Contractor is responsible for the hollow metal frame.
f. Prep and repaint all door frames in Area 1. Color Sherwin Williams No. 7605 “Gale Force”.
g. Install GFM floor tile in herringbone configuration to the new wall framing. Grout color to match the existing and approved by the COR prior to commencing work. Grout furnished by the Contractor. Install GFM carpet tiles in all areas that were previously removed. All adhesive will be pressure treated and supplied by the Contractor.
h. Install new vinyl base as needed to complete all walls within the work area. Base color, Armstrong #51”Deep Blue Sea”. The Contractor is responsible for all floor prep or repairs. Install stored tile base in Vestibule 100 side of new walls. Contractor to supply grout and adhesive.
i. Install new acoustical ceiling and components to complete all ceilings to the new walls and or soffits. All ceiling materials to match existing.
j. All finish work will be applied to meet or exceed industry standards and approval by the COR. All doors will receive a “touch up” to the wood finish to match the existing color as close as possible.
4. ELECTRICAL/COMMUNICATIONS
a. Remove the existing can lights in the soffit area and reinstall as shown on drawing.
b. Remove all electrical circuits in demo walls back to the nearest junction box and label.
c. All electrical work will meet or exceed NEC State, Federal or Local codes.
d. Remove all COMM drops in demo walls to above the acoustical ceiling and neatly wrap for future use.
5. PLUMBING/FIRE SUPPRESSION
a. Contractor to adjust the existing sprinkler heads located in the soffit area above the work counter to meet the elevation of the acoustical ceiling. Work includes draining the system, notifying the COR and SAFB Fire Department prior to commencing work.
b.
Area #2: Rooms 107, 108 & 108a (CE Operations)
6. DEMOLITION
a. Remove a portion of the existing drywall soffit, walls and metal stud work above the work counter to points noted on drawing. Demolish wall providing a new door opening in wall for Door 107a to be installed.

b. Remove and store a portion of the caesarstone countertop in the work area including all glass partitions. Remove the bookcase in the work counter and store.
c. Remove a portion of the acoustical ceiling and components on each side of the soffit above the work counter as noted on drawing.
d. Remove Door 107a, including the sidelight glass and frame. Store Door 107a, all other materials will be turned over to the 126th ARW.
e. Remove a portion of the existing herringbone floor tile, carpet tile and base along the work counter and at the point of door removal as needed to accommodate new walls. Tile should be cut to fit up to the new wall configuration.
f. The Contractor is responsible for all clean up and disposal of all demolished materials.
7. INTERIOR FRAMING
a. Frame the new walls at the work counter and for Door 107a as shown on drawings to complete the new layout.
b. All metal studs will conform to ASTM C 645, 25 gauge metal.
8. INTERIOR FINISHES
a. Furnish and install new 5/8 inch thick type X fire code drywall over metal studs. Apply joint compound, and tape all joints and screw heads until smooth.
b. Apply paint to all new or partial walls in Room 107 up to the nearest corner. Color Sherwin Williams No. 6227 “Meditative Blue” eggshell finish.
c. Apply paint to the new wall in Vestibule 100, Sherwin Williams No. 2741 “White Dove”.
d. Reinstall Door 107a, including hardware. Contractor is respomsible for the hollow metal frame.
e. Prep and repaint all door frames in Area 2. Color Sherwin Williams No. 7605 “Gale Force”.
f. Install GFM carpet tiles in all areas previously removed. All adhesive will be pressure treated and supplied by the Contractor.
g. Install new vinyl base as needed to complete all walls within the work area. Base color, Armstrong #51 “Deep Blue Sea”. The Contractor is responsible for all floor prep or repairs.
Contractor to supply grout and adhesive.
h. Install new acoustical ceiling and components to complete all ceilings to the new walls and or soffits. All ceiling materials to match existing.
i. All finish work will be applied to meet or exceed industry standards, and approval by the COR. All doors will receive a “touch up” to the wood finish to mach the existing color as close as possible.
9. ELECTRICAL/COMMUNICATIONS
a. Remove the existing can lights in the soffit area and reinstall as shown on drawing.
b. Contractor to provide new light fixtures as shown in Room 107. All fixtures to match the existing as close as possible.
c. Contractor to provide electrical and COMM in the existing floor box in Room 107. Contractor responsible for all materials from supply to floor box including tracing the existing conduits etc. Location of floor box has been identified.
d. Provide new circuit for new roll up security door at work counter.
e. All electrical work will meet or exceed NEC State, Federal or Local codes.

10. SPECIALTIES
a. Furnish and install metal new roll up security door by wayne Dalton or equal to close off the work counter area. Door will be electrical, and have locking mechanism for security.
Contractor to provide submittals to the COR for approval.
AREA #3: Room 124 (Conference Room)
11. DEMOLITION
a. Remove Door No. 124b and frame. Store door and all materials to be turned over to the 126th ARW.
b. Demo wall between hallway 121 and Room 124. Remove Door No. 124a and sidelite. Store door and all materials to be turned over to the 126th ARW.
c. Remove all wallpaper from Room 124
d. Remove a portion of the acoustical ceiling and components on wall to be demolished.
e. The Contractor is responsible for all clean up and disposal of all demolished materials.
12. INTERIOR FRAMING
a. Frame the new wall to enclose opening for Door 124b. Frame new wall between Room 124 and Corridor 121 as shown on drawings to complete the new layout.
b. Install blocking for television mount on wall as shown on plan.
c. All metal studs will conform to ASTM C 645, 25 gauge metal.
13. INTERIOR FINISHES
a. Furnish and install new 5/8 inch thick type Z fire code drywall over metal studs. Apply joint compound, and tape all joints and screw heads until smooth.
b. Apply skim coat to all walls until smooth and make ready for paint.
c. Apply paint to all new walls in Room 124. Color Sherwin Williams No. 7506 “Loggia” eggshell finish.
d. Install Door 124a, including hardware as shown on drawing. Contractor is responsible for the hollow metal frame.
e. Prep and repaint all door frames in Area 3. Color Sherwin Williams No. 7605 “Gale Force”.
f. Install GFM carpet tiles in all areas within the work area. All adhesive will be pressure treated and supplied by the Contractor.
g. Install new vinyl base as needed to complete all walls within the work area. Base color, Armstrong #51 “Deep Blue Sea”. The Contractor is responsible for all floor prep or repairs.
h. Install new acoustical ceiling and components to complete all ceilings to the new walls and or soffits. All ceiling materials to match existing.

c. Remove all speakers and wiring to above the existing ceiling and store for future use. Reinstall as shown on drawings and reconnect and install speakers as shown.Remove television wall mount and store. Reinstall wall mount as shown on plan.Furnish and install 2 new 2 x 4 light fixtures to match the existing as shown on drawing.
d. All electrical work will meet or exceed NEC State, Federal or Local codes.
AREA #4: Room 129 (126CES Break Room)
15. DEMOLITION
a. Remove Door No. 129a, hardware, frame and sidelight. Store door and all materials to be turned over to the 126th ARW.
b. Remove offset wall between Room 129 and Room 132 as shown on drawing.
c. Remove a portion of the acoustical ceiling and components on wall area to be demolished.
d. Remove all cabinetry and countertops.
e. Remove all floor tile and base.
16. INTERIOR FRAMING
a. Frame the new wall to enclose opening for Door 129a. Frame new wall between Room 129 and Room 132 as shown on drawings to complete the new layout.
b. Install blocking for television mount.
c. All metal studs will conform to ASTM C 645, 25 gauge metal.
17. INTERIOR FINISHES
a. Furnish and install new 5/8 inch thick type Z fire code drywall over metal studs. Apply joint compound, and tape all joints and screw heads until smooth.
b. Apply skim coat to all walls until smooth and make ready for paint. Patch and repair all drywall as needed.
c. Apply paint to all new walls in Room 129. Color Sherwin Williams No. 7506 “Loggia” eggshell finish.
d. Install Door 129a, including hardware as shown on drawing. Contractor is responsible for the hollow metal frame.
e. Prep and repaint all door frames in Area 4. Color Sherwin Williams No. 7605 “Gale Force”.
f. Install new acoustical ceiling and components to complete all ceilings to the new walls. All ceiling materials to match existing.
g. Prep and patch the existing concrete floor as needed. Apply water based acid stain to the floor and seal following the manufacturers recommended methods. Provide color samples for approval by the COR.
h. Install new vinyl base as needed to complete all walls within the work area. Base color, Armstrong #51 “Deep Blue Sea”. The Contractor is responsible for all floor prep or repairs.
i. Insulate walls using R-9 batts at Door openings and new wall between Room 129 and Room 132.
j. Provide new cabinets in Room 129. Submit color and cabinet shop drawings to the COR for approval.
k. Provide new solid surface countertop and subway tiled backsplash. Submit color and shop drawings for approval by the COR. Include hardware for all doors.

l. Provide a stainless steel backsplash for area behind the sink. Include new faucet and piping to make connections for a complete installation. Seal the countertop at all wall / top junctions with a mold resistant sealant color match to the top.
m. All finish work will be applied to meet or exceed industry standards, and approval by the COR.
All doors will receive a “touch up” to the wood finish to mach the existing color as close as
18. ELECTRICAL/COMMUNICATIONS
a. Remove all electrical circuits in demo walls back to the nearest junction box and label. Reinstall all receptacles and circuits as shown on plan.
b. Remove all COMM drops in demo walls to above the acoustical ceiling and neatly wrap for future use. Reinstall all COMM drops as shown on drawings.
c. Provide new receptacles and COMM as shown on drawings. Include provisions for 2 refrigerators, and 3 microwave appliances.
d. Provide under cabinet lighting LED strip light for all applicable cabinetry with a dedicated switch and associated low voltage transformer (hidden from view).
e. Provide new cable jack and plate at television location as shown on drawing. Trace the existing cable from Corridor 121 and supply new cable to the new location.
f. All electrical work will meet or exceed NEC State, Federal or Local codes.

19. HVAC
a. Remove thermostat in demo wall and store above the existinjg ceiling. Reinstall thermostadt in new wall as shown on drawing.
20. PLUMBING/FIRE SUPPRESSION
a. Connect new faucet to water and waste lines at sink location. Include all piping necessary for a complete reinstallation.
21. SPECIALTIES
a. Contractor shall furnish and install two (2) refrigerators with freezer compartments. Each refrigerator shall be stainless steel in finish with a minimum of 18 cu. Ft. of refrigerator storage.
b. Contractor shall furnish three (3) microwaves within the proposed cabinetry to reduce countertop usage. All microwaves shall be mounted with its own dedicated outlet. Each microwave shall be stainless steel in finish with a minumum wattage of 1000W.
c. All appliances shall meet Energy Star requirements.
AREA #5: Rooms 131-135 (Shops)
22. DEMOLITION
a. The 126th ARW will remove all possible obstructions from Area 5 as possible so the Contractor can freely work in all areas with few acceptions.
b. Remove all existing VCT floor covering and vinyl base.
c. Demolish wall in Room 132 as needed to allow metal stud installation for new wall from Room
139.
d. Remove a portion of the acoustical ceiling and components on wall to be demolished in
Room 132.

e. Remove all existing loose floor paint and prep the floor in Room 135 for new floor paint to be applied. Follow all manufacturers recommended methods for floor prep. Install new coat of Sherwin Williams Armorseal 1000 HS two component epoxy material to the prepared surface. Color to match the existing as close as possible. Application should follow the manufacturers recommended methods for installation, including the application of “shark Grip’ Slip Resistant material or approved equal. Newly painted surfaces shall have a 3 to 5 mil thickness when complete. In addition, apply yellow and black safety paint to the area outside of the rollup door in Room 135 following the existing configuration. Submit paint manufacturer to the COR for approval.
23. INTERIOR FINISHES
a. Room 135, install new coat of Sherwin Williams Armorseal 1000 HS two component epoxy material to the prepared surface. Color to match the existing as close as possible. Application should follow the manufacturers recommended methods for installation, including the application of “shark Grip’ Slip Resistant material or approved equal. Newly painted surfaces shall have a 3 to 5 mil thickness when complete. In addition, apply yellow and black safety paint to the area outside of the rollup door in Room 135 following the existing pattern. Submit paint manufacturer to the COR for approval.
b. Install yellow and black safety striping inside the bay following the drawing provided.
c. Prep and repaint all doors and frames in Area 5. Color Sherwin Williams No. 7605 “Gale Force”.
d. Prep and patch the existing concrete floor as needed. Apply water based acid stain to the floor and seal following the manufacturers recommended methods. Provide color samples for approval by the COR.
e. Install new vinyl base as needed to complete all walls within the work area. Base color, Armstrong #51 “Deep Blue Sea”. The Contractor is responsible for all floor prep or repairs.
f. Install new acoustical ceiling and components to complete the ceiling in Room 132. All ceiling materials to match existing.
g. Apply skim coat to all walls in Rooms 131,132,133, and 134 until smooth and make ready for paint. Patch and repair all drywall as needed.
h. Apply paint to all walls in Rooms 131, 132, 133 and 134. Color Sherwin Williams No. 7506 “Loggia” eggshell finish.
i. All finish work will be applied to meet or exceed industry standards, and approval by the COR.
24. ELECTRICAL/COMMUNICATIONS
a. Contractor will provide a controller and all programming necessary for the exsting overhead door operator to allow a total of 10 remote door controllers to be provided for all vehicles parked in this space. Controller must be compatable with the existing opener and maunal door controller. Contractor to provide submittals for the COR to approve.
AREA #6: Room 140 (Material Shop)
25. DEMOLITION
a. Demolish wall in Room 140 as shown on drawing. Prepare wall areas to accept new walls as shown on drawing.
b. Remove all existing VCT floor covering and vinyl base.

c. Remove a portion of the acoustical ceiling and components along demo wall as noted on drawing.
d. The Contractor is responsible for all clean up and disposal of all demolished materials.
26. INTERIOR FRAMING
a. Frame the new wall to enclose Room 140 as shown on drawing to complete the new layout.
Frame new door opening for Door 140b on wall between Room 140 and 138. See door schedule for details.
b. All metal studs will conform to ASTM C 645, 25 gauge metal.
27. INTERIOR FINISHES
a. Furnish and install new 5/8 inch thick type Z fire code drywall over metal studs.
b. Apply joint compound, and tape all joints and screw heads until smooth.
c. Apply skim coat to all walls until smooth and make ready for paint.
d. Apply paint to all new walls in Room 140. Color Sherwin Williams No. 7506 “Loggia” eggshell finish.
e. Install Door No. 140b. Door and hardware will be GFM, Contractor to supply the hollow metal frame. See door schedule for details.
f. Prep and repaint all doors and frames in Area 6. Color Sherwin Williams No. 7605 “Gale Force”.
g. Install new acoustical ceiling and components in Room 140. All ceiling materials to match existing.
h. Prep and patch the existing concrete floor as needed. Apply water based acid stain to the floor and seal following the manufacturers recommended methods. Provide color samples for approval by the COR.
i. Install new vinyl base to complete all walls within the work area. Base color, Armstrong #51 “Deep Blue Sea”. The Contractor is responsible for all floor prep or repairs.
j. All finish work will be applied to meet or exceed industry standards, and approval by the COR.
28. ELECTRICAL/COMMUNICATIONS
a. Remove all electrical circuits in demo wall back to the nearest junction box and label. Reinstall all receptacles and circuits as shown on plan.
b. Remove all COMM drops in demo wall to above the acoustical ceiling and neatly wrap for future use. Reinstall all COMM drops as shown on drawings.
c. Contractor to provide new receptacles and circuits in new wall. All new wall plates will match existing.
d. Rewire light fixtures to switch location as shown on drawing.
e. Furnish and install new 2 x 4 light fixtures to match the existing as shown on drawing.
f. All electrical work will meet or exceed NEC State, Federal or Local codes.
AREA #7: Corridor #121
29. DEMOLITION
a. Remove all wallpaper and select drywall from Corridor 121.
b. Remove the existing floor tile and tile base along all walls.
c. Remove the 2 existing water fountains and cap all plumbing lines.

30. INTERIOR FRAMING
a. Install new moisture resistant 5/8 drywall at new water cooler location.
31. INTERIOR FINISHES
a. Apply skim coat to all walls until smooth and prep for paint.
b. Apply paint to all new walls in Corridor 121. Color Sherwin Williams No. 2741 “White Dove” eggshell finish.
c. Prep and repaint all doors and frames in Area 7. Color Sherwin Williams No. 7605 “Gale Force”.
d. Prep and patch the existing concrete floor as needed. Apply water based acid stain to the floor and seal following the manufacturers recommended methods. Provide color samples for approval by the COR.
e. Install new vinyl base as needed to complete all walls within the work area. Base color, Armstrong #51 “Deep Blue Sea”. The Contractor is responsible for all floor prep or repairs.
32. PLUMBING
a. Furnish and install Halsey Taylor Model HTHB-HAC or equal. Provide submittals for approval by the COR. Furnish all necessary piping to complete all work according to State, Local, or Federal codes. Supply and waste lines can be accessed from the sink in Room 129.
AREA #8: Room #116 (Mechanical Room – Boiler Replacement)
33. DEMOLITION
a. Contractor to remove 2 existing boilers and all unecessary piping. Contractor will work in conjunction with the 126th Maintenance Staff during construction.
b. Contractor shall clean area for new boilers upon removal and prior to installing the new equipment.
34. INSTALLATION
a. Contractor shall furnish and install two (2) new Lochinvar Model KHN-199 - 190,000 BTU boilers with 90 Plus rating or equal to replace the two existing units. Reconnect all piping to the existing gas lines run within the room. Include Submittal to the COR for approval.
b. All electrical, including low voltage, and flue piping will be modified to fit the new boilers with installation following all national codes as well as placing the new units centrally located to allow proper access for maintenance of the equipment. All new piping will be installed using standard construction methods, following the Manufacturers recommended methods and materials as close as possible.
c. Contractor will provide the necessary O&M manuals, startup & training for the 126th Maintenance Staff upon substantial completion of the project. The Contractor will remove and dispose of all existing materials as determined by the COR staff.
d. Rework new piping to complete the existing Building loop and pump loop including new boilers.
e. Reconnect all existing gas piping to the new units and test for leaks.
f. Reconnect all electric, including low voltage to the new units. Provide BMS Gateway to LON when ordering units.
g. Furnish and install CO detector near boiler installation.
h. Fabricate and install new flue piping as needed to retrofit the existing penetrations, or provide for new penetrations as needed. Any abandoned openings will be plugged by the Contractor
Project #: VDYD162003 Page 10 of 19 Project Title: Repair Building 5002 31 May 2017 using approved methods by the COR and all abandoned piping will be removed by the Contractor and properly disposed of.
i. Remove and reconnect all points as necessary to the DDC system and test when complete.
j. Add GFM furnished Glycol to the system after work is completed as needed.
k. All contractor personnel shall be trained and qualified to install the manufacturer’s equipment, and be certified to handle environmental materials as needed.
l. Contractor will provide reasonable access for maintenance of the boilers when installing the new equipment.
CONTRACTOR RESPONSIBILITIES:
A. Construction Conditions: Building 5002 will be occupied during the construction operations.
Therefore, the contractor shall operate in a manner considered safe for an environment inhabited by non-contractor personnel. The government will make all necessary efforts to accommodate the construction work at all times. All equipment and materials owned by the 126th will be removed from the room prior to start up.
B. Scaffolding: The contractor shall provide ladders and scaffolding as necessary to perform the new construction. Scaffolding shall be installed in a safe manner and all above ground work performed in compliance with all OSHA requirements.
C. Protection of Government Property: Care shall be taken in all operations not to damage existing facilities and all other government property. Special care shall be exercised not to damage the surface of existing roof panels. Any contractor caused damage shall be repaired to the satisfaction of the Contracting Officer at no cost to the government.
D. Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this SOW. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means by which the company assures that work complies with the requirement of the contract. The contractor’s Quality Control Plan shall be delivered with the contractor’s proposal and submitted again to the Contracting Officer prior to the issuance of the Notice to Proceed (NTP). After acceptance of the quality control plan, the contractor shall receive the contracting officer’s acceptance in writing of any proposed change to his QC program.
Contractor shall have SOLE responsibility for accuracy of all measurements, estimates of material quantities and sizes, and site conditions that will affect the construction of this project.
Contractor shall furnish and submit the following items:
a. Construction Schedule (Microsoft Project Compatible)
b. Material Submittals (Use Electronic PFOIL 341 Form) – Reference Submittal Register
c. Manufacturer's Information and Shop Drawings

CONSTRUCTION COORDINATION AND PHASING PLAN
A. Contractor shall supply the 126th with a construction schedule and phasing plan of all parking, roadways and apron shoulder areas for approval by the COR prior to beginning of construction.
B. The contractor shall minimize system down time or interruption of normal operations by coordinating work of the various trades to assure efficient and orderly sequence of installation of construction elements.
C. Normal operations of apron, roadways and parking areas will continue throughout the duration of the project. Contractor must coordinate their project schedule with the COR at a minimum of 2 weeks prior to construction in order to minimize the effect on normal operations for the areas shown on the Final Plans.
GENERAL CONTRACTOR INFORMATION
A. Final plan quantities are estimated and form the basis for proposals and the contract amount.
Contractor is responsible for field verifying dimensions and areas prior to project bid.
B. Omissions from the Drawing(s) or Statement of Work (SOW) or the inaccurate description of work details which are manifestly necessary to carry out the intent of the Drawing and SOW, or which are customarily performed, shall not relieve the Contractor from performing such omitted or inaccurately described work details, but they shall be performed as if fully and correctly set forth and described in the Drawing(s) and SOW at no additional cost to the Government.
C. The Contractor is responsible for the means and methods of all products and work. The Government is paying the Contractor in the contract price to use its expertise to ensure the means and methods of all products in the project.
Contractor shall notify the COR at the pre-bid conference (Site Visit) if any discrepancies in the products or work described on the plans exist.
MATERIAL SUBMITTALS
A. General Procedures: Contractor is required to submit all shop drawings, product data, and material submittals to the KO and COR. All hard material submittals shall be sent to the COR and the KO (1 Copy Each). All submittals shall be submitted to the COR/KO within 10 days after issue of the Notice To Proceed (NTP) or as directed by the KO.
B. Electronically deliver all submittals to KO and COR using the following email addresses:

C. Prior to delivery, carefully review all submittals for compliance with Contract Document and for coordination purposes, and sign the submittal form as necessary.
D. Transmit each item listed on the PFOIL Form 341- Schedule of Submittal Materials (PDF Format).
Identify Project, Contractor, subcontractor, and major supplier; identify pertinent drawing sheet and detail number, and section number, as appropriate. Identify deviations from contract documents.

F. After review by the A/E, COR and the KO, the contractor shall revise and resubmit within 14 days.
As required, identify changes made since previous submittal.
G. Distribute copies of reviewed submittals to concerned persons. Instruct recipients to promptly report any inability to comply with provisions.
H. Coordinate each submittal with requirements of work and contract documents.
I. Contractor’s responsibility for error and omissions in submittals is not relieved by Government’s review of submittals.
J. Contractor’s responsibility for deviations in submittals from contract document requirements is not relieved by KO’s review of submittals, unless KO gives written acceptance of specific deviations.
K. Notify KO in writing at time of submission of deviations in submittals from contract document requirements.
L. Do not begin any work that requires submittals without KO’s stamp and signature indicating approval.
M. After KO’s review, make response required by KO’s stamp and distribute copies.
PRODUCT DATA
A. Collect product data into a single submittal for each element or system. Mark each copy to identify applicable products, models, options, and other data; supplement manufacturers’ standard data to provide information unique to work. Submit the number of copies Contractor requires, plus two copies, which will be retained by KO and COR. When product data includes information on several products, mark copies to indicate the following information:
1. Manufacturer's printed recommendations.
2. Compliance with recognized trade association standards.
3. Compliance with recognized testing agency standards.
4. Application of testing agency labels and seals.
5. Notation of dimensions verified by field measurement.
6. Notation of coordination requirements.
B. Submit manufacturer’s printed instructions for delivery, storage, assembly, installation, and start-up, adjusting, and finishing, for product data when required.
C. It is the intent of the KO to give the Contractor the widest possible latitude in the use of similar products of equal quality. When a brand name description or manufacturer is identified in the SOW, such identification is intended to be descriptive, but not restrictive, and is to indicate the quality and characteristics of the product that is satisfactory. The manufacturers that are listed are believed to provide acceptable products. This list is not all-inclusive and the Government is not warranting exclusive use of these products. Contractor shall verify product is fit for purpose and complies.
D. In lieu of the label, the Contractor may submit written certification from any nationally recognized testing agency, adequately equipped and competent to perform such services.
E. The contractor shall maintain and provide quality control over suppliers, manufacturers, products, services, site conditions, and workmanship.
F. Work is to be performed by qualified persons. Comply with industry standards except when more restrictive tolerances or specified requirements indicate more rigid standards or more precise workmanship.

G. Comply with quality control, references, specifications, and manufacturer’s data. Products containing asbestos are prohibited on this project. Use only asbestos-free products that are environmentally acceptable.
H. Use products with personal protection. User must read container label and material safety data sheets prior to use.
I. Required components to be supplied by Contractor in quantity shall be the same, and shall be interchangeable.
J. Materials used in this contract shall be, to the greatest extent practicable and consistent with financial prudence, made of recycled materials or with materials that are recyclable.
TRANSPORTATION, STORAGE AND HANDLING
A. Transport products by methods to avoid product damage; deliver in undamaged condition in manufacturer’s unopened containers or packaging, dry.
B. Provide equipment and personnel to handle products by methods to prevent soiling or damage.
C. Promptly inspect shipments to assure that products comply with requirements, quantities are correct, and products are undamaged.
D. Store products in accordance with manufacturer’s instructions. Maintain products within temperature and humidity ranges required by manufacturer’s instructions.
E. Arrange storage to provide access for inspection. Periodically inspect to assure products are undamaged, and are maintained under required conditions.
F. Do not re-use, recycle or dispose of material manufacturers product containers except in accordance with all applicable regulations. The user of manufactured products is responsible for proper use and disposal of product containers.
PRE-FINAL INSPECTION
A pre-final inspection of the project shall be performed to compile a list of incomplete and nonconforming work and, if necessary, to help expedite project completion. The Contractor and the COR shall conduct the Pre-final inspection. This inspection shall be performed at a time agreed upon by the COR, the KO and the Contractor prior to completion of the work. Any items found to be deficient shall be addressed in the “Punch List” and completed to the satisfaction of the COR and the KO.
FINAL INSPECTION AND ACCEPTANCE
A. At 100% completion, the Contractor shall notify the KO that all work is complete and ready for inspection. The COR shall perform a walk through inspection of the project. A list of incomplete and nonconforming work shall be created (if required). If a list is not created, the COR shall notify the KO, in writing, of final inspection and acceptance by the Government. The KO will then notify the Contractor of such inspection and acceptance. This inspection shall be performed NLT 7 days after work has been deemed complete by the contractor.
B. Cleaning shall be to the satisfaction of the COR and the KO. Any areas of work or adjacent areas disturbed shall be restored to original conditions.
Execute prior to notifying Government for a final inspection
Clean surfaces exposed to view; remove temporary labels, stains and foreign substances
Clean site
Dispose of all waste, rubbish, and surplus materials off base appropriately
FINAL PAYMENT

The KO shall not process final payment until all required documents are received and final inspection and acceptance is completed by the COR.
CLOSEOUT PROCEDURES
When Contractor has determined that the work has reached final completion, (all Incomplete and Nonconforming Work list items resolved), the contractor shall submit written certification that ontract documents have been reviewed, work has been inspected, and that work is complete in accordance with contract documents and ready for KO’s inspection.
PROJECT RECORD DOCUMENTS
Store complete set of documents on site separate from those used for construction.
Keep documents current and available for inspection.
SAFETY
Base and Safety Regulations:
A. These requirements are in addition to those referenced during the pre-construction conference.
B. The Contractor shall comply with all applicable provisions of Air Force regulations and directives pertaining to security, safety, fire, traffic, and personnel clearances insofar as they pertain to the Contractor’s activities on Scott AFB, IL.
C. The Contractor shall be responsible for providing, placing, and removing all barricades and safety devices during his construction activities.
D. In addition, the Contractor shall:
E. Caution vehicle operators to avoid intrusion on the runway or flight line.
F. Yield to all emergency vehicles including security police, fire department, ambulance, and ambulance buses when their red lights and/or sirens are operating.
G. Coordinate movement of heavy oversized equipment such as cranes, extended flatbed trailers and large earth moving vehicles with the base security police and the COR.
H. Provide personnel to direct traffic when Contractor operated vehicles block one or both lanes of a two-lane road.
I. If a mishap involves injury or death to Contractor personnel and/or damage to Air Force property as a result of Contractor operation, the Contractor shall notify the Contracting Officer (KO) and COR.
The COR will contact the Wing Safety Office on normal workdays or the Security Police at any other time. Any safety related problems encountered during the contract period should be reported to the COR who will advise Wing Safety. In case of life threatening emergency, dial 911 to reach the Security Police, Fire Department, or ambulance. Emergency treatment for serious or life-threatening injuries is available on Scott AFB, IL.
J. The Contractor shall ensure compliance with contract and OSHA provisions.
K. Contractor will ensure all application, material handling, and associated equipment shall conform to and be operated in conformance with OSHA safety requirements.
L. Site security to Scott AFB IL, Base facilities must be maintained at all times. FAA requirements state that approved temporary access openings are allowed to be opened only during the intended use times and closed during non-use. This will require the Contractor to place a temporary fence or roll back the removed fence netting across the access openings to the area as shown in the plans. The access opening will be closed during times when construction activities are not occurring and/or when access to this area is not required.

M. United States Air Force Security has jurisdiction over all activities on or relevant to Scott AFB, IL. The Contractor, Subcontractors, Suppliers, and any other persons and organizations performing or furnishing any of the Work shall fully comply with any and all lawful orders of Air Force Security Staff.
N. Contractor will comply with security requirements established at the preconstruction conference.
All employees will complete a base access form and will have a current government issued photo identification.
O. All contractor material deliveries shall only be via the Mascoutah Truck Gate located off Illinois Highway 161. All Contractor personnel access to the site shall only be via the Main Gate. Entrance to the Site shall be restricted and require issuance of a security pass or badge subject to the requirements of Base Security. Contractor shall coordinate with the KO to setup access passes for all necessary workers.
P. The following is information pertaining to law enforcement and security policies in effect at Scott AFB. Questions or comments that concern the Security Police, must be directed to the COR.
Speed Limits:
A. Speeding is not tolerated on Scott AFB. Speed limits are posted thought the Base.
B. Speed limit while passing troops is 5 mph.
Reserved/Prohibited Parking Areas:
Signs marking reserved parking list the person or category for which the space is reserved, or stating which type of decal is required to park in the space.
Sign or yellow paint on the curb marks prohibited parking.
Parking on the grass or seeded areas is strictly prohibited unless it is required for contract completion.
Crosswalks: Motorists are required to yield to anyone in or about to enter crosswalk.
Cell Phones: Hands free devices are the only communication device authorized to be used when driving on Scott AFB, Illinois.
Joggers/Cyclists: Motorists are cautioned to look out for high numbers of people engaged in exercise, especially during the middle of the day.
Traffic Citations: Violation of base traffic rules could affect Contractor or individual access to the base. All provisions of the Illinois Vehicle Code and Air Force Regulations are enforceable for all drivers on Scott AFB, Illinois. Actions taken as result of traffic citations:
1. Notification to employer with required actions.
2. Loss of base driving privileges.
3. Fines.
4. Ban from base.
Weapons: No firearms, plastic explosives, fireworks, or any object which fires a projectile or explosive can be brought on base (except approved construction equipment). Offenses of this nature will be referred to the St. Clair County Sheriffs Office, the U.S. Magistrates Court, or the Bureau of Alcohol, Tobacco and Firearms.
Gate Closures:

A. Anytime a security system is activated or a hostile act is committed on base, security procedures call for access gates to be closed until the “all-clear” is given.
B. Delays will occur when attempting to enter or depart the base during such an incident.
C. The contractor will not be compensated for such delays.
D. Controlled/Restricted Areas: Areas posted as Controlled or Restricted Areas (including the flight line, taxiways and ramps) are clearly marked with signs and constitute legal “No Trespassing” zones.
Entry into these areas is strictly prohibited unless specific written authority is granted. Persons inadvertently entering a Controlled Restricted area will be apprehended by security personnel and treated as hostile until proper identity can be confirmed. Personnel should cooperate with security personnel in these types of situations since security personnel will continue to elevate force with resistance received.
E. Required Coordination: If required to block off a street or portion of a street, before doing so, and with as much advance notice as possible, the Contractor shall coordinate with the Security Police Operations through the COR.
Base Entry:
A. Check with Guard posted 24 hours a day, identify Company, and reason to be on Base.
B. Current vehicle registration, valid driver's license and vehicle insurance are mandatory when driving on IL Air National Guard property.
C. The Contractor shall ensure all pilferable items are secured when not in use. Personal vehicles shall be locked when working on site. Tool boxes left on site will be secured after work hours.
D. For further specifics or guidance contact the COR.
FIRE PREVENTION STANDARDS
A. The following is information pertaining to fire prevention standards at Scott AFB IL ANG. These standards apply to all new construction and reconstruction, demolition, rehabilitation, alteration and modification. To report a fire, dial 911 or 618-256-5130.
B. Serious fires can occur due to accumulation of combustible materials or debris and the presence of ignition sources. Basic safeguards against construction fire hazards must be provided for during project planning. These standards outline measures which, with some pre-planning, will prevent fires or, at least minimize damage if a fire does occur.
Responsibilities:
A. A base fire department representative will attend pre-construction conference to brief the
Contractor on pertinent fire regulations and to provide guidance for fire-safe operations during contract performance. Fire Prevention guidance appropriate for each type contract will be furnished the Contractor for briefing his personnel.
B. The Contractor will furnish via the COR the fire department Fire Prevention Section and Security Police with the name(s) of responsible individuals, who, in case of a fire after normal work hours, can be notified of an incident.
C. The Contractor shall be responsible for assuring proper availability, operation and location of fire protection equipment, general supervision of safeguards and location of salamanders or portable heating equipment, and establishment and maintenance of safe cutting and welding operations.
D. The Contractor will maintain fire extinguisher within easy access whenever power tools, roofing kettles, fuels, solvents, torches, and open flames are being used.
Water Supply:

A. The Contractor will ensure an adequate water supply for fire protection is available whenever combustible materials accumulate.
B. Temporary connection to a main or fire hydrant by a contractor will first be coordinated through the COR with a fire department representative to ensure availability of water supply for fire protection is not jeopardized.
C. Any connection to the public potable water supply must comply with Illinois Plumbing Code using an approved cross-connection device and be coordinated with the Base Civil Engineer (BCE) through the COR.
Access for Firefighters:
A. Access for use of firefighting vehicles will be provided to the immediate job site at the start of construction and maintained until all construction is complete. Vehicle parking control measures will be established that will allow firefighters access to the construction site.
B. Free access from the street to fire hydrants and to outside connections for standpipes, sprinklers, or other fire extinguishing equipment, whether permanent or temporary, shall be provided and maintained at all times. No material or construction will be placed within 10 feet of hydrants or connections, or between hydrants and the centerline of the street.
C. Fire hydrants in excavations will be rigidly braced and protected from freezing.
D. Scaffolding, materials, or equipment will not block fire escape routes during performance of projects in occupied buildings.
Hazard Notification:
A. In accordance with AFM 91-201, Explosives Safety Manual, if storing more than 1,000 nail gun cartridges, approval must be requested from Wing Safety through the COR.
B. The Contractor will notify the Fire Prevention representative, through the COR, whenever an existing fire detection, alarm, or extinguishing system must be disconnected, relocated, or extended. Fire Protection system components will be handled carefully to assure reliability when the system is restored to service.
C. Any fire hazard or condition that is not specifically covered by these specifications that is recognized by anyone will be immediately brought to the attention of supervisors or anyone having authority to stop the operation.
D. For any further fire prevention guidance or specifics, contact the COR.
126 ARW STANDARD WORKDAY/WEEK
A. Government will issue base work schedule at time of contract issue.
B. When the Contractor desires to work at other than the aforementioned times, he shall submit a written request to the Contracting Officer via the COR for approval, and written approval thereof shall be required prior to proceeding with work.
C. The request shall be submitted at least five workdays prior to the proposed performance of work.
D. The request shall indicate the day/days, time/amount of work, and reason why permission should be granted to perform work during nonstandard hours.
E. Approval for work to be performed during nonstandard time will be given only in cases of necessity.
PERMITS
A. The Contractor shall obtain, through the COR, an AF Form 103 Base Civil Engineer Work Clearance Request (Dig Permit) from the Base Civil Engineer. The purpose of the permit is to locate, prior to excavation, all known underground utilities.

B. The Contractor shall clearly mark the location of the excavation or ground penetration with white, water-soluble, spray paint, or white utility marking flags prior to requesting the dig permit from the COR. The Contractor shall insure the markings are maintained until all responsible agencies/ offices clear the area for each utility.
C. The Contractor shall coordinate the Dig Permit request with the COR, a minimum of ten (10) working days prior to performing any excavation or ground penetration.
