ADDENDUM TO FAR 52.212-1 QUOTE PREPARATION INSTRUCTIONS.

Basis for Award: The Government will award to the lowest priced, responsible quoter, whose quote conforms to the requirement of the solicitation and most advantageous to the Government, price and other factors considered. Award will be made to the vendor that has the necessary licenses to perform work in Fort Lee, VA, meets or exceeds the acceptability standards for past performance, and proposes the lowest evaluated price. Government intends to make the award without discussions; however, reserves the right to hold discussion if required.

1. RFQ SUBMISSION - Send quotes via email to Contract Specialist Myrisha Mullen at Myrisha.l.mullen2.civ@army.mil. No later than the proposal due date 28 Feb 2022 at 3:00HRS ET.

2. QUESTIONS - Submit any questions or comments via email to Contract Specialist Myrisha Mullen at Myrisha.l.mullen2.civ@army.mil - QUESTIONS” in the subject line of the e-mail. Please identify the page and section of the solicitation to which your question refers. Questions are to be submitted by 18 Feb 2022.

3. FORMAT AND CONTENT - Use Times New Roman 12 point font, single spaced type with one inch margins. There is a 20 page limit.

Include the following information in order to verify credentials:

VOLUME I - BUSINESS PROPOSAL

a. Cover Letter - Restrict the following information to the cover page or first page of the submitted proposal. Company logo can be included in the header of each page as necessary, if applicable.

• Company name

• Company logo or other identify graphic

• Company mailing address

• CAGE code and DUNS number

• Name and contact information for company official with authority to obligate the vendor.

• Solicitation Number •Must state the validity of the proposal from the time of the due date, preferably 90 days.

b. Representations and Certifications, FAR 52.212-3 - Offerors shall submit one completed copy of all representations and certifications. If the Offeror has completed the Online Representations and Certifications at the System for Award Management (SAM) https://www.sam.gov , submit an electronic printout of the Representations and Certifications.

VOLUME II - TECHNICAL PROPOSAL

Mission Capability Subfactor 1: Quality Control Plan (QCP) The offeror shall describe its compliance with sections Para: 3.2-4.0 of the PWS. The description shall identify the offeror's approach to providing preventative maintenance to the systems in the referenced paragraphs.

Subfactor 2: Project Manager The offeror shall describe its compliance with Section Para: 2.4.3 and 2.4.3.1 of the PWS. The description shall identify the offeror's management approach of recruiting personnel that possess the skills, knowledge, training, certifications and licenses associated with the required positions.

Subfactor 3: Special Qualifications The offeror shall describe its compliance with Section Para: 2.17.1 and ANNEX H Para 19.0 of the PWS. The offeror shall furnish in writing to the Contracting Officer evidence of qualifications for each of his maintenance and service employees prior to commencing work under this contract. All employees assigned by the Contractor to perform the work under this contract shall be physically able to do the assigned work and have a minimum of one (1) year of experience.

VOLUME III - PAST PERFORMANCE

a) Past Performance – The offeror shall provide a list with three (3) references of similar projects completed or ongoing within the last three years. The offeror shall include:

(1) The contract number;

(2) A description of the work performed;

(3) The name, current phone number, and current e-mail address with the customer who can speak to customer satisfaction, timeliness of the work; and quality

(4) The date the work was started and completed;

(5) The total dollar value of the work.

VOLUME IV - PRICE

a) Price - The offeror must provide US dollar pricing for all CLINs.

b) FAR 52.217-8 allows the Government to exercise the Option to Extend Services for up to 6 months beyond the current performance period if needed. For evaluation purposes, the Contracting Officer will use the monthly prices submitted in the last option year to calculate the monthly amount for the extension of services.
