Annex H (CLS 411 Facilities Maintenance Vertical) to OPORD 18-011: IMCOM Enterprise Base Operations (BASOPS) Performance Work Statement (PWS) Templates (U)
1.0 General Information.
| 1.1 | Overview. The Contractor shall inspect, repair and maintain all existing buildings elevators and their components listed in Facilities List (TE 6, Real Property per Category Code (CATCODE) Unit of Measure). |
| a. | The following paragraphs describe all facets involved in the management of building and structure maintenance; they include terms used to classify work; provide information on the level of effort that will be required at Fort Lee in terms of workload and trade distributions; and describe the way in which work will be received, managed, and documented. The contractor is responsible for the maintenance and repair of all buildings elevators (TE 6, Real Property per Category Code (CATCODE) Unit of Measure). Work will include elevators and other trades that relate to the maintenance and repair of real property facilities. The Contractor shall perform both PMOs Preventive Maintenance and DMOs on all real property listed in TE 6. |
| b. | There is a wide variety of facilities at Fort Lee with a wide variety of installed equipment. Although other sections of this PWS detail the requirements for some of these types of equipment, all of this real property installed equipment for buildings elevators is the responsibility of the contractor. |
| 1.1.1 | Diagnosis and Analysis. The Contractor shall provide follow-up analysis and diagnosis of equipment failure as requested or shown in section 1.1.1 (b) |
| a. | The primary aim is to identify the factors that contributed to and/or resulted in the failure of maintenance, service, repair and/or operation of real property, infrastructure, and systems at Fort Lee, and to identify and implement actions to prevent repeated future failure due to the same failure reasons. The contractor shall: |
| i. | Provide a detailed description and explanation of the failure |
| ii. | Identify the root cause or causes of the problem |
| iii. | Provide possible actions and solutions to prevent future occurrences, an explanation of the pros and cons of these actions, a cost analysis of these actions, and the contractor’s recommended action |
| iv. | The selection and implementation of the appropriate action |
| v. | Review, documentation, and analysis of the outcome |
| vi. | Change in procedures, training, PWS, etc. as needed |
| b. | The Contractor shall provide additional analysis or diagnosis when: |
| i. | Directed by the Contracting Officer (KO), Contracting Officer Representative (COR). |
| ii. | After catastrophic, unusual, or unforeseen events that result in adverse maintenance, repair, and service requirements |
1.2 Components. The Contractor shall perform preventive maintenance (PM) and repairs. Facilities and components covered under this Common Level of Support (CLS) include, but are not limited to, the following:
a. Dining Facilities
b. Administrative buildings
c. Training facilities
d. Childcare facilities
e. Medical facilities and clinics
f. Schools, chapels, and recreational facilities
g. Childcare facilities
h. Installed building equipment
1.3 Work Management and Control.
1.3.1 Reporting Requirements. The Contractor shall furnish, with each invoice submitted, reports on the condition of the equipment monthly and provide annual reports. Reports shall be submitted electronically to KO/COR within five (5) business days after the end of the month in which the work was performed. Each report shall include a resume of the services performed by the Contractor, parts replaced, and deficiencies noted that will require future corrections and recommendations relative to methods for improving the operational effectiveness of the equipment. Specific equipment items will be clearly identified in the report. The format of the report is subject to approval of the Contracting Officer. On a monthly basis, the Contractor shall provide the monthly/annual maintenance checklist (TE 7) for each elevator serviced/maintained during the past month. This report shall include any deficiencies noted and shall be submitted electronically to the Contracting Officer Representative (COR). The Contractor shall reproduce as many copies of Technical Exhibit 7 as necessary at his own expense, for use during performance of this contract requirement.
All repairs and parts that are incidental to normal continuous and safe operation of the listed systems are covered by this contract.
1.3.1.1 Service Plan.
a. Within 30 calendar days of the start of the contract, the Contractor shall submit electronically to the KO/COR a Service Plan organized by Functional Area. The Service Plan shall describe all preventive maintenance services to be performed and their frequencies as reflected by this Contract, standard operating procedures/orders, and proposed staffing. As part of the Service Plan, the Contractor shall submit detailed management plans for high risk utility systems (elevators). These management plans shall include, at a minimum, specific maintenance tasks and frequencies, specific instructions for accomplishment of tasks, required special tools and equipment, personnel requirements, safety requirements, emergency procedures, personnel training requirements, and documentation procedures. The Contractor shall submit the plan annually for Contracting Officer approval.

2.0 Description of Services. The Contractor shall develop and implement (after KO/COR approval) a Comprehensive PM and Repair Program. PM consists primarily of inspection, cleaning, lubrication, adjustment, calibration, and minor part and component replacement on building elevators, its component equipment included at facilities which are listed in TE 6 Real Property per Category Code (CATCODE) Unit of Measure IAW the PM tasks. The Contractor shall submit the Comprehensive PM and Repair Program for this service within 30 days after receipt of approval to start. Work that is outside the scope of a PMO shall be accomplished through a DMO.

2.25 Elevator Maintenance. The contractor shall inspect, test, repair and maintain all elevators, stairwell lifts and escalator at the facilities on Ft. Lee, Virginia. A list of facilities with elevators and escalator is at TE 6, List of Facilities with Elevators. The Contractor shall coordinate directly with the COR for scheduling of all work and access to the buildings and elevators.
2.25.1 Elevator Inspection and Certification. Inspection and testing shall be performed with a Contractor-provided certified inspector. Inspection and testing shall be IAW the following:
a. State of Virginia Department of Licensing and Regulatory Affairs for Elevators
b. ASME A17.1, A17.2, A17.3, (State of Virginia)
c. NFPA standards
d. National Electrical Codes (NEC)
e. Certificate of Elevator Inspection GSA Form 55 and Acknowledgment of Elevator Inspection Certificate GSA Form 55A or a form approved by KO/COR (TE 5), Certificate of Elevator Inspection GSA Form 55 and 55-A)
The Contractor shall obtain an annual State of Virginia elevator certificate of operation. The Contractor shall post a notice inside each elevator noting the date of the completed annual certification and note that certificates are on file and available upon request. The Contractor shall create a schedule of inspection, testing, maintenance, repair and certification within 30 days of the contract start date and submit to the KO/COR/ACOR for approval. The Contractor shall coordinate any scheduled shutdown with the affected building occupants after written approval is provided by the KO/COR/ACOR. In the event that an unscheduled shutdown of an elevator occurs, the Contractor shall notify the KO/COR/ACOR within two (2) hours.
2.25.1.2 For any elevators located in Virginia, licensing and regulatory requirements apply.
2.25.2 Out of Service Time. Elevators shall not be out of service for more than 24 hours at any one time. When parts are ordered, the KO/COR will be notified on the status and due delivery date. If the Contractor knows that the elevator is going to be down more than seven calendar days, the KO/COR shall be informed in writing/email detailing the specific problem and indicating why the elevator will be down for more than seven days. The Contractor shall post a Status of Repairs on the elevator door indicating the “repair by” date.
Emergency Service Calls. In the case of an elevator being shut down between scheduled maintenance or if an emergency occurs, the Contractor shall respond to a service call from the Government within four (4) hours. The Government reserves the right to contract with other sources for services whenever the above time limit is exceeded. The Contractor shall provide emergency service after normal working hours seven (7) days a week including holidays. Calls will be made by the COR or the Fort Lee Designated Call Center for service required after duty hours. All service calls after normal duty hours and on weekends and holidays are Emergency Service Calls. The Contractor shall furnish the Contracting Officer and the COR with the names and telephone numbers of qualified maintenance personnel and company officials who can be contacted to respond to an emergency after normal duty hours. When called in under an emergency service call, the Contractor shall remain on site until the emergency has ended. Any repairs required to return the elevator to service after the emergency has ended shall be completed within five (5) working days. If the elevator cannot be returned to service, a written report/proposal shall be submitted to the COR by 11:00 AM the next working day describing the corrective action required to return the elevator to service.
2.25.3 Preventive Maintenance Requirements and Scheduled Maintenance Requirements. Includes monthly preventative maintenance; repairs; parts and supplies; inspections and reports. Monthly preventative maintenance includes but is not limited to the following: lubricate equipment, adjust parts and components, re-lamp signals, and examine all elevator equipment for optimum operation. All maintenance and work shall be performed in accordance with industry standards and equipment manufacturer’s specifications. The Contractor shall provide for each elevator; one (1) scheduled monthly Preventive Maintenance (PM)/Inspection per month and one (1) annual PM per year in accordance with (IAW) Technical Exhibit (TE) 7 for each elevator listed on TE 6. Monthly PM’s shall be performed on no more than six (6) elevators during any eight (8) hour period. The Contractor shall ensure an elevator mechanic is on site from 8:00 AM until 4:30 PM Mondays, Tuesdays, Thursdays and Fridays, except on federal holidays, to perform maintenance and respond to customer calls for repairs. All repairs shall be during regulars working hours. The Contractor shall also provide annual inspections in conjunction with their regularly scheduled PM’s on the elevator systems listed in TE 6. This work shall be performed in accordance with the Performance Requirement Summary which summarizes the services, associated standards, and the Acceptable Quality Levels (AQL). Contractor shall perform all repairs and provide all parts that are incidental to normal continuous and safe operation of the listed elevators. Parts and repairs not covered by this contract are the hoist way enclosures, hoist way vents, cab floor covering, main jack casing, cylinder, underground piping and connections. Projected annual workload for these contract requirements are presented in Technical Exhibit 4.
2.25.3.1 Periodic Inspections, Testing, and Reports. The Contractor shall perform annual inspection/testing and make a formal report in accordance with industry standards. Schedule for Annual inspections shall be submitted for approval to the KO/COR within 30 calendar days of the start of the contract. The Contractor shall also fill out DA Form 7485-R and submit the completed form to the KO/COR within 10 calendar days after completion of each Annual test. The Contractor shall perform Annual Testing and Certification of elevators IAW paragraph 2.25.1.
2.25.3.2 General PM Requirements. The following items shall be provided by the contractor to support the maintenance and repair services.
a. The Contractor shall keep each log book in the machine room of each elevator, maintain log book to include records of scheduled maintenance services, annual service, testing, service calls, repairs and parts replaced. Information shall contain description of work, dates, problems, and corrective measures taken, persons requesting services/repairs, time completed, and the name of the mechanics performing the work. The log book shall be hard bound. Existing log book may be continued if the book is in good condition. Retain previous log books and records in a binder or other suitable document container. Log books will become Government property.
Work covering routine cleaning and refinishing of the interior of cars and the exterior of hoist way doors and frames is not included in this contract. However, cleaning of equipment spaces, hoist ways, and daily cleanup of job sites in conjunction with preventative maintenance, inspection services, and repairs is included in this contract. 18.1 Anti-Terrorism (AT) Level I Training.
IAW AR 525-13, Antiterrorism, all Contractor employees, including sub-contractor employees, requiring access to Army installations, facilities, or controlled access areas shall complete Antiterrorism (AT) Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of the requirement into the contract, whichever applies. The Contractor shall submit certificates of completion for each affected contract employee and subcontractor employee to the KO or designated Government Representative/COR within 30 calendar days after completion of training by all employees and subcontractor personnel on a recurring annual basis. 18.2 iWATCH Training.
The Contractor shall brief the local iWATCH program (training standards provided by the Rock Island’s ATO) to all of Contractor’s employees and associated subcontractors. This training is used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the Fort Lee, Police Department and the COR. The Contractor shall complete the training within 30 calendar days after contract award and within 30 calendar days of new employees commencing performance and send to the KO or designated Government representative/COR via both electronic and paper copy within 30 calendar days of completion.
18.3 Operational Security (OPSEC) Awareness Training.
IAW AR 530-1, Operations Security, Contractor employees shall complete Level I OPSEC Awareness training within 30 calendar days after contract award and within five (5) calendar days of new employees commencing performance. Completion certificates will be made available to Government within ten (10) business of completion or upon request from the Government. The Contractor shall inspect, repair, and maintain elevator systems and components associated with facilities listed in real property records as required throughout this contract utilizing properly trained, skilled, and qualified employees or subcontractor personnel. The Contractor shall furnish, in writing, to the Contracting Officer evidence of qualifications for each of his maintenance and service employees prior to commencing work under this contract. Mechanic’s qualifications shall consist of a recognized Commonwealth of Virginia Journeymans Licsense for the elevator industry followed by four (4) years of experience as a repair mechanic with an established company that has repaired and installed equipment of similar type to that covered under this contract. All employees assigned by the Contractor to perform the work under this contract shall be physically able to do the assigned work.
