These representations and certifications shall be completed in the System for Award Management (SAM) database to the maximum extent practicable. If the representations and certifications are completed in SAM, the vendor need only state so in this volume and does not need to submit the representation and/or certification with the quotation.

If any representation or certification cannot be completed in SAM, fill in the provision provided in the solicitation and submit with this volume.

6. Volume II – Technical Approach

a. Technical Approach Narrative. In order to demonstrate their ability to successfully perform the solicitation requirements, vendors shall submit a written narrative, limited to no more than 12 pages, which shall include the following elements:

Detail, as specifically as possible, the actual methodology/approach that will be used to accomplish the PWS requirements

Detail or provide documentation to show compliance with the Special Qualification, detailed in PWS paragraph 1.6.7, to be accredited by one of the six US Regional Accrediting Agencies and able to grant a certificate of completions recognizable by institutions of higher education that grant graduate degrees

Provide a staffing plan, to include proposed staffing levels and labor categories, to demonstrate successful performance of all ten task areas identified within the PWS

7. Volume III – Past Performance

a. Prior Contract Reference Forms. Vendors shall submit no more than three specific relevant contract reference forms (provided as Attachment 3 to the solicitation). Each reference form is limited to five pages and shall meet the following requirements:

Prior Contract References shall be submitted for work performed (in full or in part) within three years from the closing date of the solicitation. If a prior contract reference is submitted that was wholly performed outside of three years from the closing date of the solicitation, the Government will not consider it as part of the evaluation.

Vendors shall provide enough information in the Prior Contract Reference Forms to demonstrate past performance, as a prime or major subcontractor, which is the same or similar in nature, size, and complexity to the services being procured under this solicitation. The forms shall include rationale supporting vendors’ assertion of relevance. Vendors are required to explain what aspects of the past performance is deemed relevant to the proposed effort, and to what aspects of the proposed effort they relate.

If a vendor proposes to subcontract at least 10% of the cost of work for this requirement, the vendor may submit a Prior Contract Reference Form for the subcontractor; however, any forms submitted for a subcontractor will count towards the three form limit.

b. Organization Structure Change History

Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies. In many cases, these changes have taken place between conclusion of a prior contract and this acquisition. To facilitate the Government’s evaluation, include in this volume a "roadmap" describing all such changes in the organization of your company if a Prior Contract Reference Form is being submitted for a company with a name different from that of the vendor or subcontractor. There is no page limitation for this item.

c. List of Past Performance Questionnaire Recipients

For the contracts identified on each Prior Contract Reference Form, the vendor shall forward a copy of the Past Performance Questionnaire (provided as Attachment 4 to the solicitation) to the points of contact responsible for monitoring performance under such contracts. The points of contact will return the questionnaires directly to the Government in accordance with the instructions contained within Attachment 4.

Vendors shall provide a list of Past Performance Questionnaire Recipients, to include points of contact and phone numbers, as part of this volume.

8. Volume IV - Price

Vendors shall:

Fill in the unit price and extended price for each CLIN. All extensions of unit prices will be subject to verification by the Government. In the event of a mathematical discrepancy between the unit price and extended price, the unit price will govern.

Submit prices out to a maximum of two decimal points. If prices are proposed to more than two decimal points, the Government will round the number to two decimal points.

CLIN prices cannot be left blank. If the vendor does not intend to charge for a specific CLIN, indicate such by entering “NSP” (not separately priced) in the extended price of the CLIN.

The Government reserves the right to require the submission of Other Than Certified Cost or Pricing Data as deemed necessary to determine a fair and reasonable price.

b. CLIN Totals Table
