INSTRUCTIONS - PLEASE READ prior to completing the form below. The total prices provided in the "Period of Performance" section of this form will be evaluated to determine the successful offeror.
Quotes that are not provided on this form and per these instructions will be deemed unresponsive and will not be considered.
a. Equipment - in column A, list the equipment that the chemicals will be used for, eg. Cooling Towers, Closed Loops, Steam Boilers, Chillers, etc.). The "Total System Annual Best Estimate" section should be used for your best estimate of treatment costs for the actual equipment listed on the "Facilites and Equipment" tab of this file. The totals in this section will not be evaluated for the purposes of awarding the BPA.
They are for budgeting purposes only.
The totals columns below calculate from the Units field in column O and the Unit Price provided for each period of performance to the left.
b. Chemical Stock Number and Description - list BOTH the manufacturer's stock number AND the type of chemical, or what it does (eg. Biocide, PH adjuster, Cleaner, etc)
c. Unit and Size - this should describe the way that the chemicals are packaged, eg. 55 gal drum, 10 pound bag, etc)
d. # Units Required - this is the number of items in column C that you estimate will be required for treatment/maintenance of 8,000 gallons per period of performance for each type of system, ie. 8,000 gallons cooling tower water, 8,000 gallons boiler water, 8,000 gallons closed loop, etc. THIS NUMBER MAY BE A DECIMAL - for example, if one half of a 55 gallon drum will be required for one year, then the amount in this column would be 0.5. Use numeric values only.
e. Unit Price - this is the price for the item listed in Column C, eg the 55 gallon drum, or 10 pound bag, etc.
f. Total Price - this is a calculated field that will multiply the # of Units required by the Unit Price to provide an annual total.
Repeat steps e and f for each period of performance.
