About this file

This document is an addendum to FAR 52.212-1 providing instructions and a template for offerors to solicit past performance questionnaires from current or former clients during federal contract competitions.

Offerors must prepare and distribute reference questionnaire packages to a maximum of three current or former clients. For government contracts, questionnaires should be sent to the Contracting Officer or Contract Administrator; for commercial references, they should be sent to personnel with equivalent duties. Each client receives only one reference package, though multiple contracts or orders may be listed together. The offeror is responsible for following up with references to ensure timely completion before the deadline specified in the solicitation. Reference materials must be provided in MS Word format upon request. The questionnaire package consists of a cover letter from the Contracting Officer and respondent information rating sheets that include the offeror's name, contract number(s), and other identifying information on every page.

The Performance Assessment Questionnaire template evaluates contractor performance across six key areas: compliance with specification requirements and standards of good workmanship; effectiveness of project management including subcontractor control; timeliness of performance for services and product deliverables; effectiveness in forecasting and controlling estimated costs for cost-reimbursement contracts; commitment to customer satisfaction and business-like concern for customers' interests; and general performance comments. Responses use a four-level confidence rating scale—Substantial Confidence, Satisfactory Confidence, Limited Confidence, and No Confidence—each with defined performance criteria. Rationales are required for each response. The questionnaire includes sections for contract identification, offeror representative information, and respondent identification, with instructions to return completed forms to the identified Contracting Officer within the stated timeframe.
