About this file
This is a Solicitation Survey form for federal contract opportunity W912WJ26QA175. The document serves as a mandatory compliance questionnaire that vendors must complete and submit with their quotes; failure to provide a completed survey will result in the quote being deemed non-responsive.
The survey requires vendors to provide comprehensive company information including SAM CAGE Code, business type classification, and point of contact details. Vendors must substantiate all claims with their SAM registration assertions, with particular attention to employee count discrepancies that could result in removal from consideration. Key requirements include: documentation of years of experience in the relevant services and federal contracting; disclosure of any failed contract completions with explanatory details; and detailed information on key personnel assigned to the project, including minimum and maximum employee counts and whether additional hiring will be necessary. The survey mandates submission of certifications and licenses in specified areas—First Aid/CPR, Competent Person in Fall Protection, Rope Access, applicable trade licenses, Crane Operator/Qualified Riggers, and Boat Operator—with vendors required to either attach current certificates or provide detailed plans for obtaining certifications that include course names, providers, training dates, and course links. For vendors using subcontractors, the survey requires complete subcontractor identification (name, point of contact, and location) with signed teaming arrangement documentation; submissions stating "to be determined" or "information provided upon request" will be deemed non-responsive. Past performance references must include three contract examples completed within the past three years, with Government contracts preferred, and must contain sufficient detail regarding contract value, scope, and verified points of contact to enable Government evaluation. All submissions require certification by an authorized official confirming the accuracy and completeness of statements provided.
