About this file

This is a Past Performance Questionnaire template used by the South Carolina Army National Guard to evaluate contractors' previous performance. The questionnaire is designed to be partially completed by the offeror (Section 1 only) and the remainder filled out by previous customers who must email it directly back to the government.

The form includes detailed rating scales (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Applicable) and evaluation criteria across multiple performance areas including: Program Management, Mobilization, Quality of Service, Schedule, Staffing, and Cost. Evaluators must provide narrative explanations supporting their ratings, particularly for high or low scores. The questionnaire specifically requests information about safety practices, contract compliance, quality control, personnel management, schedule adherence, and cost control. Completed questionnaires are to be returned via email to the designated government points of contact and are considered source selection sensitive information under FAR 3.104.

1. The contractor listed in Section 1 below (to be completed by the Offeror) is being considered in a Source Selection by the South Carolina Army National Guard. You have received this questionnaire because you have been identified as a source of Past Performance Information for that Offeror. One of the considerations in proposal evaluation is the verification of an Offeror’s past performance on contracts or other work efforts, which reflect the Offeror’s ability to successfully perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the Offeror, for the evaluation of the Offeror’s performance on those contracts or work efforts. Any comments you can provide regarding the past performance of the contractor providing this form are greatly appreciated.

2. Please complete the attached questionnaire for the contractor and program described in Section 1 below. Our areas of interest regarding the Offeror are summarized in the enclosed questionnaire. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to Blake Cromer via e-mail at blake.s.cromer.civ@army.mil. Please DO NOT return the completed questionnaire to the contractor. The completed questionnaire will become part of official Source Selection records. We appreciate your assistance.
