Subject: Contract No. W912P9-XX-X-XXXX (SAMPLE) RFP “Maple Island Shoreline Repairs – Floating Plant”

Gentlemen, In accordance with Contract W912P9-XX-X-XXXX we request a proposal for the following task order, as specified below and as shown in the attached Statement of Work (SOW) and Maps.
1. Specific work elements:
- The contractor shall furnish all equipment, materials, labor, and incidentals to provide for Maple Island Shoreline Repairs via Floating Plant as specified and in accordance with the attached Statement of Work and Map.
- Work Items as indicated within the Statement of Work are optional items and all or none of such items may be awarded.
- All work shall be completed within 120 Calendar Days upon issuance
The contractor shall submit a proposal including estimated quantities for labor, equipment, material, mobilization and demobilization. The contractor shall submit separate documentation for all cost associated with mobilization and demobilization, rental of misc equipment, and delivery of misc material line items. Cost shall be broke down in sufficient detail for each specific item to permit analysis of all materials, labor, equipment, subcontract, incidentals, overhead cost, as well as profit associated with the line item to which it relates. Contractor shall submit separate proposal for each item of work and a total cumulative proposal to perform all items.

Statement of Work
Maple Island Shoreline Repair – Floating Plant
All work shall be completed in accordance with the Contract Specifications, Corps of Engineers Manual EM 385-1- 1, Task Order, Drawings, and Statement of Work Contained herein.
1. Location of Work:
- Work shall be completed at multiple locations within the Upper Navigation Territory as indicated herein and within the attach Maps.
2. Period of Performance:
- All work shall be completed within 120 Calendar Days From Task Order issuance.
- See each item of work for additional performance period information and restrictions.
3. Order and Coordination of Work:
- Contractor shall be required to coordinate all activities and schedule with the Authorized Representative of the Contracting Officer.
- Task Order contains multiple items of work. Contractor shall coordinate sequence of operations with the Authorized Representative of the Contracting Officer.
4. Work Requirements:
The contractor shall furnish all equipment, materials, labor, and incidentals to provide for the following items of work. See attached maps for site specific information, area, and location of work. Work Items as indicated herein are optional items and all or none of such items may be awarded.
Item 1 – Maple Island Shoreline Repair:
- The contractor shall provide repairs to the Maple Island shoreline, downstream of the MPLD Overflow Dike as indicated in the attached Map titled, “Maple Island Shoreline Repairs”. Rip Rap shall be delivered and offloaded via floating plant to prevent damage to existing Maple Island Access.
- Repairs shall not take place and equipment shall not hinder recreational activities during Snagging Seasons which are as follows: September 15 - December 15 and March 15 - May 15.
- The contractor shall furnish and install 1000 lb Rip Rap, 400 lb Rip Rap, 3” Rip Rap Bedding, and CA-6 Aggregate. All stone furnished shall be in accordance with the contract specifications.
- Areas marked for repair shall be repaired as follows:
o Shoreline Rip Rap Placement - Construction Limits for Rip Rap Installation shall be as shown in the attached Map titled, “MPLD MO Downstream Shoreline Repairs”, and as staked and marked in the field by the Government indicating perimeter of repair and a final top surface of Rip Rap coverage resulting in a minimum 3:1 Embankment Slope. Existing deteriorated Rip Rap shall be dispersed evenly to develop a uniformed sub-base with a minimum 3:1 Cross Slope on shoreline embankments. The Contractor shall then install a uniform surface of 1,000 lb Rip Rap to a depth of 3 FT across all surface areas as indicated in the attached map. Upon placement of 1,000 lb Rip Rap the contractor shall fill voids within and between 1000lb Rip Rap with 140 lb Rip Rap to eliminate significant holes and tripping hazards. Upon placement of 140 lb Rip Rap the contractor shall cap surface area with a single layer of 3 Inch Rip Rap bedding.
o Trail Repair - Construction Limits for Trail Repair shall be as shown in the attached Map titled, “MPLD MO Downstream Shoreline Repairs” and as staked and marked in the field by the Government. Contractor shall repair all trails to the lines and grades as indicated in the field by placement of 6 inches of 3” Rip Rap Bedding capped with 6 inches of CA-6. Rip Rap Bedding and CA-6 shall be compacted in 6 Inch Lifts utilizing 4 passes with a vibratory roller.
o Dimensions - Dimensions and measurements are approximate and the contractor shall field verify
Shore Line Repair Approximate Area: 4,350SY decreased to 3,899SY Trail Repair Approximate Area: 750 FT Long by 10 FT Wide – NO CHANGE 1,000 lb Rip Rap required for Shoreline Repair: 6,814 TN 400 lb Rip Rap required for Shoreline Repair: 909.64 TN 140 lb Rip Rap required for filling of voids in Shoreline: 2,228.41 TN 3 Inch Rip Rap bedding required for filling of voids in Shoreline: 1,454.23 TN 3 Inch Rip Rap bedding required for trail repairs: 431.98TN CA-6 Required for trail repairs: 267.34 TN
- The contractor shall be responsible for restoring all damages resulting from construction to the existing parking lot to original condition.

MAPLE ISLAND ACCESS AREA
Coordinates. 38°51’55.02” N, 90°09’11.94”W
Directions. Proceed as if to RPO Storage Area, except do not turn right, and continue going straight. The paved road will turn into a gravel road. The access area is at the end of the gravel road.
