Dining Hall Attendant Services QASP
QUALITY ASSURANCE SURVEILLANCE
PLAN
for
DINING HALL ATTENDANT
CLEANING SERVICES
Reviewed: ____________________________ Approved:_________________________
Quality Assurance Evaluator Contracting Officer
185TH Air Refueling Wing Iowa Air National Guard
Sioux City, Iowa
Current Date: 15 April 2019
Section A
PURPOSE
1. This Quality Assurance Surveillance Plan (QASP) has been developed to provide the standard of surveillance for monitoring a Firm-Fixed-Price contract and a systematic approach for conducting the surveillance. It will be used by Contracting Officer Representative (COR) to insure the standards of the contract are being met.
2. Where appropriate, methods for administering and evaluating services not included in the Service Delivery
Summary are to be developed by the COR.
a. The government retains the right to inspect required tasks not shown on the Service Delivery Summary
(SDS). Inspection of these services will be in the same general manner as surveillance checklist items. The results of these inspections are documented and if necessary, provided to the Functional Commander/Functional Director
(FC/FD) and Contracting Officer (CO) for action.
b. The following non-SDS items may be observed periodically during the contract period.
NONE
3. Any nonconformance with contract requirements is a “defect”. The term “defective” is used in reference to a service output that does not meet the standard of performance specified in the contract for that service.
4. The QASP provides a systematic method to evaluate the services the contractor is required to furnish and not the details of how the contractor accomplishes the work. For all services, the plan utilizes Surveillance Inspections.
This surveillance adequately assures the government that the contractor’s performance is acceptable.
5. The QASP is based on the premise that the contractor and not the government, is responsible for management and quality control actions to meet the terms of the contract. The performance requirements recognize that the contractor is not a perfect manager and that unforeseen and uncontrollable problems do occur. Good management and use of an excellent quality control plan will allow the contractor to operate within specified performance requirements. COR’s are to be objective, fair, and consistent in evaluating contractor performance against the standards.
Section B
PROCEDURES
1. QUALITY ASSURANCE SCHEDULE
1.1. The COR will develop a checklist (see Section F) to use when conducting surveillance of the contractor’s performance as identified in the QASP.
2. ACTUAL SURVEILLANCE
2.1. Actual surveillance will be conducted as identified in the contract.
2.2. It is essential that the COR determine when periodic inspections will be accomplished in order to have an effective quality assurance surveillance system.
2.3. The COR will inspect task performance by either watching actual performance, physically checking an attribute of the completed task, checking a management information report, or otherwise inspecting the task or its results to determine whether or not the performance meets the standards contained in the contract.
2.4. All surveillance observations/inspections made by the COR will be recorded on the 185th Daily Dining Hall
Attendant Inspection Checklist (see Section F) . The completed form becomes the official record of the contractor’s performance.
2.5. When a surveillance observation results in unacceptable service, the COR will record the information on the checklist and request the contractor to initial. If the service can be re-performed, request the contractor to re-accomplish the task or service. If the service cannot be re-performed, the deficiency will be a defect. The specific reason for unacceptable performance will be recorded on the checklist.
2.6. During the performance period, the COR may receive customer complaints about the quality of service, or may observe unacceptable performance by the contractor. These complaints and observations will be noted and should reinforce the accuracy of the sample results. Surveillance may, or may not, be counted as a defect. During a surveillance period, only one surveillance method may be used to cause less than maximum payment for the listed service.
3. The COR is required to accept contractor services and determine payment due. At the completion of each contract payment period (usually monthly), the COR must certify the services actually received under the contract by accomplishing the Acceptance in Wide Area WorkFlow (WAWF).
Section C
METHOD OF SURVEILLANCE
1. Method of surveillance will be monthly inspection and surveillance.
2. Monthly Inspection Checklists may be annotated at any time and at any location during the performance period, but shall be completed and signed at the end of each month.
3. If the COR does not evaluate the performance, task, report, area, etc., the item(s) not evaluated is/are deemed to have been acceptable.
4. Surveillance may also be conducted by Customer Surveys.
Section D
UNACCEPTABLE PERFORMANCE
1. When performance is determined unacceptable, the COR must inform the contractor’s on-site representative that performance is unacceptable and why, and request his or her initials and date on the surveillance form. By initialing and dating the form, the contractor’s on-site representative is acknowledging that he or she has been told of the unacceptable performance. He or she is not necessarily agreeing that the performance is unacceptable. If the contractor wants to dispute the results of the surveillance, the COR must refer them to the Contracting Officer for resolution of the matter.
2. The COR will notify the Contracting Officer of less than acceptable performance. Re-performance, where practical, will be required for unacceptable performance. If any of the services do not conform to contract requirements, the Government may require the Contractor to perform the services again in conformity with contract requirements, at no increase in contract price. When the defect(s) in services cannot be corrected, the Government may:
2.1. Require the Contractor to take necessary action to ensure future performance conforms to contract requirements.
2.2. Reduce the contract price to reflect the reduced value of the services performed.
3. If the Contractor fails to promptly re-perform the services, or take the necessary action to ensure future performance in conformity with contract requirements, the Government may:
3.1. By contract or otherwise, perform the services and charge to the Contractor any cost incurred by the
Government that is directly related to the performance of such service or:
Request for Revised Quality Control Plan
Past Performance Documentation
Inspection of Services clause remedies
Cure Notice
Show Cause Notice
Termination for Default
Section E
Surveillance for Dining Hall Attendant
