1.3.1.3 Phase 3
The limits of construction are from Station 2696+00 to Station 2725+00 to allow for construction and removal of the construction platform. Cutoff wall construction is from Station 2697+00 to Station 2724+00.
1.3.1.4 Phase 4
The limits of construction are from Station 2566+00 to Station 2571+00 to allow for construction and removal of the construction platform. There is an adjacent Construction Contractor whose work begins at Station 2567+00 and proceeds north. This contract has been structured with Phasing such that adjacent work efforts (by others) are complete minimizing the potential for overlapping work efforts. Cutoff wall construction is from Station 2567+00 to Station 2570+00.
1.3.2 Order of Work
The Contractor will receive a separate Notice to Proceed (NTP) for each phase of work. The first NTP will be to construct a 500 linear feet cutoff wall demonstration section as shown (Phase 1). Once the demonstration section is accepted by the Government, NTP for Phase 2 will be issued. Subsequent NTPs may be issued prior to completion of Phase 2.
Upon completion of all work within a phase (including sod establishment), the Government may accept the completed work and the limits of the accepted phase shall be turned over to the Government.
1.3.3 Location and Access
The project area is located on the south side of Lake Okeechobee between South Bay and Lake Harbor in Palm Beach County, Florida.
The Contractor's entrances are shown on the drawings and are shared by others. The Contractor shall comply with all language contained within the Rights of Entries obtained by the United States Army Corps of Engineers (USACE). The Contractor is responsible for obtaining and complying with all Federal, State, County, and any other jurisdictional agency, requirements for permits, criteria, and use. The South Bay Boat Ramp existing access ramps as shown shall not be used for construction.
For construction access, construct access ramps and turn-around to accommodate construction equipment and construction means and methods.
While working in or driving through the limits of the South Bay Boat Ramp (Station 2637+00 to Station 2647+00), the Contractor shall use flagmen to direct and coordinate all traffic movement within the limits of the South Bay Boat Ramp. The South Bay Boat ramp facility will be temporarily closed when cutoff wall is being installed between said station limits.
For access and traffic control along the crest of HHD, the Contractor may elect to design and construct temporary turnouts every 1000 feet along the embankment. If so elected, the Contractor must provide a design and submit a Temporary Turnout Plan for approval by the government prior to commencement of work. The plan shall comply with the requirement of Haul Road prescribed in Specification Section 01 50 02 Temporary Construction Facilities.
The Contractor may utilize area within the limits of construction as laydown areas. The limits of construction are shown. If the lakeside slope of the embankment is used, the lakeside slope will be subject to lake-stage, lake-surge and lake-waves which may inundate the lakeside
SECTION 01 11 00 Page 3 Amendment 0004 slope, see specification section 00 33 50 .

1.7.1 Established Project Control
The Government has established monuments along the work site(s). Each monument is represented in the contract drawings by a Designation and in most cases, an alphanumeric Permanent Identifier (PID). Information concerning the location and elevations can be found within the Survey Notes on the plan sheets. The Survey Notes for each included field survey will cite a particular survey number, and specify the control used to conduct that survey. Additional information is included in the Survey Notes that must be duplicated if an attempt to recreate the subject survey is desired.
See Section 00 31 21 CONTROL MONUMENT DESCRIPTIONS.
1.7.2 Layout
From the monuments, control data and elevations established by the Government, the Contractor shall complete the layout of the work and shall be responsible for all measurements that may be required for the execution of the work to the location and limit marks prescribed in the specifications or on the contract drawings, subject to such modifications as the Contracting Officer may require to meet changed conditions or as a result of necessary modifications to the contract work.
1.7.3 Survey
The Contractor shall furnish, at his own expense, such stakes, templates, platforms, equipment, tools and material, and all labor as may be required in laying out any part of the work from the monuments, control data and elevations established by the Government. It shall be the responsibility of the Contractor to maintain and preserve all stakes and other marks established by the Contracting Officer until authorized to remove them, and if such marks are destroyed by the Contractor or through his negligence, prior to their authorized removal, they may be replaced by the Contracting Officer, at his discretion, and the expense of replacement will be deducted from any amounts due or to become due the Contractor.
The Contracting Officer may require that work be suspended at any time when location and limit marks established by the Contractor are not reasonably adequate to permit checking of the work.
1.8 DAMAGE TO WORK
The responsibility for damage to any part of the permanent work shall be as set forth in Clause PERMITS AND RESPONSIBILITIES of Section 00700 CONTRACT CLAUSES in Volume 1. However, if, in the judgment of the Contracting Officer, any part of the permanent work performed by the Contractor is damaged by flood, earthquake, hurricane, or tornado, which damage is not due to the failure of the Contractor to take reasonable precautions or to exercise sound engineering and construction practices in the conduct of the work, the Contractor will make the repairs as ordered by the Contracting Officer and full compensation for such repairs will be made at the applicable contract unit or lump sum prices as fixed and established in the contract. If, in the opinion of the Contracting Officer, there are no contract unit or lump sum prices applicable to any part of such work, an equitable adjustment pursuant to Clause CHANGES of Section 00700 CONTRACT CLAUSES in Volume 1 will be made as full compensation for the repairs of that part of the permanent work for which there are no applicable contract unit or lump sum prices. Except as herein provided, damage to all work (including temporary construction), SECTION 01 11 00 Page 5 Amendment 0004 utilities, materials, equipment and plant shall be repaired to the satisfaction of the Contracting Officer at the Contractor's expense, regardless of the cause of such damage.
1.9 EXISTING WORK
In addition to "FAR 52.236-9, Protection of Existing Vegetation, Structures, Equipment, Utilities, and Improvements":
a. Remove or alter existing work in such a manner as to prevent injury or damage to any portions of the existing work which remain.
b. Repair or replace portions of existing work which have been altered during construction operations to match existing or adjoining work, as approved by the Contracting Officer. At the completion of operations, existing work shall be in a condition equal to or better than that which existed before new work started.
1.10 EXISTING UTILITIES
1.10.1 Under Ground Utility Location
All underground utilities within the work limits must be positively identified in accordance with the American Society of Civil Engineers, Standard Guideline for the Collection and Depiction of Existing Subsurface Utility Data (latest edition). All underground utilities within the work limits must be positively identified by a private utility locating service and coordinated with the Government. Any utilities found which are not shown on the construction drawings must be brought to the Contracting Officer's attention prior to proceeding with construction.
The Contractor shall coordinate service disconnections of active utilities with the Contracting Officer at least 30 days prior to impacting the utility with construction activities and shall provide weekly progress updates until such time that the utility is taken out of service. The progress update shall include the date at which the active utility impacts construction such that there is no disruption to the Contractor's work and progress.
1.10.2 Notification Prior to Excavation
Notify the Contracting Officer at least 15 days prior to starting excavation work. Contact utility authority 15 days prior to excavating.
Contractor is responsible for marking all utilities not marked by utility authority.
1.10.3 Utilities at South Bay Boat Ramp
At the South Bay Boat Ramp there is an inactive 12 inch ductile iron pipe, raw waterline, crossing the cutoff wall alignment. The Contractor shall remove this pipe and appurtenant features from the embankment to the limits shown. Abandon any portion of pipe not removed in accordance with Section 02 41 00 DEMOLITION. The excavation shall be backfilled with compacted soil in accordance specification section 35 41 00 EMBANKMENT
CONSTRUCTION.
1.10.4 Utilities at Structure S-276

SECTION 01 11 00 Page 6 Amendment 0004 feet deep. The Contractor shall coordinate through the Contracting Officer with Operations Division staff to de-energize and secure the utilities. The Contractor shall remove the wires from each (6 are expected) conduit, remove a section from each conduit as required to install cutoff wall beneath the conduits. Once the cutoff wall is installed, install the section of removed conduit and install the wires as they were prior to construction.
1.10.5 Piezometers
Protect existing piezometers during construction. Should an existing piezometer become damaged, the Contractor shall provide piezometer(s) in accordance with Section 02 32 10 SUBSURFACE DRILLING AND SAMPLING FOR PIEZOMETER INSTALLATION at no cost to the Government. Existing piezometers shown shall be automated in accordance with Section 33 24 15
VIBRATING WIRE PIEZOMETERS.
1.11 TIME EXTENSIONS FOR UNUSUALLY SEVERE WEATHER (31 OCT 1989)
This provision specifies the procedure for the determination of time extensions for unusually severe weather in accordance with the Clause DEFAULT (FIXED-PRICE CONSTRUCTION) of Section 00700 CONTRACT CLAUSES in Volume 1. In order for the Contracting Officer to award a time extension under this clause, the following conditions must be satisfied:
a. The weather experienced at the project site during the contract period must be found to be unusually severe; that is, more severe than the adverse weather anticipated for the project location during any given month.
b. The unusually severe weather must actually cause a delay to the completion of the project. The delay must be beyond the control and without the fault or negligence of the Contractor.
1.11.1 Schedule of Monthly Anticipated Adverse Weather Delays
The schedule of monthly anticipated adverse weather delays is based upon National Oceanic and Atmospheric Administration (NOAA) or similar data for the project location for a daily precipitation greater than or equal to
0.50 inches and will constitute the base line for monthly weather time evaluations. The Contractor's progress schedule must reflect these anticipated adverse weather delays in all weather dependent activities.
See Section 00 33 50 WEATHER AND WATER STAGE DATA to obtain MONTHLY
ANTICIPATED ADVERSE WEATHER DELAY WORK DAYS BASED ON 5-DAY WORK WEEK.
1.11.2 Contractor Responsibility
Upon acknowledgment of the Notice to Proceed (NTP) and continuing throughout the contract, the Contractor will record on the daily CQC report the occurrence of adverse weather and resultant impact to normally scheduled work. Actual adverse weather delay days must prevent work on critical activities for 50 percent or more of the Contractor's scheduled work day. The number of actual adverse weather delay days shall include days impacted by actual adverse weather (even if adverse weather occurred in previous month), be calculated chronologically from the first to the last day of each month, and be recorded as full days. If the number of actual adverse weather delay days exceeds the number of days anticipated in Section 00 33 50 WEATHER AND WATER STAGE DATA (rounded to the nearest whole number), the Contracting Officer will convert any qualifying delays
SECTION 01 11 00 Page 7 Amendment 0004 to calendar days, giving full consideration for equivalent fair weather work days, and issue a modification in accordance with the Clause DEFAULT (FIXED PRICE CONSTRUCTION) of Section 00700 CONTRACT CLAUSES in Volume 1.

The RMS Center will no longer accept phone calls for support requests unless the caller has a ticket ID and the RMS Support Center has started to process the ticket.
1.1.5 Contract Database
Prior to pre-construction conference, Contracting Officer will provide Contractor with the RMS contract ID. The Contractor shall then download the initial RMS export file from the QCS SFTP repository. This will appear as "full export" file for Contractor to "add" and create a QCS contract file. Contracting Officer will provide "QA Data" export updates to Contractor on a regular basis. In order to correctly link QCS data and schedule Contractor must first build "Features" and "Subcontractor" and then use these to link Pay Activities. Prior to submission of the initial project schedule, fill in data for Administrations; Prime Contractor, Subcontractors, Pay Activities, Activity Schedule, Features Schedule, and export it to Contracting Officer.
SECTION 01 45 04 Page 4 Amendment 0004
1.1.5.1 Schedule Standard Data Exchange
Use the activity coding structure defined in the Standard Data Exchange Format (SDEF) in ER 1-1-11 , Appendix A. This exact structure is mandatory, even if some fields are not used. A template SDEF compatible schedule backup file (sdef.prx) is available on the QCS website:
http://rms.usace.army.mil. The SDEF format is as follows:
Field Activity Code Length Description
1 WRKP 3 Workers per Day 2 RESP 4 Responsible Party (e.g. GC, subcontractor, USACE) 3 AREA 4 Area of Work 4 MODF 6 Modification or REA number 5 BIDI 6 Bid Item (CLIN) 6 PHAS 2 Phase of Work 7 CATW 1 Category of Work 8 FOW1 10 Feature of Work (used up to 10 characters in length) 9 FOW2 10 Feature of Work (used up to 20 characters in length) 10 FOW3 10 Feature of Work (used up to 30 characters in length)
1.1.6 Database Maintenance
Establish, maintain, and update contract data for duration of contract.
Update QCS database each workday. Ensure sufficient resources are available to maintain QCS database, and to provide regular database updates. QCS shall be an integral part of the Contractor's management of quality control. At least monthly, generate and submit an export file to Government with schedule update and progress payment request. Submit data updates to the Government (e.g., daily reports, submittals, RFI's, schedule updates, payment requests, etc.) using the Government's SFTP repository built into QCS export function. If permitted by the Contracting Officer, e-mail or CD-ROM may be used instead of the QCS SFTP repository (see Paragraph DATA SUBMISSION VIA CD-ROM). The QCS database typically includes current data on the following items:
1.1.6.1 Administration
a. Contractor Information. Contain within the database the Contractor's name, address, telephone numbers, management staff, and other required items.
b. Subcontractor Information. Contain within the database the name, trade, address, phone numbers, and other required information for all subcontractors. A subcontractor must be listed separately for each trade to be performed. Assign each subcontractor/trade a unique Responsibility Code, provided in QCS.
c. Correspondence. Identify all Contractor correspondence to the Government with a serial number. Prefix correspondence initiated by the Contractor's site office with "S". Prefix letters initiated by the Contractor's home (main) office with "H". Letters must be numbered starting from 0001. (e.g., H-0001 or S-0001). The Government's letters to the Contractor will be prefixed with "C".
d. Equipment. Contain within the Contractor's QCS database a current list of equipment planned for use or being used on the jobsite, including the most recent and planned equipment inspection
SECTION 01 45 04 Page 5 Amendment 0004 dates.
e. Management Reporting. QCS includes a number of reports that Contractor management can use to track the status of the project. The value of these reports is reflective of the quality of the data input, and is maintained in the various sections of QCS. Among these reports are: Progress Payment Request worksheet, QA/QC comments, Submittal Register Status, Three-Phase Inspection checklists.
f. Request for Information (RFI). Exchange all Requests For Information (RFI) using the Built-in RFI generator and tracker in QCS.
1.1.6.2 Finances
a. Pay Activity Data. Include within the QCS database a list of pay activities that the Contractor must develop in conjunction with the construction schedule. The sum of all pay activities must be equal to the total contract amount, including modifications. Group pay activities Contract Line Item Number (CLIN); the sum of the activities must equal the amount of each CLIN. The total of all CLINs equals the Contract Amount.
b. Payment Requests. Prepare all progress payment requests using QCS. Complete the payment request worksheet, prompt payment certification, and payment invoice in QCS. Update the work completed under the contract, measured as percent or as specific quantities, at least monthly. After the update, generate a payment request report using QCS. Submit the payment request, prompt payment certification, and payment invoice with supporting data using the Government's SFTP repository built into QCS export function. If permitted by the Contracting Officer, e-mail or a CD-ROM may be used. A signed paper copy of the approved payment request is also required, which will govern in the event of discrepancy with the electronic version.
1.1.6.3 Quality Control
QCS provides a means to track implementation of the 3-phase QC Control System, prepare daily reports, identify and track deficiencies, document progress of work, and support other Contractor QC requirements. Maintain this data on a daily basis. Entered data will automatically output to the QCS generated daily report. Within seven calendar days of Government acceptance, submit a QCS update reflecting the information contained in the accepted CQC Plan: schedule, pay activities, features of work, submittal register, QC requirements, and equipment list.
a. Daily Contractor Quality Control (CQC) Reports. QCS includes the means to produce the Daily CQC Report. The Contractor may use other formats to record basic QC data. However, the Daily CQC Report generated by QCS must be the Contractor's official report. Summarize data from any supplemental reports by the Contractor and consolidate onto the QCS-generated Daily CQC Report. Electronically submit reports to the Government within 24 hours after the date covered by the report. Also provide the Government a signed, printed copy of the daily CQC report.
b. Deficiency Tracking. Use QCS to track deficiencies.
Deficiencies identified by the Contractor will be numerically tracked using QC punch list items. Maintain a current log of its QC punch list items in the QCS database. The Government will log the
SECTION 01 45 04 Page 6 Amendment 0004 deficiencies it has identified using its QA punch list items. The Government's QA punch list items will be included in its export file to the Contractor. Regularly update the correction status of both QC and QA punch list items.
c. QC Requirements. Develop and maintain a complete list of QC testing and required structural and life safety special inspections required by the International Code Council (ICC), transferred and installed property, and user training requirements in QCS. Update all data on these QC requirements as work progresses, and promptly provide this information to the Government via QCS.
d. Three-Phase Control Meetings. Maintain scheduled and actual dates and times of preparatory and initial control meetings in QCS.
e. Labor and Equipment Hours. Log labor and equipment exposure hours on a daily basis. This data will be rolled up into a monthly exposure report.
f. Accident/Safety Reporting. The Government will issue safety comments, directions, or guidance whenever safety deficiencies are observed. The Government's safety comments will be included in its export file to the Contractor. Regularly update the correction status of the safety comments. In addition, utilize QCS to advise the Government of any accidents occurring on the jobsite. This brief supplemental entry is not to be considered as a substitute for completion of mandatory reports, e.g., ENG Form 3394 and OSHA Form 300.
g. Features of Work. Include a complete list of the features of work in the QCS database. A feature of work may be associated with multiple pay activities. However, each pay activity (see subparagraph "Pay Activity Data" of paragraph "Finances") will only be linked to a single feature of work.
h. Hazard Analysis. Use QCS to develop a hazard analysis for each feature of work included in the CQC Plan. Address any hazards, or potential hazards, that may be associated with the work.
i. Warranty. The Government will issue warranty items whenever warranty deficiencies are observed. The Government's warranty comments will be included in its export file to the Contractor during the warranty period. Update the scheduled correction date, actual correction date, and correction status under Contractor remarks.
Response times shall comply with warranty requirements outlined in the approved warranty management plan.
j. Three-Phase Inspection Checklists. Use QCS to create and maintain all three-phase inspection checklists.
k. Subcontractors. Use QCS to track subcontractors. Record subcontractors working on site on each daily QC report.
1.1.6.4 Submittal Management
The Government will provide the initial submittal register in electronic format. Thereafter, maintain a complete list of all submittals, including completion of all data columns. Dates on which submittals are received and returned by the Government will be included in its export file to the Contractor. Use QCS to track and transmit all submittals. ENG Form 4025, SECTION 01 45 04 Page 7 Amendment 0004 submittal transmittal form, and the submittal register update must be produced using QCS. QCS and RMS will be used to update, store and exchange submittal registers and transmittals, but will not be used for storage of actual submittals.
1.1.6.5 Schedule
Develop a construction schedule consisting of pay activities, in accordance with Section 01 32 01 PROJECT SCHEDULE. Input and maintain this schedule in the QCS database. Include with each pay request the updated schedule.
1.1.6.6 Import/Export of Data
QCS includes the ability to export Contractor data to the Government and to import submittal register and other Government-provided data from RMS.
1.1.7 Implementation
Contractor use of QCS as described in the preceding paragraphs is mandatory. Ensure that sufficient resources are available to maintain its QCS database, and to provide the Government with regular database updates. QCS shall be an integral part of the Contractor's management of quality control.
1.1.8 Data Submission Via CD-ROM
The Government-preferred method for Contractor's submission of QCS data is by using the Government's SFTP repository built into QCS export function.
Other data should be submitted using E-mail with file attachment(s). For locations where this is not feasible, the Contracting Officer may permit use of CD-ROM for data transfer. Export data onto CDs using the QCS built-in export function. If used, submit CD-ROMs in accordance with the following:
1.1.8.1 File Medium
Submit in English required data on CD-ROM conforming to industry standards used in the United States.
1.1.8.2 CD-ROM Labels
Affix a permanent exterior label to each CD-ROM submitted. Indicate on the label in English, the QCS file name, full contract number, contract name, project location, data date, name and telephone number of person responsible for the data.
1.1.8.3 File Names
The files will be automatically named by the QCS software. The naming convention established by the QCS software must not be altered.
1.1.9 Monthly Coordination Meeting
Update the QCS database each workday and export to the QCS SFTP repository. At least monthly, generate and submit an export file to the Government with schedule update and progress payment request. As required in Contract Clause "Payments", at least one week prior to submittal, meet with the Government representative to review the planned progress payment
SECTION 01 45 04 Page 8 Amendment 0004 data submission for errors and omissions.
Make all required corrections prior to Government acceptance of the export file and progress payment request. Payment requests accompanied by incomplete or incorrect data submittals will be returned. The Government will not process progress payments until an acceptable QCS export file is received.
