SMALL BUSINESS PARTICIPATION COMMITMENT DOCUMENT

Other than small business offerors (large businesses) are required to complete a Small Business Participation Commitment Document. Offerors shall propose the level of participation of small businesses (as a small business prime and/or small business subcontractors) in the performance of the acquisition relative to the objectives/goals set forth in the evaluation of this area. A suggested Total Small Business Participation goal of 5% of the total contract value is assigned to this acquisition.

(a) Submit the total combined percentage of work to be performed by both large and small businesses (include the percentage of work to be performed both by prime and subcontractors). Example: If Prime proposes a price of $1,000,000 (including all options), and small business(es) will provide $250,000 in services/supplies as a prime or subcontractor, the percentage planned for small business is 25% and the percentage planned for large business is 75%, equaling 100%.

Total percentage planned for % = $ Large Business(es)

Total percentage planned for % = $ Small Business(es)

HUBZone Small Business %

Women-Owned Small Business %

Service-Disabled Veteran-Owned Small Business %

(c) List of principal small business firms by name and/or socioeconomic category and the services/supplies to be performed/acquired from the company:

(d) Describe the extent of commitment to use each of the identified small businesses.

Offerors do not need to submit copies of commitment instruments in their proposals unless and to the extent expressly stated in the RFP. Rather, identify what types of commitments are in place with each company, e.g., a written contract, letter of commitment, verbal agreement, joint venture, mentor‐protégé agreement, including how long the agreement/instrument is in place.

Company Type of Commitment

(e) Past performance of the offerors in complying with requirements of the clauses at FAR 52.219‐8, Utilization of Small Business Concerns, and 52.219‐9, Small Business Subcontracting Plan.
