Discipline Dwg Sheet/Spec Sectio Comment Response

1 Flooring 09 06 90

2.2.1.3.1 Sheet Flooring specifies Forbo Flooring Systems. Would Armstrong Amorette be an acceptable substitute? It is a comparable product.

Please see Para 2.1 of this section for acceptable use of other manufacturer's.

I did not see any mass notification which is typically present on DOD projects. I assume this building is not required to have a MNS installed?

A mass notification system is not part of this contract. There is a local PA system that will be tied into the Depot's central PA system by TYAD personel (DOIM).

3 00800

Special Clauses, Paragraph SC-1, on Page 2, which states: “… complete the entire base bid work ready for use not later than 180 calendar days …”. Project duration of 180 calendar days is unrealistic. The availability of Steel materials alone will prohibit this duration. Please confirm that the 180 calendar days allotted for the entire base bid work is correct and will not be increased.

See Amendment No. 0002.

4 Dwg A-601 Please clarify what type of gypsum wall board is to be installed for wall types 3B and 3C. To be revised in an upcoming amendment.

5 Dwg A-601 Please clarify if wall types 3A, 3B and 3C are to receive gypsum board on one (1) side or both sides of the metal framing. To be revised in an upcoming amendment.

8 Dwg I-101 Detail 2 on Drawing I-101 shows a flooring finish in Vestibule 101 as CPT1. However, the Room Finish Schedule states that CT1 is to be installed. Please clarify the floor finish for Vestibule 101.

See Amendment No. 0002.

9 Dwg I-101 The Room Finish Schedule states that Rooms 103, 104, 107, 109 and 121 are to receive a wainscoting 42” AFF. Please identify the material for the wainscoting with the supporting specification.

See Note 1 on this drawing which fully defines the extent of finish for the wainscot.

10 Furniture 1.3.1 / 12-1-64

It states furniture will be provided by DDTP/DLA.and on the drawing under general notes it states that All furniture to be provided and insatlled by DLA and/or DPW under separate contract. When and how will this contract be issued for furniture? As stated in the plans and specs, furniture is Not in Contract.

According to the spec and the blue prints it states (connect new fire alarm devices to new notifier fire alarm control panel) This statement allows only Notifier Fire Alarm company to bid this project, Is this a sole source project? To be revised in an upcoming amendment.

drawings 12-1-50,51 &

The key shows only half the building being in the contract, however, neither the roof or the elevation drawings indicate that we're dealing with just half. Should I be figuring the whole building for lightning protection? To be revised in an upcoming amendment.

Could you please clarify for me the size of the sub-base fuel tank for the generator on this project? The single line diagram lists a Kohler part number for a 247 gallon/84 hours capacity tank and the specifications call for a 53 hour capacity tank. Also, it is only a single tank for the generator, there are no day tanks or secondary tanks needed? See Amendment No. 0002.

14 Roofing A-105 Please confirm that the area of tapered insulation is only from the center (12" high point) to the dashed line (which would be 3-1/2").

The tapered insulation is as noted on the roof plan at the existing flat structure area.

15 General 01 45 01 Is the CQC Manager the only staff required for the quality control program for this project?

A "sufficient number of additional qualified personnel to ensure safety and contract compliance" is also required per the specification, which may or may not require more

16 Concrete S-002

Sikatop 126 Plus does not exist anymore. Please indicate that Sikatop 123 Plus is an acceptable product for Type B Concrete Repair. See Amendment No. 0002.

Solid Surface A402

11/A402 indicates the casework top in the Multipurpose Room 121 to be Plam. However, the finish schedule I-101 indicates that it will be solid surface. See Amendment No. 0002.

Safety /

CQC

Door Hardware 087100 - 2.3.4

The depot has an independent company / vendor who supplies and installs all cylinders & cores. Are we to exclude the cylinder & cores from the project requirements? The specs do not reflect this. To be revised in an upcoming amendment.

20 Mechanical M401, M601

Boiler schedule indicates B-1 and B-2. Mechanical room plan indicates B-1 and a location for a future boiler. Is B-2 part of the base bid or is it for future consideration only? See Amendment No. 0002

21 Mechanical MP101, MP102 Need specification for heating hydronic piping. See Spec section 23 52 00.

22 Mechanical MP101, MP102 Need specification for pre-insulated chilled water piping. See Spec section 33 61 00.

23 Architectural none

Unable to finish specification on interior & exterior signage. There is also no signage schedule as part of the drawings. Please clarify since there could be ADA and life safety issues which will delay any occupancy certificates. Signage will be provided by the Govt. Please bid as shown.

24 Roofing A-105 page 17

Accoding to the scope of work, the entire roof is to be removed and replace but your keyplan states 1/2 of the building is not in contract.

We do not believe this is correct for the roof. Please clarify. To be revised in an upcoming amendment.

5. Procedure for In-House Excavation (see Appendix A for In-House Flowchart)

a. Whenever it is necessary for any advanced planned excavation or work that has the potential to damage any underground utilities by in-house personnel this procedure shall be followed and an excavation permit shall be secured. No excavation work shall commence until the permit is approved. A copy of the approved permit must be kept by the Utilities & Grounds Division (U&G Div) Chief or Leader at all times. See below for emergency excavating situations.

b. U&G Div shall be responsible for filling out the attached application (See Enclosure 1). For planned excavations, it shall be submitted to Chief Engineering Division at least 5 working days prior to the planned start of work. U&G Div shall call the PA One-Call Service if excavation work is in the vicinity of the Natural Gas (GAS), phone, or cable TV lines.

c. Prior to requesting the marking of utilities, U&G Div shall stake out proposed excavations and limits of work with white lines ("White Lining") or other acceptable means.

d. For emergency conditions, no TYAD permit will be necessary. U&G Div shall notify the PA One Call Service if deemed necessary by Division Chief. U&G Div shall notify the Engineering Div Point of contact (POC) either telephonically or via e-mail and shall explain the requirements.

An emergency is defined as a sudden or unforeseen occurrence involving a clear and immediate danger to life or property, including but not limited to, serious breaks or defects in utility lines that must be repaired within 24 hours. U&G Div shall identify proposed excavation location or mark up a plan showing planned excavation areas.

e. Prior to starting work, U&G Div shall visually survey and verify that all utility markings are consistent with existing appurtenances such as manholes, valve boxes, poles, pedestals, pad-mounted devices, gas meters, etc. prior to any excavation. U&G Div will also, verify that any abandoned utilities are not active. Any discrepancies noted should be brought to the attention of Eng Div. All utility markings shall be preserved for the duration of the project to the maximum extent possible.

f. When excavation is performed within 2 feet of any utility line, hand digging shall be used when feasible.

g. Work shall always proceed in an orderly and effective manner to preclude potential damage or problems. Additional precautionary measures may be required if differing site conditions are determined to exist.

h. Should excavation be required around abandoned steam piping that has asbestos insulation, Environmental Management Division shall be notified at least 14 days in advance.

SECTION 01 11 00 Page 7

i. Additional Requirements for work at or near the GAS line.

For any excavation in the vicinity of the GAS line, crossing, or running alongside the GAS line, the requesting organization SHALL NOTIFY the Energy Savings Performance Contract (ESPC) Contracting Officer's Representative (COR) (Contact- PHONE X5-7546,CELL PHONE (570) 269-9778,E MAIL RANDY.S SIMPSON@ TOBYHANNA.ARMY.MIL or the ESPC contractor (AMERESCO Energy Services Inc.) representative (Phone 894-5624 or e-mail to CHAZELTON@AMERSCO.COM prior to start of work and shall comply with accepted GAS industry standards before backfilling around the gas line.

j. If there is damage to the GAS line, immediately notify the TYAD Fire and Emergency Services Division at 911 from a depot phone or (570) 615-7300 from outside phones. Then notify the ESPC COR (Phone X5-7199, Pager 9-480-1537), a representative (Phone 894-5624), or the GAS Company. If GAS is leaking, no personnel should be in the excavation or within 200 feet from the leak. Abandon equipment in place and do not turn off or reposition the equipment. Do not operate any electrical switches or machinery within 200 feet of the leak. DO NOT TOUCH THE GAS LINE.

k. Off-Post Digging - TYAD must follow all One-Call requirements for any excavation NOT on federal grounds (OFF-POST).

6. Specification Section for Contractors Excavating Permits (See Appendix B for Contractors Excavation Permit flowchart)

a. TYAD fully complies with the PA One Call System's notification requirements. This is done in order to ensure that all Government and Non-Government utilities are covered by the requirements.

b. When the project is in design phase the designer must call PA one call and assign a serial number to the project.

c. Any contractor, agency, company, or individual must comply with the provisions of the Underground Line/Facilities Damage Prevention Act of 1996, 73 Pa.C.S.A. §176 et. seq. The contractor shall also obtain an excavation permit from TYAD prior to commencement of any excavation work on the installation or any work that has the potential to damage any underground utilities, to include, but not limited to, installation of signs, fence posts, utility poles, planting trees or shrubs, etc. When the permit is approved, it must be kept on site at all times.

d. The Contractor shall complete and sign an EXCAVATION PERMIT AND RISK ASSESSMENT application (see enclosure 2), and submit it to the COR at least 3 working days prior to the planned start of any excavation or other work in the vicinity of a utility. No excavation work shall commence until the permit is approved by the DPW.

e. Prior to requesting the marking of utilities, the Contractor shall stake out proposed excavations and limits of work with white lines ("White Lining"). The Contractor shall contact the PA One-Call Service (1-800-242-1776) and the COR in order to get the existing utility lines marked and shall ensure the utility locations are physically marked on the ground by the utilities' owners prior to submittal of the permit request.

The COR needs at least 10 working days advance notice to mark utilities.

In addition, the Contractor shall visually survey and verify to the COR that all utility markings are consistent with existing appurtenances such as manholes, valve boxes, poles, pedestals, pad-mounted devices, natural gas (GAS) meters, etc. prior to any excavation. Also, verify that any abandoned utilities are not active. Any deviations from approved markings

SECTION 01 11 00 Page 8 must be consulted with COR prior to proceeding. All utility markings shall be preserved for the duration of the project to the maximum extent possible. The Contractor shall not proceed with any excavation until all utilities, including abandoned utilities, have been marked and the permit is issued. For any excavations required beyond the originally designated limits of construction defined by the white lines, the Contractor shall co-ordinate with the COR and another permit may be necessary.

f. When excavation is performed within 2 feet of any utility line, hand digging shall be used. The contractor shall exercise caution to verify the exact depth and location of all utilities prior to any mechanical excavation. COR may grant exception to this requirement in some extreme conditions. Other non-damaging methods of excavation, such as; soft digging, vacuum excavation, pneumatic hand tools, may be considered subject to approval by the COR.

g. Granting of an excavation permit by the government does not relieve the contractor from any responsibility for damage to utilities. Work shall always proceed in an orderly and effective manner to preclude potential damage or problems. The contractor will be liable for all actual or consequential damages that occur as a result of the contractor's fault or negligence.

h. The Government reserves the right to specify additional precautionary measures to be employed if differing site conditions are determined to exist.

i. Failure to obtain necessary permits prior to excavation may result in the issuance of a "Stop Work" order by the Contracting Officer, which shall not be justification for contract delay or time extension.

j. The Contractor shall ensure that all excavation requirements herein are met and shall also establish and document contingency plans. These actions should be listed on the permit application in the event that existing utilities are damaged or interrupted. Locations of shut-off or isolation devices along with other safety features shall be established and their operation reviewed with all contractor site personnel and sub-contractors and the COR.

k. Any other work that could damage or interrupt a utility, such as, exterior or interior work near transformers, power lines, poles, above ground GAS lines, GAS meters, etc., shall be done with extreme care and coordinated with the COR prior to scheduling of the work. The Contractor shall utilize accepted construction techniques to preclude damaging or interrupting any utility.

l. Should asbestos containing materials (ACM) be involved with any excavations such as around abandoned steam piping or transite piping, additional requirements will be specified in the permit. Should any potential ACM be encountered during excavations, the Contractor shall stop work immediately and notify the COR.

m. Additional Requirements for work at or near the GAS line.

n. For any excavation in the vicinity of the GAS line, crossing, or running alongside the GAS line, the Contractor shall also notify the COR one day prior to start of work and shall comply with accepted GAS industry standards before backfilling around the GAS line. All backfilling within 2 feet of the GAS line shall be witnessed by the COR.

SECTION 01 11 00 Page 9

o. If there is damage to the GAS line the contractor shall immediately notify the TYAD Fire and Emergency Services Division at (570) 615-7300 or (911 from a depot phone). The contractor shall then notify the COR or GAS Company.
