WAGE DETERMINATION APPEALS PROCESS

1.) Has there been an initial decision in the matter? This can be:

* an existing published wage determination

* a survey underlying a wage determination

* a Wage and Hour Division letter setting forth a position on a wage determination matter

* a conformance (additional classification and rate) ruling

On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.)

and 3.) should be followed.

With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations. Write to:

Section 00815 - 13

Branch of Construction Wage Determinations Wage and Hour Division U.S. Department of Labor 200 Constitution Avenue, N.W.

Washington, DC 20210

2.) If the answer to the question in 1.) is yes, then an interested party (those affected by the action) can request review and reconsideration from the Wage and Hour Administrator (See 29 CFR Part 1.8 and 29 CFR Part 7). Write to:

Wage and Hour Administrator U.S. Department of Labor 200 Constitution Avenue, N.W.

1.2 GENERAL REQUIREMENTS

The well shall be located as shown, and be constructed in accordance with these specifications. The well shall be installed to prevent aquifer contamination by the drilling operation and equipment, intra- and inter-aquifer contamination, and vertical seepage of surface water adjacent to the well into the subsurface, especially the well intake zone.

An experienced, licensed well installation subcontractor shall be obtained by the Contractor to install the water well that will be used for drinking water and other potable water applications on this site. Based upon review of a limited amount of information on the design and operation of the existing well on site, it is anticipated that the new well will have to extend to a depth of 250 feet below the surface in order to obtain a water quality and quantity required for this facility. The suitability of this depth shall be confirmed by the drilling contractor during installation of the test hole that will be adapted for the final well, based on the results of the following requirements for the test hole, as specified herein: a capacity test, a downhole geophysical survey, and either a visual or acoustic televiewer survey.

Review of the boring log from test boring DLB-38 (which is located in generally close proximity to the proposed well location) indicates that fractured rock is encountered below a thin surficial layer of soil, to a depth of around 15 feet, which is the termination depth of the boring. The boring log from DLB-18 (located a little further east of DLB-38) extended to a depth of around 30 feet and encountered fractured rock to that level, also after a thin surficial layer of soil. In order to isolate this fractured rock that is in contact with the surface, an outer steel casing is required for the well as specified herein. This casing shall be 10-inches in diameter and installed in a boring excavation that shall be at a minimum 14-inches in diameter in order to permit grouting of this casing into place at the outset of the well construction. After this outer casing

Section 02521 Page 2 has been set and grouted completely from its bottom in the rock to the surface, drilling of the remaining portion of the well may proceed.

In order to assist the driller in determining the required well design, depth of well boring and selection of a suitable screen length, pump location, etc., the driller will be required to perform a well capacity test, a downhole geophysical survey, and a visual or acoustic televiewer survey of the test well, all prior to installation of inner casing and the well screen. The proposed final well design shall be prepared by the Contractor and drilling subcontractor and submitted to the CO for review.

Government approval of the well design is required prior to well installation. A final performance test and well disinfection will be required prior to acceptance of the permanent well.

The permanent well is required to be constructed of a PVC inner casing and well screen, with the screen slot size, length, gravel pack design and bentonite seal level determined by the drilling contractor based on the well evaluation and testing indicated above and CO approval.

1.3 SUBMITTALS

1.6 TEST HOLE

1.6.1 Test Hole

After installation of the outer casing, a test hole of at least 10 inches in diameter shall be drilled at the location of the well into the target water bearing strata as directed by the Contracting Officer. The test hole shall be drilled in a manner to protect the subsurface from surface contamination. The test hole shall be carefully advanced and sampled to determine the presence of the upper aquiclude if one exists. The boring shall then be properly cased, grouted, and sealed into the aquiclude before the boring is advanced through the aquiclude into the aquifer. The test hole shall be used to determine the expected yield from the well, water quality, optimum depth, and to log the strata encountered. Before conducting a capacity test, the well shall be cased, and screened in accordance with these specifications. The test hole shall be logged in accordance with paragraph BOREHOLE LOGS. A permanent outer casing shall be used. The outer casing shall be 10 inches in diameter and be seated to a depth of at least 50 feet below the surface, into the top of the rock. The test hole shall be converted to the permanent well, in accordance with these specifications. 2.3 CENTRALIZERS

Centralizers shall be utilized to center inner casing and the screen within the outer casing and open borehole. When the filter material is introduced between the side of the bedrock excavation and the casing and screen, the centralizer ensures that the filter material is of even radial thickness in the annular space. Centralizers consisting of PVC, stainless steel, or other material approved by the COR shall be utilized along the entire length of inner casing at a spacing of 25 feet along the length of the casing. It is important in the installation of the screen that the centralizer is axially fixed in place; therefore, the spacing on the screened sections of the well shall be 20 feet apart. Centralizers shall not be installed on the screened portions of the well screen. A short (2-foot), solid blank section of pipe may be utilized between the well screen sections, if necessary to secure the centralizer to the well screen sections. The centralizers shall be securely fixed to the casing and well screen sections. The centralizers shall be oriented at a spacing of 120 degrees around the casing and screen sections. A centralizer which is not fixed in place will tend to slide along the screen and string of pipe if its projecting ears become hung up within the casing, such as on a casing joint or rock fracture. Such movement of the centralizer with respect to the casing and screen will negate the centralizer's function and is prohibited.

2.4 FILTER PACK

Filter pack material shall be a product of a commercial sand and gravel supplier, shall be properly sized and graded for the surrounding formation conditions encountered, and shall be composed of clean, round, hard, waterworn siliceous material, free of flat or elongated pieces, organic matter, or other foreign matter. The filter material shall be of a size which will allow the maximum flow of water into the well and prevent the infiltration of sand and silt. The gradation of the filter material shall be such that the uniformity coefficient is not more than 2.5. The filter material shall be thoroughly sterilized with chlorine or hypochlorite immediately before being placed and placed as directed. Sterilization will not be required if this material is delivered to the site in sealed bags or containers and is certified as clean by the manufacturer.

2.5 BENTONITE SEAL

The bentonite seal, intended to keep grout from entering the filter pack, Section 02521 Page 10 shall consist of hydrated granular, or pelletized, sodium montmorillonite furnished in sacks or buckets from a commercial source and shall be free of impurities which adversely impact the water quality. If the bentonite seal is located above any borehole fluid levels, a layer of fine sand shall be placed at the top of the bentonite seal, to provide an additional barrier to any downward migration of grout.

2.6 CEMENT AND BENTONITE GROUT

2.6.1 Cement Grout

Cement grout shall consist of Portland cement conforming to ASTM C 150, Type I or II, sand and water. Cement grout shall be proportioned not to exceed 2 parts, by weight, of sand to 1 part of cement with not more than 6 gallons of water per 94 lb bag of Portland cement, with a mixture of such consistency that the well can be properly grouted. No more than 5 percent by weight of bentonite powder may be added to reduce shrinkage.
