The purpose of this amendment for Indefinite Delivery Indefinite Quantity (IDIQ) for Dam Maintenance and Repairs is to incorporate Section 01 32 01.00 10 "General Requirements, Section 02 83 30 "Removal And Disposal Of Coatings Containing Heavy Metals and Section 09 96 20 "Coatings" into the solicitation. CLINs 0001 through 0006 are deleted in their entirety. Task Orders 0001 through 0006 have been incorporated into the solicitation, w hich provides a more detailed description. All Sub-CLINs are deleted in their entirety and have been replaced w ith a Pricing Schedule for Task Orders 0001 through 0006, w hich provides a more indepth line item breakout.

The proposal due date is extended to September 8, 2011, 1:00 pm local time and place.

All other terms and conditions remain unchanged.

Task Order 1

Pine Flat Dam Tainter Gate Coating

Statement of Work

1 GENERAL:

A. SCOPE – Provide all supplies, materials, equipment and labor necessary for preparation and coating of 6 radial arm Tainter gates and their components at Pine Flat Dam. The gates are 38 feet tall and 42 feet wide with a radius of 39 feet. Remove existing coating, clean surface and recoat all components of the gates, including but not limited to the arms, trunions, mechanisms, and catwalks. Completely clean, prepare the surface and coat the upstream and downstream faces of each radial gate with new coating per Specification 09 96 20 – Coating or the Coating Manufacturer recommendations. Plan, coordinate, organize, manage, and perform the activities described within this specification.

B. INTENT – Removal of all coatings, repair minor corrosion damage and reapply protective coating on 6 radial arm Tainter gates, their components, including but not limited to the arms, trunions, mechanisms, and catwalks, and other associated equipment at Pine Flat Dam. Primary focus will be on the metal components. Work shall be in accordance with the attached Scope of Work, Technical Specifications, Contract Clauses and Davis Bacon Wage Rates.

a. Tainter gate dimensions (approximate): 38 feet tall and 42 feet wide with a foot radius of 39 feet (upstream and downstream), six total

b. Catwalk dimensions (approximate): 3 feet wide (enough room for one person to walk comfortable or two persons pass closely) x 45 feet length (width of Tainter gates plus jogs around mechanical equipment), six total

C. LOCATION DESCRIPTION – Work site is located at 27295 Pine Flat Road, Sanger, CA 93657

D. PERMITS – Without any additional expense to the government, provide any necessary/required licenses and/or permits for the completion of this contract. The Contractor shall comply with all Federal, state, county and municipal laws, codes, and/or regulations in connection with the performance of specified work.

E. SAFETY – Perform all work in accordance with safety requirements set forth in the current EM 385-1-1, Safety and Health Requirements Manual. The US Army Corps of Engineers, Sacramento District Safety office will review all Safety Plans, Confined Space Plans, Hot Work Permits, Ventilation Assessment Plans, Abrasive Blasting Requirement, Respiratory Protection Plan, Explosion Proof Equipment systems, Medical Surveillance Plan, Lead- Based Paint Worker Protection Plan, and Air Monitoring Test Plan.

F. ENVIRONMENTAL PROTECTION PLAN - In addition to the safety requirements, comply with Environmental Protection requirements. Prepare an Environmental Protection Plan that complies with the following required environmental protection criteria: Waste Classification, Handling, and Disposal Plan, Waste Manifest, Containment Plan, Visible Emissions Monitoring Plan, Water Quality Plan, Manufacturer's Product Data Sheets, Product Samples: Special Paint Formulas & Solvent and Thinners Information sheets.

G. CONTRACTING OFFICE REPRESENTATIVE – The Contracting Officer’s Representative (COR) for this task order shall be Tom Ehrke.

2 SPECIAL CONSIDERATIONS

A. Repair work shall comply with the safety and health requirements contained in the Corps of Engineer's SAFETY AND HEALTH REQUIREMENTS MANUAL, EM 385-1-1, the National Electric Code, and in accordance with the applicable requirements of the GENERAL SAFETY REQUIREMENTS. The Contractor, in coordination with dam operations personnel, shall develop and utilize a check-in and check-out procedure to ensure the safety of personnel entering and leaving the job site. Provide scaffold or other high work platform as needed.

B. Ensure that all materials resulting from the activities outlined in this contract are contained and properly disposed of. Impermeable tarps and a dust collector shall be used to collect dust and materials generated by abrasive blasting in order to prevent these materials from escaping into the environment, if this method of removal is utilized.

All surfaces will be abrasive blasted prior to painting. The original paint system on the radial gates may have contained a lead based primer.

a. The Contractor shall implement upstream protection devices to prevent existing coating removal debris from falling into the lake.

b. The Contractor is allowed to collect the existing coating removal debris in the flip bucket on the downstream side of the dam. The Contractor shall construct a 5 foot minimum barrier on top of the flip bucket along the entire spillway area to prevent existing coating removal debris from entering into the downstream water. The Contractor shall collect, haul and dispose of existing coating removal debris off site and in compliance with local, State and Federal requirements. Test and submit water quality before, during and after existing coating removal to ensure water quality downstream of the dam has not changed.

C. Walls, equipment, rubber J-seal, fixtures, motors, greased bearing and track, as well as all other items in the vicinity of the surfaces being painted shall be maintained free of damage by paint of painting activities. Cleanup of any paint spillage and/or repair of any painting activity damage shall completed with 24 hours.

D. Personnel accessing the work area:

a. The Contractor shall provide a lift system to lower equipment and/or personnel into the gate area. Current access to this area is only by the catwalks with personal fall protection required.

b. The Contractor shall design and submit all scaffolding plans and calculations for Government approval and conform to all local, State, and Federal requirements.

E. Equipment accessing the work area:

b. In addition to the requirements outlines in Appendix A of EM 385-1-1, the following is required:

i. Crane Critical Lift Plan. Prepare and sign weight handling critical lift plans for lifts over 75 percent of the capacity of the crane or hoist at any radius of lift; lifts involving more than one crane or hoist; lifts of personnel; and lifts involving non-routine rigging or operation, sensitive equipment, or unusual safety risks. Submit plans no later than 15 calendar days prior to on-site work and include the requirements of EM 385-1-1, paragraph 16.H. and the following:

1. For lifts of personnel, demonstrate compliance with the requirements of 29 CFR 1926.550(g).

F. Areas of work being performed:

a. The entire Tainter gate, catwalks, and all associated components will be available for work.

b. The areas of the gates, frames, guides, catwalks, and associated components that receive grease, contain lubricating oils, provide bearing or gate operations shall be excluded from items of work.

4 PERIOD OF PERFORMANCE:

A. WORK SCHEDULE – Anticipate construction work will be from 01 September 2012 to 30 November 2012.

This would allow for unrestricted access to all gates on the dam structure. Coordinate with Dam Personnel.

B. PREWORK CONFERENCE – At least ten (10) days prior to commencement of work, contact the COR to schedule a pre-work meeting. This meeting will be held at the Pine Flat Lake Park Headquarters, located at 27295 Pine Flat Road, Sanger, CA 93657. During the pre-work meeting, provide the COR with the following for approval:

a. Submit a Hazard Analysis and copies of applicable material safety data sheets (MSDS) for all chemical products to be used by the Contractor or their employees during the execution of this contract.

b. Use a qualified Safety and Health Manager (SHM) to prepare the written site-specific Accident Prevention Plan (APP) in accordance with the format and requirements of USACE EM 385-1-1. Cover all paragraph and subparagraph elements in Appendix A of USACE EM 385-1-1 "Minimum Basic Outline for Preparation of Accident Prevention Plan.” Activity Hazard Analyses (AHAs) shall be appended to the

APP.

c. The Contractor shall develop an Injury and Illness Prevention Program (IIPP) for projects within the State of California as an appendix to the APP. Where a paragraph or subparagraph element is not applicable to the work to be performed, indicate "Not Applicable" next to the heading. The APP shall be job specific and shall address any unusual or unique aspects of the project or activity for which it is written. The APP shall interface with the Contractor's overall safety and health program (SHP). Any portions of the Contractor's overall safety and health program referenced in the APP shall be included in the applicable APP element and made site-specific. The Government considers the Prime Contractor to be the "controlling authority" for all work site safety and health of the Subcontractors. The Contractor is responsible for informing their Subcontractors of the safety provisions under the terms of the contract and the penalties for noncompliance, coordinating the work to prevent one craft from interfering with or creating hazardous working conditions for other crafts, and inspecting Subcontractor operations to ensure that accident prevention responsibilities are being carried out. The Contractor, COR, and Site Safety and Health Officer (SSHO) shall sign the APP.

d. Submit the APP to the USACE Project Safety Officer within 10 days of award. The Government will review the documents within 10 days. The Contractor shall resubmit the documents, with any requested changes, within 5 days of request.

e. The Contractor’s APP and subsequent activities shall comply with the following referenced documents (most current version, if different than the date referenced), at the latest version of: EM 385-1-1, USACE Safety & Health Requirements Manual.

f. Contractor Vehicle Identification: All Contractor vehicles shall be identified with the company name or logo conspicuously displayed on door panels. Professionally made magnetic signs or pressure sensitive decals may be used to comply with this specification.

5 CONTRACTOR OBLIGATIONS:

A. LICENSING – Prior to starting work, submit a copy of current qualified contractor’s license, business licensing, registration with state, county, and local Business Administrations and/or any licenses required to comply with Federal, state and local laws. Submit the above to the COR at the pre-work conference.

B. EQUIPMENT – All equipment shall have all safety features intact and in operating condition. All equipment must be suitable for use on the described task order and is subject to the approval of the COR or his/her authorized representative.

C. REPORT – Submit a man-hour exposure report to the COR prior to the completion of the contract.

a. Maintain an accurate record of, and notify the COR immediately following the occurrence/discovery, of any accidents resulting in death, injury, occupational disease, and/or damage to property or equipment, incidental to work performed under this contract.

b. Submit an invoice for payment within 10 days of service completion, verifying that all units, quantities and amounts are true and correct. The Contractor shall sign the invoice.

6 CONTRACTOR RESPONSIBILITIES:

A. The contractor is liable for all damage to property, equipment, and landscape features in performance of this work. Immediately report any such damage to the COR and repair at Contractor's expense.

B. Remove all waste material generated in performing this contract from government property and dispose of at an approved waste disposal site. Comply with all Federal, State and local guidelines as they pertain to the proper excavation, removal, and disposal of waste material generated during the performance of this contract specification.

C. The Contractor and their employees shall be responsible for maintaining accurate and detailed records as required by the contract.

D. The Contractor and their employees shall be capable of communicating with other contractors, outside agencies and project personnel in a respectful and courteous manner.

E. Furnish competent personnel. If employees furnished prove to be incompetent, the Contractor shall remove them and furnish satisfactory employees at the discretion of the COR.

F. Maintain telephone services that will enable the COR and his/her designated representative to have communication capability during business hours. The Contractor shall return all calls to the COR or his/her designated representative within 24 hours.

7 FACILITIES AND EQUIPMENT:

A. STORAGE – The Government may provide storage space under this contract. Identify equipment and material storage area needs during the pre-work conference.

B. GOVERNMENT FURNISHED SUPPLIES AND MATERIALS – The Government will not provide any supplies or materials under this contract.

a. The Contractor shall be responsible for restoring any Government facilities or property damaged as a result of the Contractor’s operations. Reasonable care shall be used to avoid damage to existing structures, equipment, facilities, and vegetation in the work areas. Any damage shall be repaired or replaced as directed by the COR, and at no cost to the Government. If the Contractor does not make such repairs or replacement, the cost thereof will be deducted from normal contract payments.

C. CONTRACTOR FURNISHED SUPPLIES AND MATERIALS – Furnish all labor, materials, equipment, transportation, and supplies necessary to fulfill the terms and conditions stated in this contract. The government shall make all reasonable required amounts of water and electricity available to the contractor from existing outlets and supplies. All other supplies and resources shall be provided by the Contractor.

a. The Contractor shall be responsible for all materials and equipment in their possession. The Contractor shall be accountable for any loss, damage, or misuse of government equipment being used during the performance of contractual services.

b. The Contractor shall be responsible for all keys issued. Keys are for the exclusive use of the Contractor while performing contractual services. The Contractor shall be liable for any costs associated with changing locks due to lost keys. Key loss shall be reported to the COR immediately. Keys shall be turned into the COR immediately upon completion of contractual services for each service day.

c. The disposal of all chemicals and supplies utilized for the performance of this contract shall be done off Government property and in accordance with the manufacturer’s instructions.

8 PERSONNEL:

A. MINIMUM MANPOWER REQUIREMENTS – Provide an adequate number of fully qualified personnel to perform the specified task order safely, properly and efficiently within the time constraints of this contract. The superintendent/project manager may also act as the quality control manager. The Contractor and their employees shall be required to adhere to the following standards of dress and conduct while performing work:

a. All Contractor personnel shall be identifiable by wearing a company uniform or clothing with the company name or logo conspicuously displayed on the clothing. Use personal protective equipment (PPE) as identified in EM 385-1-1, Safety and Health Requirements Manual. EM 385-1-1, Section 5 states, “The use of PPE is a control measure that is to be used only after a hazard evaluation identifies hazards associated with a particular job or activity, and it is determined that the hazards cannot be eliminated and/or controlled to an acceptable level through engineering design or administrative actions.” The use of proper PPE shall be determined at the pre-work conference following the Contractor’s submittal of an activity hazard analysis. All PPE shall be in compliance with the MSDS for chemical(s) being used.

b. The use of any Government property without proper authorization will not be permitted.

c. Comply with all CFR Title 36 regulations while performing services under this contract. The Contractor and their employees shall cooperate fully with all Corps of Engineers employees, local law enforcement and State agencies. The Contractor and their employees are not authorized to direct the work of other contractors.

B. QUALITY CONTROL – Ensure all work performed under this contract, including that performed by subcontractors, is in accordance with the specifications. To accomplish this, appoint in writing, subject to the approval of the Park Manager, a Quality Control Inspector (QCI).

a. Establish and maintain a complete QCP to assure the requirements of this contract are provided as specified. This QCP shall be forwarded to the COR at time of award. The COR will review the QCP and list any needed clarifications, and return to the Contractor for response, if necessary. The Contractor’s QCP shall include the following, or have incorporated into the document during performance of the contract, at a minimum:

i. An inspection plan covering as services as required by this contract. The inspection plan shall specify the areas to be inspected on either a scheduled or unscheduled basis, how often inspections shall be accomplished and documented, and shall specify the title of the individual(s) who will perform the inspections.

ii. On-site records of all inspections conducted by the Contractor noting necessary corrective actions taken. The Government reserves the right to request copies of any and/or all inspections.

iii. Active or established internal policies and procedures for updating equipment and procedures that may affect the performance of the contract.

iv. Methods for identifying and preventing deficiencies in the quality of service performed, before the level of performance becomes unacceptable, and organizational functions noting intermediate supervisory responsibilities and overall management responsibilities for ensuring total acceptable performance.

v. Identify the character, physical capabilities, certifications and ongoing training of each employee performing services under the contract.

vi. Maintain a log to account for all requests for immediate services. The log shall indicate the date and time of services, and description of results and completion of these services. The log shall also contain any complaints or problems, with procedures taken, to allow for corrections and/or elimination before effects caused interruption of performance of the contract.

9 INSPECTION:

A. GOVERNMENT QUALITY ASSURANCE – The government will monitor the Contractor’s services through scheduled, unscheduled, and in some cases, 100% inspections of the task order(s) as described.

B. CONTRACTOR PERFORMANCE DEFECTS, DEFICIENCIES AND REPERFORMANCE – The COR will determine and address Defects and Deficiencies in Contractor performance and re-performance. The government reserves the right to deduct portions of payment for any deficiencies in completed task order.

a. The COR will verbally advise or give a written inspection report to the Contractor of discrepancies the first time they occur and ask the Contractor to correct the deficiency. A notation will be made on the COR checklist of the date and the time the deficiency was discovered and the date and time the Contractor was notified.

b. If the Government creates any discrepancies in contract services, these will not be counted against the Contractor’s performance.

c. When the Contractor is not meeting the limits of satisfactory performance, a Contractor Discrepancy Report (CDR) will be issued to the Contractor. The seriousness of the failures should govern whether to issue a CDR at the end of the period, or as soon as the limits of satisfactory performance is exceeded.

d. When a CDR is issued for a service, the Contracting Officer must deduct from the periodic payment, a percentage of that payment as indicated in the Performance of Work Requirements Summary.

e. If the Contractor does not achieve satisfactory performance by the end of the next period or agreed suspense date, another CDR will be issued and the appropriate amount will be deducted from the periodic payment in accordance with the disincentive rate listed in the Performance of Work Requirements Summary.

f. A third CDR will be the cause for a Cure Notice. However, the Contracting Officer may issue a Cure Notice or Show Cause letter at any time he/she deems appropriate. Depending on the overall performance of the Contractor, an unsatisfactory reply to the Cure Notice should require a Show Cause letter to be issued, followed by consideration of termination of the contract.

10 PAYMENT

A. Submit invoices for payment upon completion of tasks as outlined in the pricing/payment schedule. Invoices shall include all necessary information, including any necessary supporting documentation required as part of this Performance Work Statement. All invoices must reflect the contract number W91238-XX-X-XXXX, see Block 2 of the Standard Form 1449. The final invoice must be marked “FINAL.” Provide copies of all invoices to the following:

11 TECHNICAL SPECIFICATIONS:

A. GENERAL SCOPE OF WORK

a. Planning – Provide all written permits which will comply with all OSHA, Federal and state standards.

Provide all permits to the US Army Corps of Engineers Pine Flat Park Manager two weeks prior to the commencement of contract start date. The COR and the Corps of Engineers Safety Officer will review the procedure listed in the documents and address all concerns with the contractor. Provide certifications as a Hazardous Paint Removal Contractor, Qualified Paint Applicator and as a Coating Thickness Gage Qualification contractor.

b. Testing and Abatement – The exiting coating should be assumed to contain lead and may contain other heavy metals, asbestos, and PCBs. The Contractor shall test all items receiving application of new coating for the presence of lead and/or heavy metals. The Contractor shall perform the necessary abatement as required by Specification 02 83 30 – Removal and Disposal of Coatings Containing Heavy Metals.

c. Surface Preparation – The Contractor shall prepare all surfaces that will receive new coating in accordance with Specification 09 96 20 – Coatings or the Coating Manufacturer’s recommendations; whichever is stricter or provides a superior final product. The Contractor shall provide all the necessary equipment, tools and safety materials to recoat all surfaces that will be receiving new coating.

d. Repair – The Contractor shall ensure that all steel surfaces will be free of rust and debris materials.

e. Application of New Coating Process – The contractor shall take the following the steps, in conjunction with Specification 09 96 20 – Coatings or the Coating Manufacturer’s recommendations; whichever is stricter or provides a superior final product:

i. Ambient Conditions – Apply new coating only to surfaces that are above the dew point temperature and that are completely free of moisture as determined by sight and touch. Do not apply new coating to surfaces upon which there is detectable frost or ice. Except as otherwise specified or recommended by the manufacturer, the temperature of the surfaces to receive the new coating and of air in contact therewith shall be not less than 9 degrees C (45 degrees F) during the new coating application nor shall new coating be applied if the surfaces can be expected to drop to 0 degrees C (32 degrees F) or lower before the new coating has dried to a reasonably firm condition or recommended by the manufacturer.

During periods of inclement weather, new coating application may be continued by enclosing the surfaces and applying artificial heat, provided the minimum temperatures and surface dryness requirements prescribed previously are maintained or recommendations followed by the manufacturer.

Do not apply the new coating to surfaces heated by direct sunlight or other sources to temperatures that will cause detrimental blistering, pinholing, or porosity of the film.

ii. Cleaning Prior to Primer Application - The COR will inspect all surfaces before the primer application to ensure that the surfaces are free of all foreign matter or rust. Any area not to the satisfaction of the COR will be re-cleaned to an acceptable level and quality.

iii. Application of Primer – Apply a commercial grade primer to meet or exceed Specification 09 96 20 – Coating or the Coating Manufacturer recommends. Adhere to the manufacturer’s required method of application and standards. In the areas where the spray coating application is inaccessible the Contractor will use a brush coating method. Check all spray surfaces for color uniformity and the finish coating thickness will be equal to or greater than Specification 09 96 20 - Coatings. Formulate and apply the new coating to produce a finished interior surface with optimum toughness and resistance to native environmental conditions as specified by the Coating Manufacturer.

iv. Primer Curing Time – Allow the primer coating to cure as specified by the manufacturer prior to any cleaning of the area.

v. Cleaning Prior to New Coating Application – Thoroughly clean the surfaces as recommended by the manufacturer after the primer coating and prior to the application of the new coating. All surface areas will be completely dry before the application of the new coating. The COR will inspect the surfaces after the primer application and prior to the new coating to ensure that the surfaces are free of all foreign matter or rust. Any area not to the satisfaction of the COR will be re-cleaned to an acceptable level and quality.

vi. Application of the New Coating – Apply the new coating to meet or exceed the Specification 09 96 20

– Coatings or the Coating Manufacturer recommendations. Adhere to the manufacturer’s required method of application and standards. In the areas where the spray coating application is inaccessible the contractor will use a brush coating method. Check all spray surfaces color uniformity and the finish coating thickness will be equal to or greater than Specification 09 96 20 - Coatings. Formulate and apply the epoxy coating to produce a finished interior surface with optimum toughness and resistance to conditions normally found on the service and emergency gates.

vii. New Coating Curing Time – Allow the new coating to cure as specified by the manufacturer prior to any cleaning of the coated area.

viii. Final Cleaning – After completion of the new coating, thoroughly clean the gate surfaces by a wash and rinse process followed immediately by air drying. Remove all wash water from the conduits area and dispose of at an approved waste disposal site. The final inspection of the conduits and gates shall be completed by the COR to ensure that all areas are free of all foreign materials. Any such area found not to be acceptable shall be re-cleaned to an acceptable level of quality.

f. Paint Job Warranty - The paint job shall include a warranty for the gate materials and coating. As a minimum, this warranty shall provide assurance against defects in material or workmanship and corrosion of the interior surface for a minimum period of one year.

State the warranty as follows:

“(Insert Contractor Name) hereby warrants that the gate surfaces shall be free from any defects in materials or workmanship for a period of one year.”

QUANTITY CONTRACT CLAUSE.

1. Prices must be submitted on all individual items of this Pricing Schedule. Failure to do so may be cause the proposal to be determined "unacceptable".

2. If a modification to a price based on unit price is submitted which provides for a lump sum adjustment to the total estimated price, the applications of the lump sum adjustment to each unit price in the Pricing Schedule must be stated. If it is not stated, the offeror agrees that the lump sum adjustment shall be applied on a pro rata basis to every unit price in the Pricing Schedule.

3. The offeror shall distribute his indirect costs (overhead, profit, bond, etc.) over all the items in the Pricing Schedule.

The Government will review all submitted Pricing Schedules for any unbalancing of the items. Any submitted Pricing Schedule determined to be unbalanced may cause the proposal to be determined "unacceptable".

4. The lump sum, "LS", line items above are not "estimated quantity" line items and therefore are not subject to the Variation in Quantity contract clause.

New Hogan Dam Service and Emergency Gates Coatings

Statement of Work

12 GENERAL:

B. SCOPE – Provide all materials, labor, transportation, and equipment necessary to perform the contract, as specified. Perform all job planning, programming, permitting, scheduling, administration, inspection, and management required to accomplish specified task order. Repair general corrosion and pitting corrosion on all service gate frames, slide gates and gate guide slots. Remove coating, repair and recoat all properly restored surfaces with a protective/rust preventive coating material. Accomplish all work in accordance with contract specifications and scope of work, all applicable laws, municipal codes, regulations and/or written directives issued by the Contracting Officer.

C. INTENT – Removal of all coatings, repair minor corrosion damage and repaint 8 slide gates (4 Service gates and 4 Emergency gates), guides, gate frames, air intakes (18 inches into the shaft), steel liner, access hatches with associated clips and bolts, and other associated equipment at New Hogan Dam Control Chamber, Calaveras County, Valley Springs, California. Primary focus will be on the metal work. Work shall be in accordance with the attached Scope of Work, Technical Specifications, Contract Clauses and Davis Bacon Wage Rates.

c. Service gate dimensions (approximate): 5 foot 8 inches wide x 10 feet high (downstream) and 8 feet high with 2 foot safety opening (upstream), three total (gates 1 through 3)

d. Emergency gate dimensions (approximate): 5 foot 8 inches wide x 10 feet high (downstream only), four total; the upstream face cannot be accessed because there the bulkhead gate will not operational at this location

e. Penstocks dimensions (approximate): 5 foot 8 inch wide x 5 feet high penstock passing though both the Service and Emergency gate, one total (gate 4)

i. Penstock Service gate dimensions (approximate): 5 foot 8 inches wide x 5 feet high (downstream) and 3 feet high with 2 foot safety opening (upstream), one total (gates 4)

f. Access hatch dimensions (approximate): 16 inch wide x 26 inch long oval opening with 20 clips and bolts per access hatch, four total

g. Steel liner dimension (approximate): 5 feet 8 inches wide x 10 feet high x 17 feet 9 inches long (located at the face of the Emergency gate and extending to the end of the access hatch), four total

D. LOCATION DESCRIPTION – Work site is located at 2713 Hogan Dam Rd., Valley Springs CA 95252

a. Mouth of outlet structure at flip bucket is approximately 23 feet wide for gates 1 through 3 and an additional 7 feet for gate 4.

b. The conduit outlets are 4 openings approximately 5 feet 8 inches wide by 10 feet high.

c. From the flip bucket to the gate is approximately 578 feet in length.

d. The distance between the Service and Emergency gate is approximately 9 feet 6 inches.

e. The steel liner extends an approximate 8 feet 3 inches downstream from the center of the Service gate. The steel liner extends an approximate 9 feet 6 inches from the center of the Service gate to the center of the Emergency gate. The steel liner spans the distance between the Service and Emergency gate, for a total distance of approximately 17 feet 9 inches.

f. The Service and Emergency gate maximum opening is approximately 5 feet 8 inches wide x 10 feet high.

g. The gates are numbered 1 to 4 from left to right looking upstream for both Service and Emergency gates.

h. Contractor to verify dimensions provided above prior to commencing work.

E. PERMITS – Without any additional expense to the government, provide any necessary/required licenses or permits for the completion of this contract. The Contractor shall comply with all Federal, state, county and municipal laws, codes, and/or regulations in connection with the performance of specified work.

F. SAFETY – Perform all work in accordance with safety requirements set forth in the current EM 385-1-1, Safety and Health Requirements Manual. The US Army Corps of Engineers, Sacramento District Safety office will review all Safety Plans, Confined Space Plans, Hot Work Permits, Ventilation Assessment Plans, Abrasive Blasting Requirement, Respiratory Protection Plan, Explosion Proof Equipment systems, Medical Surveillance Plan, Lead- Based Paint Worker Protection Plan, and Air Monitoring Test Plan.

G. ENVIRONMENTAL PROTECTION PLAN - In addition to the safety requirements, comply with Environmental Protection requirements. Prepare an Environmental Protection Plan that complies with the following required environmental protection criteria: Waste Classification, Handling, and Disposal Plan, Waste Manifest, Containment Plan, Visible Emissions Monitoring Plan, Water Quality Plan, Manufacturer's Product Data Sheets, Product Samples: Special Paint Formulas & Solvent and Thinners Information sheets.

H. CONTRACTING OFFICE REPRESENTATIVE – The Contracting Officer’s Representative (COR) for this task order shall be Tom Ehrke.

13 SPECIAL CONSIDERATIONS

I. The conduit is a Confined Space. Repair work shall comply with the safety and health requirements contained in the Corps of Engineer's SAFETY AND HEALTH REQUIREMENTS MANUAL, EM 385-1-1 for work in Confined Spaces, the National Electric Code, and in accordance with the applicable requirements of the GENERAL SAFETY REQUIREMENTS. The Contractor, in coordination with dam operations personnel, shall develop and utilize a check-in and check-out procedure to ensure the safety of personnel entering and leaving the conduit work area. Provide scaffold or other high work platform as needed.

J. Personnel accessing the work area:

a. Enter conduits from the downstream side of the service gates and travel approximately 578 feet upstream inside the 23 foot diameter tunnel to reach the work area then branching out to the individual gate conduits.

Standing and or flowing water may be present, not in excess of several inches. Slippery surfaces will be encountered.

b. Enter the conduits from the approximate 16 inch wide x 26 inch long oval service access hatch with radius ends within the control structure to reach the work area. The access hatch shall remain uncovered when personnel are present within the conduit to maintain communication, facilitation of equipment to the work area, and in the event of an emergency response action.

K. Equipment accessing the work area:

a. The Contractor shall provide a lift system to lower equipment and/or personnel into the flip bucket area.

Current access to this area is only by vertical ladder with personal fall protection required.

b. In addition to the requirements outlines in Appendix A of EM 385-1-1, the following is required:

i. Crane Critical Lift Plan. Prepare and sign weight handling critical lift plans for lifts over 75 percent of the capacity of the crane or hoist at any radius of lift; lifts involving more than one crane or hoist; lifts of personnel; and lifts involving non-routine rigging or operation, sensitive equipment, or unusual safety risks. Submit plans no later than 15 calendar days prior to on-site work and include the requirements of EM 385-1-1, paragraph 16.H. and the following:

1. For lifts of personnel, demonstrate compliance with the requirements of 29 CFR 1926.550(g).

L. Areas of work being performed:

a. The entire downstream face of the Service and Emergency gates will be available for work.

b. Approximately 8 feet of the available 10 feet of the upstream face of the Service gate will be available for work due to the required 2 foot minimum raised gate position necessary to access the upstream side of the gates as well as provide safety and emergency access for personnel within the conduit.

i. The upstream face of the Emergency gate will not be available for work. The bulkhead gate will not operational at this location which will prevent access to this location.

ii. When working on the upstream face of the Service gate, the Contractor shall provide a blocking device on both ends of the gate (1 gate at each end for a total of 2 devices) with enough capacity to support the gate in the event of hydraulic failure. The blocking devices shall not harm, damage or impede the gate guides or seals. The approximate weight of one gate is 20,000 pounds.

c. The gate frames, guides, steel liners, and access hatches with associated clips and blots for both upstream and downstream of the Service and Emergency gate shall be available for work.

i. The Contractor shall maintain a finished smooth surface at the access hatch to prevent leakage.

ii. The Contractor shall protect non-steel gate guides/seals or other equipment to avoid damage.

14 PERIOD OF PERFORMANCE:

M. WORK SCHEDULE – Anticipate construction work will be from 01 August 2012 to 30 November 2012. This would allow for unrestricted access to all service and emergency gates in the tower structure.

N. PREWORK CONFERENCE – Schedule a pre-work conference between the Contractor, his/her representatives, supervisors, workers, etc. and the COR, prior to the start of any work. Submit to the COR, all required reports, plans, permits, licenses, and other items outlined in this contract.

15 CONTRACTOR OBLIGATIONS:

O. LICENSING – Prior to starting work, submit a copy of current qualified contractor’s license, business licensing, registration with state, county, and local Business Administrations and/or any licenses required to comply with Federal, state and local laws. Submit the above to the COR at the pre-work conference.

P. EQUIPMENT – All equipment shall have all safety features intact and in operating condition. All equipment must be suitable for use on the described task order and is subject to the approval of the COR or his/her authorized representative.

Q. REPORT – Submit a man-hour exposure report to the COR prior to the completion of the contract.

16 CONTRACTOR RESPONSIBILITIES:

R. The contractor is liable for all damage to property, equipment, and landscape features in performance of this work. Immediately report any such damage to the COR and repair at Contractor's expense.

S. Remove all waste material generated in performing this contract from government property and dispose of at an approved waste disposal site. Comply with all Federal state and local guidelines as they pertain to the proper excavation, removal, and disposal of waste material generated during the performance of this contract specification.

17 FACILITIES AND EQUIPMENT:

T. STORAGE – The Government may provide storage space under this contract. Identify equipment and material storage area needs during the pre-work meeting.

U. GOVERNMENT FURNISHED SUPPLIES AND MATERIALS – The Government will not provide any supplies or materials under this contract.

V. CONTRACTOR FURNISHED SUPPLIES AND MATERIALS – Furnish all supplies and materials to be consumed or incorporated in the execution of this contract. Maintain an adequate supply of materials each working day as necessary for performing the task order as described.

18 PERSONNEL:

W. MINIMUM MANPOWER REQUIREMENTS – Provide an adequate number of fully qualified personnel to perform the specified task order safely, properly and efficiently within the time constraints of this contract. The superintendent/project manager may also act as the quality control manager.

X. QUALITY CONTROL – Ensure all work performed under this contract, including that performed by subcontractors, is in accordance with the specifications. To accomplish this, appoint in writing, subject to the approval of the Park Manager, a Quality Control Inspector (QCI).

19 INSPECTION:

Y. GOVERNMENT QUALITY ASSURANCE – The government will monitor the Contractor’s services through scheduled, unscheduled, and in some cases, 100% inspections of the task order as described.

Z. CONTRACTOR PERFORMANCE DEFECTS, DEFICIENCIES AND REPERFORMANCE – The COR will determine and address Defects and Deficiencies in Contractor performance and re-performance. The government reserves the right to deduct portions of payment for any deficiencies in completed task order.
