A. All applicable requirements of other portions of the Contract Documents apply to the Work of this Section, including but not limited to Division 1, General Requirements.

B. Definition: COR - Contracting Officer's Representative.

1.2 PROJECT DESCRIPTION

A. SECTION INTENT: This section is intended to provide a summary of the project and the various elements of work associated with it. This summary should be used in conjunction with other Specification sections and the construction Drawings. This section does not provide the technical detail for particular Work Items, but describes the work as a whole, providing an overall perspective to the separate tasks and their interrelationships.

B. GENERAL: The purpose of the work shown on these plans is the modification of the Union Street Dam including demolition of the existing dam structures and appurtenances, construction of new weirs, bypass channel, fish-sorting channels, research building, pedestrian bride, park areas, and restoration after construction. The work generally includes, but is not limited to concrete removal, removal and disposal of fill/sediment materials, construction of the bypass and fish-sorting channels, construction of instream habitat including channel bed material and boulders, construction of shown bridges/buildings and park features, and restoration of the river bank and other disturbed areas within the project limits. This work will be conducted while the contractor actively manages water levels on the Boardman River and flow over/through the Union Street dam within the constraints specified in the drawings and specifications.

The work to be completed consists of six major components, and includes installation and maintenance of erosion and sedimentation control measures for each:

1. The demolition and removal of the Union Street Dam embankment, intake structure, spillway, and fish ladder and construction of new concrete low flow and labyrinth weirs. (Latest dam safety inspection report is attached at the end of this section.)

2. Construction of concrete experimental fish sorting channel and headworks structure.

3. Construction of naturalized bypass channel and in-channel features and new concrete low flow and labyrinth weirs.

4. Construction of downstream tailwater entrance pad.

5. Construction of Research Building on north bank of Boardman River including new utilities, electrical and communications.

6. Construction of park features, landscaping design, and pedestrian bridge, including site security features, signage, communications, and site lighting.

SECTION 01 11 00 Page 2

C. DEMOLITION AND DAM REMOVAL SUMMARY:

1. Phased demolition and Dam Removal generally consists of:

a. Prior to starting demolition, construct water main to relocated position, connecting to existing city mains in the alley and in Union Street.

b. Complete testing on the new water main.

c. Place sheet pile upstream of Union Street Dam to a depth of 60-ft from noted Top of Wall (TOW) elevation 594 with an overtopping section at elevation 591.

d. Install temporary riprap overtopping protection immediately downstream of the overtopping section. [At this time all river flow will be through the ten (1) existing culverts in the Union Street Dam toward the north shore of the Boardman River. Operate the existing stoplogs in front of the culverts as indicated in the drawings so as to maintain existing water surface conditions for varying flow events.

e. Complete any dewatering operations required within sheet pile area including providing for any bypass pumping or routing of any existing stormwater outlets that are within the cofferdam area.

f. Excavate and dispose of any contaminated sediments within the cofferdam area upstream of the existing Union Street Dam embankment.

g. Construct the concrete low flow and labyrinth weirs and southern wall of headworks.

h. Place sheet pile downstream of Union Street Dam to a depth of 48.8-ft from noted TOW elevation 586.8.

i. Demolish existing dam features within limits of Phase 1 sheet pile, including existing auxiliary spillway, fish ladder, concrete intake structures, and water main.

j. Construction temporary channel to a minimum width of 45-ft from the installed median sheet pile wall and install temporary R-700 riprap armoring to be utilized for the duration of Phase 1 and Phase 2.

k. Construct downstream Articulated Concrete Block entrance pad to the extent possible.

l Demolish existing building and foundation on the south side of the river.

D. CONSTRUCTION OF FISH SORTING CHANNEL AND HEADWORKS:

a) Realign center sheet pile wall to match southern headworks wall.

b) Bulkhead gate openings on the southern headworks wall temporarily

c) Remove downstream sheet pile cut off wall placed during Phase 1.

d) Remove upstream sheet pile cut off wall placed during phase 1 and replace sheet pile as part of Phase 2 sheet pile from southern wall of headworks tie in point to the north shoreline. [At this time river flow would be passing through the newly constructed weirs and temporary channel.

e) Place Phase 2 downstream sheet pile cut off wall from center wall to the north shoreline.

f) Complete dewatering operations required within sheet pile area and provide for bypass pumping or routing of any existing stormwater outlets that are within the cofferdam area.

g) Excavate and dispose of any contaminated sediments within the cofferdam area upstream of the existing Union Street Dam embankment.

h) Construct headworks structures, install upstream stoplogs, and construct the concrete fish sorting channel as able between headworks and existing dam features.

i) Upon headworks structure reaching sufficient cure time and installation of stoplogs, demolish existing dam features within limits

SECTION 01 11 00 Page 3 of Phase 2 sheet pile including dam embankment, remaining portion of existing water main, tech existing culverts and associated inlet structures.

j) Construct remaining concrete fish sorting channel and downstream ACB entrance pad to the extent possible.

k) Install gates at headworks and downward tilting gates at downstream end of fish sorting channel.

l) Construct the pedestrian bridge abutment on the north side of the river.

m) Remove all temporary sheet piling.

EMERGENCY ACTION PLAN

UNION STREET DAM

Boardman River Traverse City, Michigan

Revised: December 14, 2018

I. PURPOSE.

The purpose of this Emergency Action Plan is to define responsibilities and identify procedures to be followed by the City of Traverse City and other agencies in response to emergencies at the Union Street Dam. An emergency at the dam would include a hazardous situation, an imminent failure, or an actual failure of a dam structure or earthen dike that could lead to an uncontrolled release of flow and could result in property damage or loss of life.

II. NOTIFICATION FLOWCHARTS

Notification Flowcharts list contact information for personnel and agencies that should be notified when an emergency situation develops at the Union Street Dam. There are two flowcharts representing sequences of notification in two possible failure situations:

Condition A. Failure is imminent or has occurred.

General Definition: A determination has been made that there is no longer anytime available to attempt corrective measures to prevent failure.

Condition B. Potentially hazardous situation is developing.

General definition: Some amount of time is still available to take action that may prevent failure.

The Notification Flowcharts appear on pages 3 and 4 of this Emergency Action Plan.

III. DESCRIPTION OF DAM

The Union Street Dam is located on the Boardman River between Union Street and Cass Street in the City of Traverse City. It is composed of a number of structures as well as the surrounding grounds. The Dam consists of approximately 250 lineal feet of earth embankment. Within the embankment are two spillways and a fish ladder. The principal spillway consists of a concrete overflow section with five 10.5-foot wide bays. Each bay is connected to two 48-inch diameter corrugated metal outlets pipes (for a total of ten) that extend through the embankment. Each pipe has a control gate and operator on the upstream end of the pipe. Immediately upstream of the outlet pipes are steel bar trash racks. Stop logs in each of the five bays control the normal water surface in Boardman Lake.

The auxiliary spillway consists of a concrete inlet section and overflow structure with two 11-foot wide stop log bays. Downstream from the stop log section are trash racks that are mounted on the concrete headwall. Two 48-inch diameter corrugated metal pipes extend through the embankment and outlet into a separate channel connected to the Boardman River downstream.

A steel sheet pile wall connects the two spillways on the upstream face of the embankment.

The outlet structure for the principal spillway consists of a concrete headwall and concrete apron with a steel sheet pile cut-off-wall. The outlet structure for auxiliary spillway consists of concrete headwall and concrete slab erosion protection on the banks of the outlet channel.

A six-foot wide concrete fish ladder connects to the inlet structure of the auxiliary spillway and outlets at the Boardman River to the left of the principal spillway. The fish ladder has several weir sections that step down from the lake elevation to the tail water elevation in the river.

The normal headwater elevation at the Union Street Dam is 588.5 feet and the normal tail water elevation is 581.5 feet. The principal spillway has a crest elevation of 592 feet. The earth embankment has a crest elevation of 595 feet.

IV. EMERGENCY DETECTION, EVALUATION, AND CLASSIFICATION

A. Detection:

The most likely method of detection of an emergency situation at the Union Street Dam is from an observer or reliable source. A phone call may be received indicating that an abnormal, hazardous, or emergency situation exists.

Another method of detection would be notification from a City employee working on or near the dam. Routine on-site inspection of the dam by Parks and Recreation personnel is conducted. Employees may also be dispatched to the dam to observe, monitor, and report on conditions during storms or high runoff periods.

Conditions or events that may cause an abnormal, hazardous, or emergency situation include gate failure, structural failure, dike failure, heavy continuous rainfall or runoff, earthquake, terrorism, etc.

B. Evaluation:

Information and data received must be evaluated so as to assess the severity and magnitude of the situation.

The rate of change of headwater and tail water levels and weather conditions are two important factors to consider. In the case of an on-site observation, either by a reliable observer or employee, an immediate decision can reliably be made as to the severity and magnitude of the emergency. An employee may also be dispatched to the dam to observe, monitor, and report on conditions when circumstances indicate that a potential for a hazardous situation exists.

C. Classification:

Two emergency classifications have been developed:

Condition A. Failure is imminent or has occurred.

Condition B. Potentially hazardous situation is developing.

Once detection and evaluation phases are completed, a determination as to whether Condition A or Condition B exists will be made. General definitions of these conditions are provided in Section II. Responsibilities regarding each condition are identified in Section V. An early decision and declaration is critical to maximize available response time.

V RESPONSIBILITIES UNDER THE EMERGENCY ACTION PLAN

A. General Responsibilities: The Union Street Dam is owned by the City of Traverse City.

Operation and maintenance of the dam is the responsibility of the Director of Public Services.

The Parks and Recreation Department is designated to provide manpower and funding to execute operations and maintenance projects on the dam with backup support from other departments within the Department of Public Services.

B. Responsibility for Notification: Upon becoming aware that abnormal headwater and tail water conditions exist, the Department of Public Services (DPS) will evaluate the available information, make a determination regarding the severity of the problem, and initiate “Condition A” or “Condition B” response procedure, if appropriate.

The Public Services Department, or its agent, will take appropriate action to determine the nature of the failure or dangerous situation. If notification has been received from an observer, the DPS will obtain the name, address, and telephone number of the caller, the information about the type and time of failure. If necessary, the DPS will need to inspect the dam. They will then inform the appropriate authority of the situation, and continually monitor the situation closely. The DPS will also contact Central Dispatch, who will contact local agencies and keep them informed of the situation.

C. NOTIFICATION PROCEDURES

Condition A. Failure has occurred or is imminent

A Notification Flowchart delineating the contacting responsibilities for the various individuals or agencies involved in a rapidly developing or instantaneous failure at the Union Street Dam is presented on page 2 of this plan.

The dam shall be considered about to fail if a serious structure deficiency or an uncontrolled flow of water that cannot be corrected or contained develops resulting in imminent failure, or if the structural deficiency or uncontrolled flow has the likely potential of increasing to catastrophic proportions causing failure to part of the dam embankment. Over topping can occur because of flooding and/or incorrect operation of the dam.

The failure would most likely be detected by employees of the City of Traverse City becoming aware that an abnormal headwater or tailwater condition exists that is likely to cause imminent failure; the Owner/operator will initiate the "Condition A" warning procedure outlined below.

1. Union Street Dam Owner/Operator Upon visual detection, or upon receiving a call from a reliable source that there has been or is about to be a dam failure at Union Street Dam, the Owner/operator is to notify the contacts listed below by the most rapid means possible. The following information is to be relayed:

4. Other Actions

After completion of the warning procedures, the owner/operator will assess the damage to the dam, with the appropriate authority, and formulate a plan for emergency repairs.

Condition B. Warning Procedure if Potentially Dangerous Situation is Developing

A Notification Flowchart delineating the contacting responsibilities for the various individuals or agencies involved in a rapidly developing or instantaneous failure at the union Street Dam is presented on page 5 of this plan.

A potentially dangerous situation is present when a failure may develop, but preplanned actions taken during certain events (such as major floods, earthquakes, evidences of piping, etc) may prevent or mitigate failure. Even if failure is inevitable, more time is generally available to issue warnings and/or take preventable actions.

Visual inspection is used for continuous headwater and tailwater monitoring. Upon becoming aware that an abnormal headwater and tailwater condition exists, the owners/operators will make the determination with regard to the severity of the problem and initiate the appropriate response procedure. If the Owner/operator believes that failure is not imminent but a potentially dangerous situation is developing, they will implement "Condition B" warning procedure.

The notification procedures are as follows:

1. Union Street Dam Owner/Operator (Department of Public Service)

a. Upon detecting through visual surveillance, or upon receiving a call from a reliable source that there is a potentially dangerous situation at Union Street Dam, the Owner/operator will immediately inspect the situation.

b. If failure is not imminent, the Owner/operator will inform Central Dispatch and will closely monitor the situation. Central Dispatch will coordinate the City Warning Agencies to keep them informed of the situation as it develops.

Use "Condition B" (Page 3) flowchart for telephone numbers.

c. If the Owner/operator determines that a dam failure cannot be prevented, he/she will contact the following:

(1) Grand Traverse County/Traverse City Communications/Warning officer

Central Dispatch 911 (24 hours)

(2) Union Street Dam EAP Coordinator Frank Dituri Office: (231) 922-4900 ext. 116 Cell: (231) 866-1851

The following information is to be relayed to the above contacts:

Name of dam Type of failure Time of failure Name, address and phone number of person calling

(3) Immediately after making the two contacts listed, the

Owner/operator shall inspect the downstream areas for controlling flow.

2. Central Dispatch

Immediately after being notified by the Owner/operator, the Central Dispatcher will notify the following that a potentially dangerous situation is developing:

a. Emergency Management Coordinator Gregg Bird

Business: (231) 995-6059 Cell: (231) 590-2373

The Emergency Management Coordinator will be responsible for organizing and coordinating warning procedures with appropriate local agencies. If the Emergency Management Coordinator is not available, the Traverse City Fire Department will maintain a state of readiness for implementation of the Grand Traverse County warning and evacuation procedures should such action become necessary.

b. Traverse City Fire Department 24 hours via Central Dispatch

c. State Police 24 hours via Central Dispatch

d. Traverse City Police Department 24 hours via Central Dispatch:

e. Grand Traverse Sheriff 24 hours via Central Dispatch

If a potentially dangerous situation is developing at Union Street Dam the inundation maps for the potential areas subject to flooding are shown in Appendix A of this Emergency Action Plan. In the event of an imminent or actual failure, potentially affected property owners between the Union Street Dam and Grand Traverse Bay can be contacted by using the CodeRED notification system.

3. Other Actions

After completion of the warning procedure, the Owner/operator will assess the damage to the dam, with help from the proper authority, and formulate a plan for emergency repairs and/or mitigation of damage.

D. Evacuation Responsibility: The Department of Public Services is responsible for notifying appropriate emergency management officials (Central Dispatch) when major flooding is anticipated, a dam failure is imminent or has occurred, or a potentially hazardous situation is developing. Central Dispatch and the Emergency Management Coordinator will manage the evacuation of people endangered by any failure. If time does not allow for the involvement of Central Dispatch, the Department of Public Services may notify people directly or with the assistance of other emergency agencies.

E. Responsibility for Termination and Follow-Up: The EAP coordinator will ensure that DPS personnel are monitoring the situation at the dam and keeping Central Dispatch informed of developing conditions at the dam from the time that an emergency starts until the emergency has been terminated. Central Dispatch will contact local agencies and keep them informed of the situation.

The Director of Public Services will be responsible for declaring that the emergency at the dam is terminated. Once notified by the DPS staff that the emergency is terminated, Central Dispatch will notify all other emergency agencies.

Within 60 days after the emergency has been terminated, the EAP Coordinator will conduct a follow-up evaluation, including input from other outside participants. The evaluation will be documented in a written report.

F. EAP Coordinator Responsibilities:

1. Conduct necessary training.

2. Conduct an annual plan review, prepare revisions as required, and distribute revisions to all plan holders.

3. Conduct an annual readiness teat.

IV. PREVENTIVE ACTIONS

A. General Provisions for Surveillance: The headwater and tail water levels are monitored routinely through visual checks by the Department of Public Services. During periods of actual or projected high flows, a DPS employee with a two-way radio may be stationed at the dam.

B. Response During Periods of Darkness: Presently there is no lighting available for illumination of the spillway at the dam. Such lighting would have to be provided by vehicles or portable means. The notification Flowcharts includes 24-hour telephone numbers, which can be used day or night.

C. Response During Periods of Adverse Weather: During periods of actual or forecasted high flows, the visual surveillance will be increased to check the dam. In the event of a failure, the Owner/operator could contact Central Dispatch (communications/warnings offices) by telephone.

A rapidly developing dam failure could inundate some downstream low areas and parking lots.

D. Availability and Use of Alternate Systems of Communication: The principle means of communication would be the commercial telephone system. In the event of an emergency, the emergency operations center will be the communications control point. The emergency operation center is equipped with one radio console and outgoing phone lines.
