MEMORANDUM FOR Questionnaire POCs SUBJECT: Request for Source Selection Information
1. The Mission and Installation Contracting Command (MICC) at Joint Base Lewis-McChord (JBLM), Washington is in the process of selecting a contractor for Fort Bliss Grounds Maintenance Services at Fort Bliss, Texas. Request for Proposal Solicitation Number W911S8-18-R-0014. Detailed information regarding this solicitation may be found at www.fbo.gov.
2. One of the considerations in proposal evaluation is the verification of the offerors' past performance on contracts, which reflect the Offeror's ability to perform on the proposed effort. The Government depends on information received from agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the Offeror's performance on those contracts.
3. The Government’s areas of interest in the Offeror are summarized in the enclosed questionnaire. Request your written response in accordance with FAR Clause 52.212-1 Addendum, Paragraph (3)(iii). This schedule will allow sufficient time to analyze the data prior to the start of negotiations. It is important that you return the completed questionnaire directly to the Government.
4. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and "emailed" to jose.f.diaz1.civ@mail.mil. Please call (253) 966-3483 prior to transmission or if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.
5. Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
