Identification of Contractor Employees (1.6.18)The Contractor (to include subcontractors) shall provide each employee an Identification (ID) Badge, which includes at a minimum, the Company Name, Employee Name and a recent color photo of the employee, prior to them entering Government facilities. IAW PWS paragraph 1.6.18

A listing of issued identification cards shall be furnished to the COR prior to the listed employees entering Government facilities and updated as needed to reflect Contractor and Subcontractor personnel changes. 100% Incentive:

Positive CPARS narrative. Full payment for services provided.

Disincentive:

Non-conforming services will result in a Corrective Action Request (CAR) being issued and a potential negative CPARS narrative.

Lack of supplying ID Badges will inhibit the ability to access the facility to perform services.

The KO may also reduce payment to reflect the value of the services performed if re-performance is not possible, or may have the identified non-conforming service performed and charge the contractor for any related cost incurred.

Use of herbicides (5.1.1) The contractor shall not use any chemicals in the performance of this contract with the exception of Paragraphs 5.3.5, 5.6.1, and 5.6.2, with the prior approval of the COR through the 88th RD Pest Manager.

In accordance with Lawn Type and Drawing. 100% Incentive:

Positive CPARS narrative. Full payment for services provided.

Disincentive:

Non-conforming services will result in a Corrective Action Request (CAR) being issued and a potential negative CPARS narrative.

The KO may also reduce payment to reflect the value of the services performed if re-performance is not possible, or may have the identified non-conforming service performed and charge the contractor for any related cost incurred.

Service Tickets (5.7) The contractor shall provide a service ticket for all grounds maintenance services provided each time services are performed on location and shall be submitted with the monthly invoice. The service ticket shall indicate which services were performed on which date and shall have a signature from the Contractor POC and COR or DR. COR or DR signature does NOT indicate acceptance of services, but simply indicates that the Contractor was on site working that day. The government still holds the right to perform surveillance as indicated in PWS 1.6. A sample service ticket for use is included in Part 7.4 of the PWS. The Contractor may use their own service ticket provided it contains which services were completed, the date they were completed and signature lines for the Contractor and COR or DR. Within 30 days of work being performed, the Contractor shall enter the service ticket into the appropriate invoicing system or email the service ticket to the COR.

General Services (5.7.1) A service ticket shall be provided for any Type 1, 2, 3, or 4 lawn maintenance service performed that day. The Contractor shall enter the service ticket into the appropriate invoicing system.

In accordance with Lawn Type and Drawing. 100% Incentive:

Positive CPARS narrative. Full payment for services provided.

Disincentive:

Non-conforming services will result in a Corrective Action Request (CAR) being issued and a potential negative CPARS narrative.

The KO may also reduce payment to reflect the value of the services performed if re-performance is not possible, or may have the identified non-conforming service performed and charge the contractor for any related cost incurred.

Spring Lawn Clean-up service ticket (5.7.2) A separate service ticket shall be provided upon completion of Initial or Spring Clean-up, aeration, and fertilizer application. The Contractor shall enter the service ticket into the appropriate invoicing system (or email to the COR if unable) that includes as a minimum all the information on the 88th RD Universal Pest Management Application Reporting Form (see Section 7.1) for the completion of fertilizer application.

In accordance with Lawn Type and Drawing. 100% Incentive:

Positive CPARS narrative. Full payment for services provided.

Disincentive:

Non-conforming services will result in a Corrective Action Request (CAR) being issued and a potential negative CPARS narrative.

The KO may also reduce payment to reflect the value of the services performed if re-performance is not possible, or may have the identified non-conforming service performed and charge the contractor for any related cost incurred.

Fall Lawn Clean-up service ticket (5.7.3) A service ticket shall be provided upon completion of Fall clean-up. The Contractor shall enter the service ticket into the appropriate invoicing system (or email to the COR if unable).

In accordance with Lawn Type and Drawing.

100% Incentive:

Positive CPARS narrative. Full payment for services provided.

Disincentive:

Non-conforming services will result in a Corrective Action Request (CAR) being issued and a potential negative CPARS narrative.

The KO may also reduce payment to reflect the value of the services performed if re-performance is not possible, or may have the identified non-conforming service performed and charge the contractor for any related cost incurred.

Initial Lawn Clean-up service ticket (5.7.4) A service ticket shall be provided upon completion of Initial clean-up. The Contractor shall enter the service ticket into the appropriate invoicing system (or email to the COR if unable).

In accordance with Lawn Type and Drawing. 100% Incentive:

Positive CPARS narrative. Full payment for services provided.

Disincentive:

Non-conforming services will result in a Corrective Action Request (CAR) being issued and a potential negative CPARS narrative.
