Re: ES# 12-6195 – Limited Asbestos Inspection of Buildings 310 & 384 Located at 397 Armed Forces Drive in Grand Prairie, Texas.

Ms. Cooper:

ERI Consulting, Inc. performed a Limited Asbestos Inspection of the above referenced buildings on May 30, 2012. Mr. Kevin M. Sonnonstine of ERI, licensed through the Texas Department of State Health Services (TDSHS) as an Asbestos Consultant (Lic. # 10-5641) performed the inspection. The sampling involved the collection of building materials which could be disturbed during planned renovation activities. The inspection was limited to readily accessible suspect asbestos-containing materials (ACM) only as directed by a representative from your company. These areas include Shower Room 151 from Building 310 and Men’s RR 102A & Women’s Restroom 102B from Building 384 (herein referred to as the Inspected Areas).

The initial phase of the inspection involved the identification of the homogeneous areas which (1) could be disturbed during planned renovation activities and (2) are suspected of containing asbestos fibers. Each homogeneous area was quantified to determine the approximate amount of suspect material present. These areas were then divided into sectors and samples collected in a statistically random manner. A sufficient number of samples were collected in each homogeneous area based on the amount of suspect material present. The inspection was performed in accordance with applicable TDSHS, OSHA and EPA Standards. A total of 27 samples were collected.

All samples were delivered to the TDSHS licensed laboratory of ERI Analytical (License No. 30- 0007) immediately following the field inspection, and analyzed by Polarized Light Microscopy (PLM) in accordance with 40 CFR 763 Subpart F Appendix A. This asbestos inspection report documents the field inspection and laboratory findings for the following homogeneous areas located in the Inspected Areas only:

Building 310

1. Homogeneous Area A – Ceiling Texture and Drywall

2. Homogeneous Area B – Ceiling Tape Bedding Joint Compound

3. Homogeneous Area C – Wall Ceramic Tile Mortar

4. Homogeneous Area D – CMU Block Texture

5. Homogeneous Area E – Floor Ceramic Tile Mortar

Building 384

6. Homogeneous Area F – Wall Texture and Drywall

7. Homogeneous Area G – Wall Tape Bedding Joint Compound

8. Homogeneous Area H – Wall Ceramic Tile Mortar

9. Homogeneous Area I – Lay-in Ceiling Tiles (2’ x 4’)

By definition, an asbestos-containing building material is a material that contains greater than one percent (>1%) asbestos fibers. Of the samples collected, Homogeneous Area B from Building 310 was confirmed to be asbestos containing. Prior to renovation or demolition activities which would disturb these materials, they must be removed and disposed of in the regulated manner by a TDSHS licensed asbestos abatement contractor, under the supervision of a TDSHS licensed consultant. Or the asbestos-containing materials (ACM) may remain in place as long as they are kept in good condition and are not disturbed. As of this inspection, Homogeneous Area B was in good condition with potential for damage.

Current EPA and TDSHS regulations require that an asbestos survey be performed prior to disturbing building materials (demolition or renovation) in a public building. TDSHS regulations require that a notification be filed only if structural members are removed (demolition), or regulated asbestos-containing materials are disturbed. TDSHS does not require nor desire copies of the survey report to be filed with them. TDSHS regulations also require that a copy of the asbestos survey be kept on the job site at all times during renovation activities. Local building inspectors may also require an asbestos inspection to be performed prior to issuing permits. This letter should meet those requirements.

Limitations: This inspection was limited to readily accessible suspect ACMs from Buildings 310 and 384 only which could be disturbed during planned renovation activities. Limited destructive sampling was performed as part of this inspection; therefore, there may be other materials located throughout the buildings which contain asbestos but were not sampled. Such materials, if present, could be located in other parts of the buildings; or within other parts of the Inspected Areas such as within wall cavities, under floor coverings, within pipe chases, or other inaccessible areas. Such materials would only be encountered during extensive renovation and/or demolition activities. If additional materials, not identified within this report, are uncovered, sampling should be conducted to confirm or deny the presence of asbestos or the materials must be assumed to be asbestos-containing and handled accordingly.

We enjoyed serving you on this matter. Please find enclosed the following documentation which should be kept as part of your permanent records: (1) Bulk Sample Log/Chain of Custody, (2) Laboratory Analysis Report, (3) Sample Location Diagram, and (4) ERI Licensing and Accreditation.

If you have any questions concerning this inspection, please feel free to contact us at (817)560-1613.

Sincerely, Kevin M. Sonnonstine TDSHS License # 10-5641, Exp. 06/28/2013
