1. Each offeror shall complete and include the Total Evaluated Price Worksheet in their response
(i.e., proposal) to the solicitation. This information shall be provided in an Excel Spreadsheet format and include all formulas. The offeror shall indicate the hourly price for each labor category listed on the Pricing Worksheets for each year of the Five Year Ordering Period. The labor category hourly rates provided are to be fully burdened.
2. Should a contract be issued to the offeror, the proposed labor rates provided on these Total
Evaluated Price Worksheets shall be incorporated into the resulting contracting and shall serve as the maximum rate allowable for that labor category when pricing task orders.
The labor categories included in the Total Evaluated Price Worksheet represent what will typically be required to perform the services under the individual task orders. However, the list is not all inclusive and there may be circumstances when individual task order requirements involve labor categories not contained in the Total Evaluated Price Worksheet. In these instances, the contract will be modified to incorporate the additional labor category(ies) into the
Total Evaluated Price Worksheet. The rates incorporated will be based upon the rates used by the contractor in pricing the competitively awarded task order.
3. The rates proposed for each performance period cannot be adjusted except for instances where the labor categories are subject to the Service Contract Act and a revision is permitted under the provision of FAR Clause 52.222-43. Therefore, offerors are cautioned to adjust for inflation where warranted.

If there are risks in the Offeror’s proposed technical approach, the Offeror shall identify the risks and provide their plan to mitigate the identified risks.
L.4. VOLUME 2: FACTOR 2 – Past Performance.
L.4.1. The Offeror shall provide documentation outlining the offeror’s past performance with contracts, as a prime or subcontractor, which is the same or similar in nature, size, and complexity to the services being procured under this solicitation. The submittal shall include rationale supporting your assertion of relevance and how it was determined that the work performed previously was the same or similar in nature, size, and complexity to the work specified by this solicitation.
L.4.2. The Past Performance evaluation will be accomplished by reviewing aspects of an Offeror's recent and relevant Past Performance, focusing on and targeting performance which is relevant to the effort as it relates to the services being procured under this solicitation.
L.4.3. Past Performance information described herein is required on the offeror and all subcontractors, teaming partners, and/or joint venture partners (collectively, partners) proposed to perform 25% or more of the proposed effort based on the total proposed price. The offeror shall submit, along with the information required in this paragraph, a consent letter, executed by each partner, authorizing release of adverse past performance information to the offeror so the offeror can respond to such information. For each identified effort for a commercial customer, the offeror shall also submit a client authorization letter, authorizing release to the Government of requested information on the offeror's performance.
L.4.4. The offeror shall include documentation regarding their relevant past performance as it directly relates to the work being procured under this solicitation. The offeror SHALL NOT go back any farther than the last three (3) years from issuance of the solicitation for the submitted data. The past performance data shall document a successful history of past contract performance.
L.4.5. In conducting the Past Performance evaluation, the Government reserves the right to use the information provided in the offeror’s Past Performance Volume, Past Performance Questionnaire, and information obtained from other sources, such as the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), Contractor
Performance Assessment Reporting System (CPARs), or other databases; and may be obtained from other sources available to the Government, such as the Defense Contract Management Agency (DCMA) and interviews with
Program Managers, Contracting Officers, and Fee Determining Officials. The past performance evaluation team will review this past performance information and determine the quality and usefulness as it applies to the performance confidence assessment. See FAR 15.101-2(b)(1) for treatment of past performance relative to small business. Offerors are reminded that both independent data and data provided by offerors in their proposals may be used by the Government to evaluate offeror past performance. However, the burden of providing thorough, complete, and current past performance information as requested in this paragraph remains with the offerors.
Proposals that do not contain the information requested by this paragraph risk rejection or a less than acceptable performance rating by the Government. All past performance comments received will be taken into account and could affect the overall rating. The overall past performance evaluation is a subjective decision based on the whole of all data received.
L.4.6. Submission Requirements. The offeror shall submit a Past Performance Volume containing the following:
•Table of Contents
• Summary Page describing the role of the offeror and each subcontractor, teaming partner, and /or joint venture partner that the offeror is required to provide Past Performance Specific Relevant Contract Reference Sheets for the past performance experience IAW Attachment 4.
• Consent Letters executed by each partner, authorizing the release of past performance information so the offeror can respond to such information (A sample consent letter is attached as Attachment 4).
• Client Authorization Letters for each identified effort for a commercial customer authorizing release to the
Government of requested information on the offeror’s performance.
• Organization Structure Change History: Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider present and past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the
Government receives information from other sources.
• Specific Relevant Contracts Format - Sheets are limited to 3 pages per reference. The offeror shall provide documentation outlining the offeror’s past performance with contracts, as a prime or major subcontractor, which is the same or similar in nature, size, and complexity to the services being procured under this Solicitation. The submittal shall include rationale supporting your assertion of relevance and how it was determined that the work performed previously was the same or similar in nature, size, and complexity to the work specified by this solicitation. Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort, and to what aspects of the proposed effort they relate. Offeror’s past performance shall demonstrate that it has successfully staffed and performed multiple contracts and/or task orders, simultaneously.
Past Performance Questionnaire - See Attachment 4, Past Performance Questionnaire. For the contracts identified on each Specific Relevant Contract Reference, the offeror shall forward a copy of the Past Performance
Questionnaire to the points of contact responsible for monitoring performance under such contracts. The points of contact shall return the questionnaires directly to the KO, Ms. Gina Dedmon (gina.d.dedmon.civ@mail.mil) and Ms.
Bethany Rosser (bethany.r.rosser.civ@mail.mil) not later than 12 October 2017. Any questionnaires not returned directly to the KO will not be reviewed and evaluated. The information contained in the questionnaires will be used to evaluate the offeror’s past performance. New entities that have no past customers shall annotate on the Summary
Page that they have had no previous clients and that the minimum number of questionnaires cannot be provided.
L.5. VOLUME 3: FACTOR 3 – Price.
L.5.1. This volume shall contain all required pricing information described below. The Offeror shall not include pricing information in Volumes 1, 2, or 4 of its proposal. The Pricing proposal (with the exception of the Pricing
Worksheets) shall be in Microsoft Excel and shall be compatible with Microsoft Office 2013. Microsoft Excel spreadsheets shall not be read only or password protected. All formulas, lookup tables, and links shall be intact, and no links shall exist to files not included with the submission. Spreadsheets shall not contain hidden worksheets. PDF mailto:bethany.r.rosser.civ@mail.mil or flat files will not be considered adequate. Failure to comply with these formatting requirements may result in rejection of your proposal. Offerors shall perform a virus check prior to proposal submittal. All pricing shall be complete and error free.
L.5.2. It is expected that all contracts awarded will be based upon adequate price competition; therefore, the Offeror is not required to provide certified cost/pricing data with its proposal. If, after receipt of the proposals, the KO determines that adequate price competition does not exist IAW FAR 15.403-1, the Offeror shall provide certified cost/pricing data as requested by the Contracting Officer. Similarly, if the contracting officer determines a fair and reasonable price cannot be established with price analysis alone, the offeror may be asked, and shall then provide, other than certified cost or pricing data IAW FAR 15.403-1(b) and/or FAR 15.404-1(a)..
L.5.3 Contractor Pricing for MAIDIQ. The Offeror’s proposal shall be for a five year ordering period. The Offeror is required to complete Attachment 1 – Total Evaluated Price Worksheets.
