About this file

This is a Past Performance Assessment Questionnaire issued by the US Army Mission and Installation Command (MICC) at Fort Riley in support of RFP W911RX-27-R-A001 for footwear manufacturing services.

The questionnaire requests candid assessments of contractor performance on recent and relevant federal, state, local government, or commercial contracts comparable to NAICS 316210 (Footwear Manufacturing). Responses must be submitted to Contracting Officer Kevin Jones at kevin.jones185.civ@army.mil and Contract Specialist Vernard D. Ross at vernard.d.ross.civ@army.mil no later than 30 November 2026 at 12:00 PM. Part I requires the offeror to provide contract identification details including contractor name, address, program title, contract number, contract type, prime contractor name (if applicable), contract award date, completion date, nature of effort, and total contract value. Part II requires the respondent to evaluate contractor performance across five evaluation areas—compliance with specification requirements and workmanship standards (including supply chain management and equipment allocation), effectiveness of overall management and organization (including subcontractor control and quality control), timeliness of performance for services and product deliverables, commitment to customer satisfaction and business-like concern for customer interests, and general comments on relevant performance information. Each evaluation area uses a three-tier rating system: Acceptable (performance met contractual requirements with few or minor problems resolved effectively), Neutral (no knowledge or past experience with contractor performance in this area or performance occurred outside the evaluation period), or Unacceptable (performance did not meet contractual requirements with serious unresolved or marginally effective corrective actions). Detailed rationale is required for each rating. The questionnaire also requests identification of other relevant past efforts by the company and contact information for additional information sources.
