The ADDENDUM to FAR 52.212-2 is changed from:
Paragraph (a) is hereby replaced with the following:
The Government intends to use simplified acquisition procedures in accordance with FAR Subpart 13, Simplified Acquisition Procedures. The source selection process will be informal. Award will be made using the Tradeoff source selection process. This process permits tradeoffs among cost or price and non-cost factors and allows the Government to accept other than the lowest priced proposal. Award will be made to the responsible offeror whose proposal conforms to the solicitation and is deemed best value to the Government. Award may be made without discussions with offerors (except communications conducted for the purpose of minor clarification). Therefore, each initial offer should contain the offeror's best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if it is later determined by the contracting officer to be necessary.
The following evaluation factors will be used to evaluate proposals:
a. Factor 1: Technical and Management Capability. The term “technical,” as used herein, refers to factors other than past performance and price. The purpose of the technical evaluation is to assess whether the offeror will satisfy the Government’s minimum requirements of the solicitation. The Government will evaluate technical capability based on the written proposals for Factor 1 and its Subfactors 1A-1J. As stated in the ADDENDUM TO
52.212-1, the technical narrative shall not exceed 25 pages. If the technical narrative exceeds 25 pages, the evaluators will stop reading after page 25 and any information on subsequent pages will not be considered in the technical evaluation. Note: Merely restating the Government’s requirements may be deemed insufficient and rated marginal or unacceptable.
Subfactors 1A through 1J will be evaluated and will result in an overall technical and management capability evaluation rating for Factor 1. The following Combined Technical/Risk Adjectival Ratings will be used in evaluating Subfactors 1A though 1J and for the overall rating of Factor 1. The evaluation process will consider the following:
Subfactor 1A: Extended Warranty. This Subfactor will be evaluated to determine the quoter’s ability to provide the required services in accordance with the PWS. The following will be evaluated: response time to support calls that do not require onsite services; response time to support calls that require onsite services; the ability of the contractor to provide an extended warranty that extends the OEM’s warranty; the extended warranty terms/conditions;
the warranty’s coverage of configured peripherals/hardware configured with the VH carts; and the contractor’s ability to provide an extended warranty for the CISCO Codec.
Subfactor 1B: Remote Technical Support. This Subfactor will be evaluated to determine the quoter’s ability to provide the required services in accordance with the PWS. The following will be evaluated: remote technical support hours of operation; and response time to call for remote technical support.
Subfactor 1C: Hardware Support. This Subfactor will be evaluated to determine the quoter’s ability to provide the required service in accordance with the PWS. The following will be evaluated: methods for providing hardware support; availability of hardware support (i.e., hours of operation); support provided (i.e., parts repair, parts replacement when repair not possible, reinstallation of parts repaired and/or parts replaced, testing connectivity and ensuring optimal operating capability); and response times for hardware support calls.
Subfactor 1D: Software Support. This Subfactor will be evaluated to determine the quoter’s ability to provide the required services in accordance with the PWS. The following will be evaluated: methods for providing software support; availability of software support (i.e., hours of operation); software support provided; response times for software support calls; approach to coordinating diagnostic access for software support; approach to providing software updates and patches; and approach to providing the means of accessing required software updates.
Subfactor 1E: Cybersecurity Support. This Subfactor will be evaluated to determine the quoter’s ability to provide the required services in accordance with the PWS. The following will be evaluated: approach to integrating CS into their programmatic processes; documenting decisions; specifying and tracking CS requirements; documenting assessment efforts; identifying possible solutions; and maintaining operational systems security, and securing DoD information on Non-DoD information systems.
Subfactor 1F: Software Licenses. This Subfactor will be evaluated to determine the quoter’s ability to provide the required software licenses in accordance with the PWS.
Subfactor 1G: Routine/Preventative Maintenance and Repair Support. This Subfactor will be evaluated to determine the quoter’s ability to provide the routine/preventative maintenance and repair support service IAW the PWS. The following will be evaluated: routine repair services provided; response time for providing routine and preventative maintenance and repair; routine and preventative maintenance schedule; and routine repair parts provided.

Subfactor 1J: Global Med Certification. This Subfactor will be evaluated to determine the quoter’s ability to provide services under the resultant contract.
The following Combined Technical/Risk Adjectival Ratings will be used in evaluating Subfactors 1A through 1J and for the overall rating of Factor 1:
Color Rating Adjectival Rating Description
Blue Outstanding Quote indicates an exceptional approach and understanding of the requirements and contains multiple strengths, and risk of unsuccessful performance is low.
Purple Good Quote indicates a thorough approach and understanding of the requirements and contains at least one strength, and risk of unsuccessful performance is low to moderate.
Green Acceptable Quote indicates an adequate approach and understanding of the requirements, and risk of unsuccessful performance is no more than moderate.
Yellow Marginal Quote has not demonstrated an adequate approach and understanding of the requirements, and risk of unsuccessful performance is high.
Red Unacceptable Quote does not meet requirements of the solicitation and, thus, contains one or more deficiencies, and/or risk of unsuccessful performance is unacceptable. Quote is unawardable.
Technical Risk Rating. Assessment of technical risk, which is manifested by the identification of weakness(es), considers potential for disruption of schedule, increased costs, degradation of performance, the need for increased Government oversight, and/or the likelihood of unsuccessful contract performance.
Combined Technical/Risk Adjectival Ratings take into consideration the risk descriptions in the table below:
Adjectival Rating Description Low Quote may contain weakness(es) which have little potential to cause disruption of schedule, increased cost or degradation of performance. Normal contractor effort and normal Government monitoring will likely be able to overcome any difficulties.
Moderate Quote contains a significant weakness or combination of weaknesses which may potentially cause disruption of schedule, increased cost or degradation of performance. Special contractor emphasis and close Government monitoring will likely be able to overcome difficulties.
High Quote contains a significant weakness or combination of weaknesses which is likely to cause significant disruption of schedule, increased cost or degradation of performance. Is unlikely to overcome any difficulties, even with special contractor emphasis and close Government monitoring.
Unacceptable Quote contains a material failure or a combination of significant weaknesses that increases the risk of unsuccessful performance to an unacceptable level.
b. Factor 2: Past Performance. Performance risk analysis provides insight into the offeror’s probability of successfully completing the solicitation requirements based on the quoter’s performance record on recent, relevant, and similar contract efforts. For this requirement, recency is defined as performance that occurred within the last 3 years. Relevancy is defined as performance in which services under FSC Code: J065, Maintenance/Repair/Rebuild of Equipment - Medical, Dental, and Veterinary Equipment and Supplies were provided. The Government may consider and use any information from its own files, databases, references provided by the quoter, or from any source, as it deems appropriate. Evaluation of past performance will be subjective and will take into consideration areas such as schedule performance; quality of supplies; business relations; management of key personnel; and customer satisfaction. If adverse past performance information is obtained, the respective point of contact will be contacted to obtain additional information about the circumstances surrounding the situation. The Government will not consider the past performance efforts of subcontractors since the Government does not have privity of contract with subcontractors and subcontractors are not obligated to perform under any resultant contract (i.e. delivery orders). Quoters will be given an opportunity to address unfavorable reports of past performance to which the quoter has not had a previous opportunity to respond. The quoter’s response, or lack thereof, will be taken into consideration. If a quoter does not have a past performance history relating to the solicitation, the quoter will not be evaluated favorably or unfavorably and will receive a Neutral Confidence rating for this factor. Providing incorrect references may affect the overall rating for Factor 2.
The following adjectival ratings will be used in evaluating Factor 2:
Adjectival Rating: Description:
Substantial Confidence
Based on the quoter’s recent/relevant performance record, the Government has a high expectation that the quoter will successfully perform the required effort.
Satisfactory Confidence
Government has a reasonable expectation that the quoter will successfully
Neutral Confidence No recent/relevant performance record is available or the quoter’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The quoter may not be evaluated favorably or unfavorably on the factor for past performance.
Limited Confidence
Government has a low expectation that the quoter will successfully
No Confidence
Government has no expectation that the quoter will be able to successfully
Factor 3: Price . This factor will be evaluated to determine the extent to which the total evaluated price is fair and reasonable in terms of the Government’s requirement. To determine price fair and reasonable the Government will use price analysis techniques, such as comparison of quoted prices with the prices of all quoters that meet the minimum Factor 1 requirements of the solicitation, comparison of quoted prices to historical prices paid, comparison of quoted prices with competitive published market prices, comparison of quoted prices with independent Government cost estimates, comparison of quoted prices with prices obtained through market research for the same or similar items, and analysis of data of other than certified cost or pricing data provided by the offeror. Price will not receive an evaluation rating.
The relative order of importance for the evaluation factors is as follows: Subfactors 1A through 1J are equal in importance and when combined make up the total evaluated rating for Factor 1. Factor 1 is more important than Factor 2. Factors 1 and 2 when combined are significantly more important than Factor 3.
To receive consideration for award, a rating of no less than Green, Acceptable must be received for Factor 1, Technical and Management Capability and for each of the Subfactors 1A-1J; and a rating of no less than Neutral Confidence must be received for Factor 2, Past Performance. In addition, pricing must be considered fair and reasonable.
Prior to award, the KO will conduct a responsibility assessment, in accordance with FAR Part 9.104, of each offeror being considered for award; the offeror must be otherwise qualified and eligible to receive an award under applicable laws and regulations.

Unacceptable Quote contains a material failure or a combination of significant weaknesses that increases the risk of unsuccessful performance to an unacceptable level.
Government has a high expectation that the quoter will successfully
