About this file

This is Amendment 0006 to solicitation W81K0026QA240, issued by the Medical Readiness Contracting Office West at JBSA Fort Sam Houston, Texas, effective October 21, 2026. The amendment modifies the source selection strategy from Lowest Price Technically Acceptable (LPTA) to Past Performance Tradeoff (PPT), extends the proposal due date from October 16, 2026 to November 9, 2026, incorporates updates to the Performance Work Statement dated October 20, 2026, and adjusts the period of performance for all ordering periods.

The contract involves five ordering periods with CLINs for services. All ordering periods have been shifted forward approximately 57 days, with Ordering Period 1 now running from January 11, 2027 to January 10, 2028 (previously November 15, 2026 to November 14, 2027), and subsequent periods adjusted accordingly through Ordering Period 5 (January 11, 2031 to January 10, 2032). The amendment adds three new attachments: Attachment 1 (Performance Work Statement dated October 20, 2026), Attachment 2 (Past Performance Summary Sheet), and Attachment 3 (Past Performance Questionnaire). Under the revised PPT evaluation methodology, proposals will be evaluated on Technical Capability (Acceptable/Unacceptable), Past Performance (using Performance Confidence Assessment ratings from Substantial Confidence to No Confidence), and Price, with past performance rated as significantly more important than price. Contractors must submit proposals in four separate volumes: Administrative, Technical (not exceeding 15 pages), Past Performance, and Pricing. The contract clause 252.237-7023 regarding Continuation of Essential Contractor Services has been incorporated. All proposals must be submitted electronically to Jaclyn Evans and James Davis at the provided email addresses, with offerors required to acknowledge all amendments and maintain SAM registration throughout contract performance.

The Contractor shall notify the Contracting Officer of an increase or decrease in costs within ninety days after continued performance has been directed by the Contracting Officer, or within any additional period that the Contracting Officer approves in writing, but not later than the date of final payment under the contract. The Contractor's notice shall include the Contractor's proposal for an equitable adjustment and any data supporting the increase or decrease in the form prescribed by the Contracting Officer. The parties shall negotiate an equitable price adjustment to the contract price, delivery schedule, or both as soon as is practicable after receipt of the Contractor's proposal.

Statements that the offeror understands, can or will comply with all specifications, or statements paraphrasing the specifications of parts thereof, or phrases such as "standard procedures will be used" or "well-known techniques will be used" will be considered unacceptable. Insufficient explanations may result in your offer receiving an UNACCEPTABLE rating. The technical proposal shall not exceed 15 pages.

FACTOR 2 - PAST PERFORMANCE:

The Offeror shall submit with its proposal evidence of recent and relevant past performance under which YOUR company (the Offeror) provided the same or similar services with similar magnitude and scope in the public and /or private sector. Performance must have been by the Offeror (not by a subsidiary, affiliate or sister company) either as the prime contractor or subcontractor. For a description of the characteristics or aspects the Government will consider in determining recency and relevancy, see the Evaluation Criteria for Factor 2 - Past Performance in the Addendum to 52.212-2.

Offerors shall submit with their proposal consent letters executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing the release of past performance information.

The Contract Number, Total Contract Dollar Value, Period of Performance, Type and Complexity of Services provided submitted shall be for the Task

/Delivery Order the Offeror has performed under this ordering type contractual vehicle. Not the ordering vehicle itself or a combination of orders under the vehicle. The Government will not evaluate the Project Summary Sheet if the Offeror fails to submit a specific delivery/task order or elects to submit a combination of delivery/task orders.

NOTE: Past performance reference projects performed by a proposed teaming partner or a proposed subcontractor will only be considered if: (1) the teaming partner or subcontractor is identified as a teaming partner or subcontractor in the administrative volume of the proposal, (2) the role and percentage of work proposed to be performed by the teaming partner or subcontractor on the proposed requirement is detailed in the proposal, (3) items (1) and (2) are identified in the administrative volume, and (4) one of the three past performance projects submitted is the Offerer's, as a prime, and is evaluated to be recent, and relevant or very relevant.

Past Performance Questionnaires (PPQs) (Attachment 3) may be submitted (however not required) for each past performance reference identified on the project summary sheet (Attachment 2). PPQs that have been completed for other solicitations in the past 12 months from the close of the date of the solicitation will also be accepted.

The Government may validate the information with the POCs listed.

Contractor Performance Assessment Reports (CPARs) will also be accepted. For PPIs that have CPARs, Offerors can submit CPARs with PPIs in lieu of PPQs. Offerors shall ensure CPARs are finalized and reference the exact same projects detailed in the PPIs.

All past performance proposals must contain a project summary sheet (Attachment 2).

Note. The Project Summary sheet must contain contract numbers, descriptions of work, periods of performance, contract value, and POC emails/phone numbers. If a PPQ or CPARs was not received for any of the past performance references identified on the project summary sheet, the Government may conduct interviews to validate performance/quality information. Any past performance reference that does not contain valid/current POC contact information or, if the POC provided cannot validate work identified on the project summary sheet, the past performance reference will not be evaluated.

The Government reserves the right to use both the information provided in the Offeror's Past Performance submittals and information obtained from other sources available to the Government, to include, but not limited to, the MQS MILDEPT report, the Contractor Performance Assessment Reports (CPARs), Federal Awardee Performance and Integrity Information System (FAPIIS), SAM.gov or other databases; interviews with Program Managers, Contracting Officers, and commercial sources and/or non-Government sources, such as any commercial business or customers referenced by the Offeror in any of the past performance information disclosed.

NOTE: It is the responsibility of the Offeror to track and ensure the completion/submission of the PPQs and Project Summaries by the proposal due date. The Government will not accept incomplete or late PPQs. An electronic copy of the Offerors PPQs shall be submitted with the Offerors proposal (Do not refer Government personnel to a previous PPQ without enclosing a copy).

FACTOR 3 - PRICING:

a. Offerors shall complete the pricing, to include both unit pricing and extended price, for each of the Contract Line Item Numbers (CLINs). Pricing for each item shall be annotated as 2 decimals ONLY (i.e. $250.46). Please provide pricing for ordering periods, as well as a grand total of the entire proposal. Certified Cost and Pricing Data is not required under commercial acquisitions; however, if only one proposal is received - Other than Certified Cost or Pricing data may be required.

NOTE: This is an "all or none" solicitation. Failure to provide a unit price for any line item, unless specifically identified as "not separately priced" by the offeror, will be construed to mean the offeror is not quoting that item and thus, not eligible for award.

Paragraph 52.212-1(m) is hereby added:

(m) The non-FAR Part 12, discretionary FAR, DFARS, AFARS provisions included herein are incorporated into this solicitation either by reference or in full text. If incorporated by reference, see provision 52.252-1 herein for locations where full text can be found.

Evaluation Factors for Award

Miscellaneous text in this section has been modified from:

Addendum to 52.212-2, Evaluation - Commercial Products and Commercial Services

Paragraph 52.212-2(a) is hereby amended, as follows:

The Government will award a Firm-Fixed-Price (FFP) contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation is the best value to the Government.

Award will be made using the Lowest Price Technically Acceptable (LPTA) process. The Government will initially list proposals from lowest to highest price. The Government will evaluate the lowest priced proposal first. If that proposal is determined technically unacceptable, the Government will evaluate the next lowest priced proposal and continue in that manner until a proposal is rated Technically Acceptable. If a proposal is rated Technically Unacceptable, the

Government will not evaluate any other factor. USING THIS PROCEDURE, THE GOVERNMENT The following evaluation MIGHT NOT EVALUATE ALL PROPOSALS. factors shall be used to evaluate the offer:

Factor 1 - Technical Capability:

Technical capability ratings reflect the Government's confidence in the offeror's ability, as demonstrated in its written technical proposal, to perform the requirements stated in the solicitation. The Government will make an independent judgment of the probability of success.

Prospective contractors shall demonstrate that the service offered complies with the technical requirements described in the Performance Work Statement by submission of a written capability statement with their offer which specifically must address adherence to the PWS. To receive consideration for award, a rating of no less than "Acceptable" must be achieved in technical acceptability. If an offeror does not indicate whether its proposed services meets a certain minimum requirement, the Government will determine that it does not. adjectival The following ratings will be used in evaluating each offeror's written technical proposal:

ACCEPTABLE: To receive an acceptable rating the proposal shall clearly demonstrate that the offeror fully understands and is capable of performing the minimum requirements of the Performance Work Statement.

UNACCEPTABLE: An unacceptable rating will be assessed on any offeror that presents a proposal that fails to clearly demonstrate that the offeror fully understands and is technically capable to perform the minimum requirements of the Performance Work Statement .

Factor 2 - Price:

The Government will evaluate the pricing to determine if it represents a fair and reasonable price once the proposal has received an acceptable rating for technical capability. Offerors' pricing must be determined to be fair and reasonable in order to be eligible for an award. Award will be made on a Lowest Price Technically Acceptable (LPTA) basis. A Responsibility Determination will be made prior to award in accordance with FAR 9.104-1. Offerors shall indicate a price for each CLIN to be eligible for award.

Paragraph (b) is hereby replaced with the following:

The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. In addition, given that FAR Clause 52.217- 8, Option to Extend Services, provides that the Government may require continued performance of any services within the limits and at the rates specified in the contract (i.e., the rates in effect when the Option to Extend Services clause is exercised), for purposes of evaluating this option, the contracting officer will consider the prices submitted for each CLIN, since those would be the binding prices should this option be exercised. Evaluation of this option shall not obligate the Government to exercise the option.

(c) A written notice of award or acceptance of an offer, within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

ADDENDUM TO FAR 52.212-2 EVALUATION--COMMERCIAL PRODUCTS AND

COMMERCIAL SERVICES

Paragraph 52.212-2 (a) is hereby added: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation is the best value to the Government, price and other factors considered. Award will be made using the Tradeoff source selection process. This process permits tradeoffs among cost or price and non-cost factors and allows the Government to accept other than the lowest priced offer. The following factors shall be used to evaluate offers:

The Government will review proposals received to determine if they are compliant to the solicitation requirements. A compliant proposal is one that meets all of the terms, conditions and specifications in the solicitation. The Offeror must provide all documents listed and comply with all proposal instructions in order for their proposal to be considered "compliant" to this solicitation.

Only compliant proposals will be evaluated for award.

Award shall be made on a "Past Performance Price Tradeoff" basis. Past performance is significantly more important than Price. An Offeror's proposed Technical Proposal will be evaluated to determine if the proposed Technical Proposal is acceptable or unacceptable in accordance with the evaluation criteria stated in this solicitation. If an Offeror's Technical Proposal is determined to be unacceptable, the Offeror will no longer be considered for award, and no further evaluation will be conducted.

The Government seeks to award to the Offeror who gives the Agency the greatest confidence that it will best meet or exceed our requirements in a way that will be advantageous to the Government and whose compensation plan (s) has been determined to be "Acceptable." This may result in an award to a higher rated, higher priced Offeror where the decision is consistent with the evaluation factors and the Contracting Officer reasonably determines that the past and present performance of the higher priced Offeror outweighs the price difference. This may result in an award to a higher-rated, higher-priced offer if the Government determines that the past performance of the higher-priced offer outweighs the price difference. The selection process is subjective by nature and professional judgment is implicit throughout the tradeoff process. The contract will be awarded to the Offeror whose proposal, based upon the evaluation criteria, represents the best value to the Government.

Award may be made without discussions with Offerors (except communications conducted for the purpose of minor clarification). Therefore, the offer should contain the Offeror's best terms and conditions.

EVALUATION CRITERIA

The following evaluation factors will be used to evaluate each proposal: technical proposal, past performance, and price. The Offeror's technical proposal will be evaluated as either "Acceptable" or "Unacceptable". If an Offeror's Technical Proposal is "Unacceptable," the Offeror will not be eligible for award and will not be further evaluated. Past Performance is significantly more important than Price.

Factor 1 - Technical

Factor 2 - Past Performance

Factor 3 - Price

FACTOR 1 - TECHNICAL

The following adjectival ratings will be used in evaluating each offeror's written technical proposal:

ACCEPTABLE: To receive an acceptable rating the proposal shall clearly demonstrate that the offeror fully understands and is capable of performing the minimum requirements of the Performance Work Statement.

UNACCEPTABLE: An unacceptable rating will be assessed on any offeror that presents a proposal that fails to clearly demonstrate that the offeror fully understands and is technically capable to perform the minimum requirements of the Performance Work Statement.

FACTOR 2 - PAST PERFORMANCE

This factor is assessed with an overall Performance Confidence Assessment (PCA) rating (Table 3), which is based on an integrated assessment of the Offeror's recent, relevant, and quality past performance.

The resulting performance confidence assessment rating is determined at the factor level and represents an overall evaluation of contractor performance. Offerors without a record of recent, relevant, and quality past performance or for whom information on past performance is so sparse that no meaningful confidence assessment rating can be reasonably assigned will not be evaluated favorably or unfavorably on past performance and, as a result, will receive a "Neutral Confidence" rating for the Past Performance factor.

NOTE: Past performance reference projects performed by a proposed teaming partner or a proposed subcontractor will only be considered if: (1) the Offeror submits at least one recent, relevant or very relevant past performance project that the Offeror has performed, as a prime; (2) the teaming partner or subcontractor is identified as a teaming partner or subcontractor in the administrative volume of the proposal, and (3) the role and percentage of work proposed to be performed by the teaming partner or subcontractor on the proposed requirement is detailed in the proposal. Items (2) and (3) shall be identified in the administrative volume.

In the instance that an offeror submits more than three (3) Project Summary Sheets, only the first three (3) will be evaluated.

Recency Assessment: An assessment of the past performance information contained in the Offeror's submittals will be made to determine if it is recent. To be recent, the effort must be ongoing or must have been performed within the past three (3) years from the closing date of this solicitation. Past performance information that fails this condition will not be evaluated.

Relevancy Ratings: An assessment of the recent past performance information will be made to determine its relevancy. This information will be evaluated to determine how closely the services performed under those contracts relate to the scope, magnitude of effort and complexities this solicitation requires.

Table 2. Relevancy

Relevancy Rating

Adjectival Rating Description

Very Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.

Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.

Somewhat Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.

Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.

Performance Quality Assessment

After the determination of recency and relevancy, the Government will assess the performance quality (how well the contractor performed on the contracts). The Government will review all past performance information collected and determine the quality of the Offeror's performance, general trends, and usefulness of the information and incorporate these into the Performance Confidence Assessment rating. A separate quality assessment rating will not be assigned; rather, the Past Performance Confidence Assessment rating is based on the Offeror's overall record of recency, relevancy, and quality of performance.

Confidence Ratings: The Government will use the following definitions/ratings in Table 3 when Proposals with a Performance making an overall Performance Confidence Assessment rating.

Confidence Assessment Rating of less than Neutral shall not be further evaluated.

Table 3. Performance Confidence Assessment Rating

Performance Confidence Assessments Rating Methods

Rating Description

Substantial Confidence

Based on the Offeror's recent/relevant/performance record, the Government has a high expectation that the Offeror will successfully perform the required effort.

Satisfactory Confidence Based on the Offeror's recent/relevant/performance record, the Government has a reasonable expectation that the Offeror will successfully perform the required effort.

Neutral Confidence No recent/relevant/performance record is available, or the Offeror's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned.

Limited Confidence Based on the Offeror's recent/relevant/performance record, the Government has a low expectation that the Offeror will successfully perform the required effort.

No Confidence Based on the Offeror's recent/relevant/performance record, the Government has no expectation that the Offeror will successfully perform the required effort.

FACTOR 3 - PRICE

The Government will evaluate the pricing to determine if it represents a fair and reasonable price once the proposal has received an acceptable rating for technical capability and past performance.

Offerors' pricing must be determined to be fair and reasonable in order to be eligible for an award.

Award will be made on a Performance-Price Tradeoff (PPT) basis. A Responsibility Determination will be made prior to award in accordance with FAR 9.104-1. Offerors shall indicate a price for each CLIN to be eligible for award.

The Government will evaluate offers for award purposes by adding the total price for all ordering periods. The Government may determine that an offer is unacceptable if the prices are significantly unbalanced.

The award decision will be based on the best value offered to the Government and not solely on price and price related factors. In making the award decision, the Contracting Officer will consider the evaluated rating of all factors. Proposals which receive ratings below "Neutral Confidence" and /or a rating of "Unacceptable" will not be eligible for award and will not be further evaluated.

A written notice of award or acceptance of an offer, within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
