Offerors shall ensure all aspects of the PWS are addressed and that the Offeror’s understanding of services and the work effort required are fully demonstrated in their proposal.

As part of the managerial approach, Offerors shall submit a staffing plan that demonstrates the intended methodology for obtaining and maintaining the skills needed in performance of this contract. Proposed personnel should reflect labor categories and qualifications as established in the basic contract and this RFP. At a minimum the following shall be included:

(i) Describe and demonstrate the approach to ensuring that the personnel hired and retained are qualified with the requisite knowledge, skills, and training to successfully execute the task areas set forth in the PWS.

SECTION III – FACTOR 2 – PAST PERFORMANCE:

This information is required on the Offeror and shall contain all aspects of the effort the Offeror considers critical to overall successful performance. Offerors are cautioned that the Government will use data provided by each Offeror in this section and data obtained from other sources in the evaluation of past performance. Each Offeror shall submit a Past Performance Section containing the following:

Offerors shall complete and provide the Past Performance Information (PPI) Sheet (Exhibit B) on at least two (2) but no more than five (5) recent task orders or stand-alone contracts for a Government or commercial client that they consider most relevant in demonstrating their ability to perform the proposed effort. Include rationale supporting the assertion of relevance. For a description of the characteristics or aspects the Government will consider in determining recency and relevance, see the Evaluation Criteria for Factor 2 – Past Performance.

PPI shall explain what aspects of the task orders or contracts are deemed relevant to the proposed effort and to what aspects of the proposed effort they relate. This may include a discussion of efforts accomplished by the Offeror to resolve problems encountered on prior task orders or contracts as well as past efforts to identify and manage performance risk. Merely having problems does not automatically equate to a limited or no confidence rating, since the problems encountered may have been on a more complex requirement, or an Offeror may have subsequently demonstrated the ability to overcome the problems encountered. The Offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions in terms of improvements achieved or problems rectified. This may allow the Offeror to be considered a higher confidence candidate.

Past Performance Questionnaires (PPQs) (Exhibit C). The Offeror will send blank PPQs to Government agencies and private sector companies requesting past performance information on applicable projects.

(Minimum of two (2) PPQ's are required to be eligible for award).

It is the responsibility of the Offeror to track and ensure the completion/submission of the PPQs and PPI Sheets by established due date. The Government will not accept incomplete or late PPQs.

An electronic copy of the Offerors PPQs shall be submitted with the Offerors proposal. (Do not refer Government personnel to a previous PPQ without enclosing a copy).

Note: The PPI sheets prepared shall reference the exact same projects addressed in the PPQs. Each PPI Sheet is limited to 3 pages per project. PPQ’s documents are due to the Mr.

Anthony Browner, Contract Specialist at the above email address in section I by 21 August 2018, 1:00 pm CST.

Past Performance Information Worksheet (PPI) Should be COMPLETED BY Contract Company and returned with the proposal on 21 August 2018 @1:00 pm CST.

The Government reserves the right to use both the information provided in the Offeror’s Past Performance submittals and information obtained from other sources available to the Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and commercial sources and/or non-Government sources, such as any commercial business or customers referenced by the Offeror in any of the past performance information disclosed.

SECTION IV - FACTOR 3 - PRICE:

The Offeror shall submit proposed pricing for all contract line item numbers identified in the RFP. All pricing information shall be addressed ONLY in the Price Proposal Volume. Prices shall be provided in whole-dollar amounts in the electronic version of the Schedule outlined in Section B = Line Items/CLINs of the RFP.
