About this file

The document is a Past Performance Questionnaire (PPQ) form used by Army Contracting Command - Rock Island (ACC-RI) to assess potential contractors for a solicitation related to Propellant Retainer Assemblies. The PPQ is designed to gather detailed performance information from previous contract references, with respondents rating the contractor's performance across multiple dimensions including quality, delivery, problem resolution, work satisfaction, and compliance with security requirements.

The questionnaire provides a structured evaluation framework with ratings ranging from Excellent to Unsatisfactory, allowing past project managers to comprehensively document a contractor's performance. Completed questionnaires must be submitted directly via email to MichelleAnn Hulett (Contract Specialist) and Bethany Carbajal (Contracting Officer), with specific instructions for email subject line and submission. The associated federal contract opportunity (Solicitation W519TC-24-R-2027) is for a five-year Firm Fixed Price Indefinite Delivery Indefinite Quantity contract to supply Propellant Retainer Assemblies to Pine Bluff Arsenal, with the solicitation closing date most recently extended to 31 July 2025.
