RE: 2023-0194
B3400 Asbestos and Lead Paint Survey
Dear Captain MacNeal, Muranaka Environmental Consultants, Inc. (MEC) was retained to provide professional consulting services for JBPHH as they plan to demolish two walls and install two doors in two locations within Building B3400. The services included an asbestos and lead paint inspection of the Locker Room and Room 108 within Building B3400.
MEC conducted the inspection sampling on August 22, 2023. A total of twelve
(12) suspect asbestos-containing samples were collected and three (3) paint chip samples were collected for analysis. Samples were logged on to a Chain of Custody form and submitted to NVL Laboratories, Inc. for asbestos and lead paint analysis.
Detectable amounts of asbestos were found in the joint compound on the eastern wall in the northeast corner of the Locker Room. Table 1 lists the sample locations, descriptions, and laboratory results.

Asbestos If materials containing asbestos are disturbed during demolition or renovation, compliance with EPA, OSHA, and State of Hawaii-Department of Health, is required.
Air sampling should be performed during the removal of ACM to document that air concentrations of asbestos fibers are below the Hawaii Occupational Safety and Health (HIOSH) permissible exposure limit (PEL) of 0.1 fibers per cubic centimeter of air (f/cc) as an 8-hour time weighted average. Once the ACM have been removed, visual clearance by a certified technician must be performed to document adequate clean up. Asbestos-containing waste must be handled and disposed of in accordance with DOH Title 11 HAR, Chapter 501 Asbestos Requirements.
Lead-Containing Paint (LCP) If LCP is disturbed during demolition or renovation, compliance with EPA, OSHA, and State of Hawaii DOH is required.
Air sampling should be performed during any demolition or renovation activities to document that air concentrations of lead dust are below the OSHA PEL of 50 micrograms per cubic centimeter (µg/m3) of air.
After lead abatement, a visual inspection should be conducted to confirm that all visible lead-containing material, dust and debris have been removed from the work area. Clearance wipe samples should be collected and analyzed for lead, according to the HUD “Guidelines for the Evaluation and Control of Lead Based Paint Hazards in Housing”.
The conclusions, observations, and recommendations made in this letter are based on the limitation of the contract and the condition of the property at the time of the sampling and inspection was conducted. MEC accepts no
Joint Base Pearl Harbor-Hickam, HI 96853 Page 4 responsibility for the inaccuracy or inapplicability of any part of this report that may be attributed to a change in the condition of the property after the survey was conducted or attributable to property conditions that were not readily accessible or observable at the time of the survey.
In addition, we accept no responsibility for inaccurate or missing information provided by existing documents.

Case Narrative:
The following summarizes samples received by NVL Laboratories, Inc. from customer indicated on the accompanied cover letter. Samples were logged in for analysis per customer request using both customer sample ID's and laboratory assigned ID's as listed on NVL Labs Receiving Record. All samples as received were processed and analyzed within specified turnaround time without any abnormalities and deviations that may affect the analytical results. All quality control requirements were acceptable unless stated otherwise. The conditions of all samples were acceptable at time of receipt and all samples submitted with this batch were analyzed unless stated otherwise on NVL Labs Receiving Record .
Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light microscopy (PLM) with dispersion staining in accordance with EPA 600/R-93/116 Method for the Determination of Asbestos in Bulk Building Materials.
For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer). The asbestos concentration in the sample is determined by calibrated visual estimation.
For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAPS, 40 CFR Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentration of asbestos.
The detection limit for the calibrated visual estimation is <1%, 400 point counts is 0.25% and 1000 point counts is 0.1%
Definition of Data Qualifiers:
J1 = Composite result for whole sample is less than 1% asbestos
J2 = Qualitative analysis was conducted for the presence of asbestos fibers in this sample
J3 = Insufficient sample amount for thorough analysis
J4 = Composite result for whole sample provided per client request
J5 = Sample appears to be contaminated from outside source
J6 = Surface appears to be contaminated by chrysotile-containing material
J7 = Description on field data sheet does not match sample composition
J8 = Unable to estimate with confidence due to very small amount of material
J9 = Unable to separate components for layer analysis
J10 = Unsure of correct layer sequence
J11 = Sample not analyzed per client request
J12 = Sample bag empty
R = Reported data are not reliable due to possible contamination or loss of material during preparation or analysis. Re-sampling and reanalysis are necessary for verification.

Dear Mr. Tanaka, Enclosed please find test results for samples submitted to our laboratory for analysis. Preparation and analysis of these samples were conducted in accordance with methods specified on the attached test reports. Method: EPA 7000B
The content of this package consists of the following:
Case Narrative & Definition of Data Qualifiers NVL Labs Receiving Record Client Chain-of-Custody (CoC) Analytical Test Results Sample Raw Data QA/QC Analysis Report Equipment Check & Calibration
The analytical data and test results as provided are authentic and are considered highly confidential.
This report along with other related documents will not be released without your written consent and approval. Samples are archived for one month following analysis. Samples that are not retrieved by the client will be discarded after two weeks.
Thank you for using our laboratory services. If you need further assistance, please contact us at 206-547-0100 or 1-888-NVLLABS.

Definition of Data Qualifiers:
J = The reported concentration is an estimated value because something may be present in the sample that interfered with then analysis.
J1 = The reported concentration is an estimated value because the laboratory control sample (LCS) is out of control limits.
J2 = The reported concentration is an estimated value because the percent recovery for matrix spike is out of control limits.
J3 = The reported concentration is an estimated value because the relative percent difference (RPD) for duplicate analysis is out of control limits.
J4 = Percent recovery is outside of established control limits.
B = Blank contamination. The recorded results is associated with a contaminated blank.
R = The data are not reliable due to possible contamination or loss of material during preparation or analysis. Re-sampling and reanalysis are necessary for verification.
< = Below Reporting Limit (RL) or Limit of Quantitation (LoQ) of the instrument
RL = Reporting Limit. The minimum concentration that can be quantified under routine operating conditions.
ND = Analyte not detected or below the reporting limit of the instrument or methodology.
Other Notations:
ug/mL = Micrograms per milliliter ug/sample = Micrograms per sample ug/sq. ft. = Micrograms per square foot mg/Kg (ppm) = Milligrams per kilogram (parts per million)
