5. A complete bid package must contain the following:

(e) When completing the “Offeror” portion of the SF 1442 (Blocks 14 – 20), an official having the authority to contractually bind the company must sign the SF 1442 in accordance with FAR 4.102. One copy of the SF1442 is required to have an original signature.

(f) All amendments to the solicitation must be acknowledged.

(g) Mark the front of the envelope with the following: “BID No. W50S97-20-B-0001”, and the Name and Address of Bidder. See Block 13A of SF 1442 for required number of copies.

6. Joint Venture Requirements - Parties wishing to propose as a joint venture must submit, with the Pro Forma documents, an executed copy of the joint venture agreement. The joint venture agreement shall be executed in accordance with FAR 4.102(d). Each joint venture partner shall submit evidence satisfactory to the Contracting Officer that its Board of Directors has approved its participation in the joint venture or by an instrument of similarly binding character in the case of an unincorporated entity. Offerors are reminded that to obtain an award the awardee, to include a joint venture, must be registered in the System for Award Management (SAM) database.

Applicable to set-aside solicitations: Joint Ventures must meet applicable size standards (See FAR Subpart 19.1).

Submit with the proposal any size determination for the Joint Venture entity received from the Small Business Administration. A Joint Venture where-in one party is not a small business may be referred to the Small Business Administration ( IAW FAR 19.302 (b)) to determine eligibility. All questions regarding Joint Ventures must be directed to the Small Business Administration (SBA) and not the Contracting Officer.

NOTE: An Original Bid bond and Original Power of Attorney is required with submission of the bid. The bid bond is due on the date as stated on the SF1442 block 13. The bid bond must be on a SF24 form. Rejection of a bid bond shall be in accordance with FAR 28.101-4 Noncompliance with bid guarantee requirements.

SECTION 00700 - CONTRACT CLAUSES

252.236-7001 CONTRACT DRAWINGS AND SPECIFICATIONS (AUG 2000)

(a) The Government will provide to the Contractor, without charge, one set of contract drawings and specifications, except publications incorporated into the technical provisions by reference, in electronic or paper media as chosen by the Contracting Officer.

(b) The Contractor shall--

(1) Check all drawings furnished immediately upon receipt;

(2) Compare all drawings and verify the figures before laying out the work;

(3) Promptly notify the Contracting Officer of any discrepancies;

(4) Be responsible for any errors that might have been avoided by complying with this paragraph (b); and

(5) Reproduce and print contract drawings and specifications as needed.

(c) In general--

(1) Large-scale drawings shall govern small-scale drawings; and

(2) The Contractor shall follow figures marked on drawings in preference to scale measurements.

(d) Omissions from the drawings or specifications or the misdescription of details of work that are manifestly necessary to carry out the intent of the drawings and specifications, or that are customarily performed, shall not relieve the Contractor from performing such omitted or misdescribed details of the work. The Contractor shall perform such details as if fully and correctly set forth and described in the drawings and specifications.

(e) The work shall conform to the specifications and the contract drawings identified on the following index of drawings:

SECTION 00800 - SPECIAL CONTRACT REQUIREMENTS

STATEMENT OF WORK

REMOVE PAVEMENT FOR ATFP

BERRY FIELD, NASHVILLE, TENNESSEE

1) GENERAL

a) The work covered by this Statement of Work (SOW) consists of furnishing all labor, equipment, and materials to improve the force protection around building 802 and building 803 by removing existing asphalt areas and increasing standoff to parking areas in accordance with this written statement of work, all construction plans, specifications, and the building code. The work location is the 118th Wing, Nashville, Tennessee.

Contractor will be fully responsible for compliance with all base, local, state, and federal laws and regulations that apply to include environmental and occupational safety.

2) PERFORMANCE PERIOD

a) The performance period for this project is 90 calendar days.

3) SCOPE OF WORK

a) Contractor will perform all work as described herein and as shown on drawings for project BKTZ202010, Remove Pavement for ATFP.

b) Work around building 802

1. Remove existing asphalt pavement within 20 feet of building 802 as shown on plans.

2. Install new concrete sidewalks to existing doors.

3. Install new catchbasins, downspout extensions, drainage pipes, and concrete headwalls.

4. Install new curb and gutter with 12” high face.

5. Add topsoil and grass sodding where asphalt was removed.

6. Includes other work indicated on drawings.

c) Work around building 803

1. Remove existing asphalt pavement within 20 feet of building 803 as shown on plans.

2. Install new concrete sidewalks to existing doors.

3. Install new catchbasins, downspout extensions, drainage pipes, and concrete headwalls.

4. Install new curb and gutter with 12” high face.

5. Add topsoil and grass sodding where asphalt was removed.

6. Includes other work indicated on drawings.

7. Install (4) new removable bollards.

d) Contractor Manpower Reporting

1. The contractor will report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for Air Force collection site. The contractor is required to completely fill in all required data fields using the following web ad-dress: https://afcmra.hqda.pentagon.mil/

2. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September

30. While inputs may be reported any time during the FY, all data must be reported no later than October 31 of each calendar year. Contractors may direct questions to the help desk at http://www.ecmra.mil/.

e) Contract Requirements

1. The contractor will adhere to all contract requirements in the solicitation and subsequent contract or delivery order.

2. In addition to those items in the solicitation/contract, the contractor will also be responsible to:

a. Obtain all dig permits, crane permits, or any other permits prior to starting construction.

b. Verify all field and plan measurements and notify the contracting officer’s representative (COR) of any discrepancies.

c. Perform all required construction staking.

d. Prepare project site for construction by placing adequate signage and other devices indicating construction zone for vehicular traffic and pedestrians.

e. Submit all project submittals to the COR for approval prior to the manufacture or placement of any construction materials.

f. Perform all material testing as required by specifications and building code.

g. Maintain adequate erosion control measures around any disturbed soil areas and prevent any soil discharge into stormwater systems.

h. Perform site finishing and cleanup to include grass establishment.

i. Remove all work related debris and dispose of it off the base property upon completion of project.

j. Contractor will acquire portable toilets for use by employees and any subcontractors working on this project.

4) REFERENCES

a) In general, all technical specifications are provided within the Construction Documents.

In the event any project element is not covered in the specifications provided, the contractor will refer to the Specification Hierarchy for direction related to the element.

All work in this project will adhere to the Unified Facilities Criteria.

5) SPECIFICATION HIERARCHY

a) The specification hierarchy is the collection of technical specifications to be used for all projects. The contractor should refer to the specifications provided in the Construction Documents first before moving to the next technical specification listed in the specification hierarchy. The hierarchy is listed in the order of precedence (i.e. “1” supersedes “2”). The hierarchy is as follows:

2. For MATOC contracts, the 118th Base Design Standards

3. Unified Facilities Guide Specifications on www.wbdg.org.

6) SECURITY REQUIREMENTS

a) Access and General Protection/Security Policy and Procedures. This standard language is for contractor employees with an area of performance within Government controlled installation, facility, or area. The contractor will comply with all applicable installation/facility access and local security policies and procedures, which may be obtained from the Contracting Officer’s Representative (COR). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by the local installation’s Security Forces, Director of Emergency Services or local Security Office. The contractor shall ensure compliance with all personal identity verification requirements as directed by DoD, HAF and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.

WELDERS - Receive rate prescribed for craft performing operation to which welding is incidental.

Note: Executive Order (EO) 13706, Establishing Paid Sick Leave for Federal Contractors applies to all contracts subject to the Davis-Bacon Act for which the contract is awarded (and any solicitation was issued) on or after January 1, 2017. Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).

The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of ""identifiers"" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).

Union Rate Identifiers

A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that classification in the survey. Example:

PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.

Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.

Survey Rate Identifiers

Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.

Survey wage rates are not updated and remain in effect until a new survey is conducted.

Union Average Rate Identifiers

Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.

A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.

WAGE DETERMINATION APPEALS PROCESS

1.) Has there been an initial decision in the matter? This can be:

* an existing published wage determination

* a survey underlying a wage determination

* a Wage and Hour Division letter setting forth a position on a wage determination matter

* a conformance (additional classification and rate) ruling

On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.)

and 3.) should be followed.

With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations. Write to:
