STATEMENT OF WORK

Project WYTD252003 Repair CT Parking Apron

TOLEDO ANG BASE

1. GENERAL REQUIREMENTS

1.1 The contractor shall furnish all labor, material and appurtenances to perform work in accordance with the drawings, specifications and 3-270-01 Unified Facilities Criteria (UFC) for:

A. Saw cut and remove section of concrete aircraft apron that have failed.

B. Replace the concrete sections installing proper reinforcement.

C. Repaint airfield markings that are lost due to this projects concrete work.

This work shall include all accessories, parts and equipment necessary to make a complete and usable system. Only U.S. manufactured equipment shall be installed for this project.

1.2 The location of the proposed work is within the Toledo ANG Base, Swanton, Ohio.

1.3 The contractor shall not exceed the scope of work without prior approval from the Contracting Officer.

1.4 The contractor shall protect all government property from damage. Any damages incurred in pursuance of this project shall be replaced and/or restored to original condition.

1.5 The contractor shall maintain a clean and safe job site at all times to prevent damage to aircraft on concrete parking apron, other existing utilities, and surrounding facilities that may be affected; this is also intended to protect building occupants from potential health hazard associated with the proposed work. This shall include dust, debris, and waste created in performance of this project; along with tools, equipment, and vehicles located at the job site. The contractor shall comply with the provisions of the Occupational Safety and Health Act of 1972 and all provisions thereto.

1.6 The Contractor is responsible for properly laying out the work, and for lines and measurements for the work executed under the Contract Documents.

A. The Contractor, NOT the Government, is responsible to field verify all existing underground utilities within the excavation limits and/or as shown on the Drawings, before laying out the work. The Contractor is to report errors or inaccuracies in writing to the Contracting Officer's Representative before commencing work.

B. The Contracting Officer or his Representative will in no case assume the responsibility for any layout of the work or damages to existing underground utilities while performance of work is underway.

1.7 THE CONTRACTOR SHALL FIELD VERIFY ALL DIMENSIONS. The

dimensions provided by the government are only approximations and they are not responsible for their precision or accuracy. This includes but is not limited to blueprints, drawings, schedules, and statements of work.

1.8 The contractor shall provide a one-year warranty for the work contained herein.

They shall pass along extended guarantees for equipment and products.

1.9 All trash, waste, and construction debris generated from this project shall become property of the contractor and shall be removed from the base. The disposal of these materials shall follow the appropriate Federal and State laws. The 180th FW Solid Waste Disposal and Tracking form needs completed by the contractor at the time of disposal.

This includes the disposal or recycling of hazardous materials.

1.10 Bids for this project shall be valid for 90 days.

1.11 Attending the site visit is highly encouraged for this project.

1.12 Repairs will be phased into operational sections to allow the 180th to continue flight access and aircraft staging. Phasing shall allow the 180th to always access the main hanger. Free zone areas will be established for each phase to allow contractor ease of movement to conduct work. The contractor shall advise the COR how they will man the project prior to starting work so the government can determine how many escorts may be needed for each item of work.

2. PROJECT DESCRIPTION

2.1 Scope: The contractor shall provide all labor and materials to install the following, as outlined in the specifications and drawings:

A. Provide joints as detailed in the drawings and supported by the UFC for full depth concrete repairs. Spacing of transverse and longitudinal joints shall match existing adjacent slabs.

B. Submit technical data, cut sheets, samples, test reports, or other items listed below using an AF Form 3000 filled out and signed in appropriate areas.

i. Concrete Mix Design.

ii. Project Schedule with percentages.

iii. Joint Detail Plan based on the Concrete Repair Plan

iv. Paint product data

v. Quality control & test reports for concrete including slump, air content, temperature and compressive strength.

vi. Fill out the 180th FW Solid Waste Disposal and Tracking Form.

C. The contractor shall mark all areas per the drawing for each step. The government reserves the right to take square footage and re-prioritize to another area within the contract area.

D. Provide lighted low profile barricades around free zone work area until the new concrete is properly cured and ready for normal aircraft traffic.

E. Remove existing concrete sections as indicated on the drawings by means of a saw and no overcut will be allowed on the corners. Existing concrete is 11-14” thick. Remove slabs so adjacent existing slabs which are to remain are not damaged. The contractor shall evaluate the affected concrete area and possibly increase the size up to 15% for final saw cut dimensions.

F. Slabs and slab joints are joined with dowels or reinforced as shown on the

Details Drawings. The contractor shall provide materials and install required joints and reinforcements to prepare for new concrete.

G. Place, consolidate and finish new concrete at the areas where concrete was removed. Concrete mix shall be a minimum of 4500 psi concrete. Concrete shall have a slump between 1” and 4” maximum with an air content of 5% +/- 1%. Water to cement ratio of mixed concrete shall not exceed 0.45.

H. Provide a finished surface at the specified grades free of defects, bumps and depressions. Concrete placed that has low or high spots (exceeds 1/4” in 12’ measured in any direction) will be replaced with no additional cost to the government. Match existing elevations at perimeter of repairs. Finish shall be a medium broom finish.

I. Saw cut contraction joints the same day that the concrete is placed. Initial saw cut shall be 1/8” wide and 3” deep (+/- 1/2”).

J. Curing: Apply 1 coat of spray applied white curing compound to new concrete as recommended by the manufacturer. Apply uniformly at a rate of 1 gallon per 200 square feet or per manufacturer recommendations.

K. Protection of concrete: Protect concrete from loss of moisture and rapid temperature change for at least 7 days.

L. Joints and Joint Sealants: ASTM 05893, DOW sill 888 or approved equal.

Provide joints as described on the drawings and in this statement of work.

Widen joints by saw cutting cured concrete so as not to damage the surface.

Joints shall be ½” wide by 1” deep. Sealant shall fill joint to within 1/16” of the top of the joint. Provide backer rod as needed. Install joint sealants according to the manufacturer’s written requirements AND dimensions for the specified joint size. Compression type preformed sealants may also be used if approved by submittal.

M. Provide concrete testing services, samples and test results for each day’s work. Test concrete for air content and for slump. Make test cylinders for testing compressive strength of the new concrete. For concrete placed during cold weather, provide one additional specimen cured under the same conditions as the installed work. Make enough test cylinders for 7-day, 14-day, and 28-day compressive strength test results for each day’s work.

N. Provide reports for each day’s sampling and testing performed. Weekly progress reports submitted to govt. on the following Monday will be required under this contract using electronic or paper means of reporting. Contractor shall submit a project schedule with percentages for payment progress.

Progress meetings shall be weekly.

O. Pavement Markings: Paint yellow taxi-lines at concrete repairs. Security markings shall also be painted if removed by existing concrete demolition.

Paint Specification. Paint products shall meet the requirements of Federal Specification TT-P-1952D, Type I. For airfield pavement applications, use the following color chip numbers from SAE-AMS-STD-595 when ordering or specifying paint colors.

Yellow – 33538, Red – 31136, White – 37925, Black – 37038

Apply paint at a rate of 12-14 mil wet film thickness for the taxi-line and red security lines.

Apply paint at a rate of 7 to 9 mil wet film thickness for the restricted area warning signs.

REFLECTIVE GLASS BEADS ARE NOT REQUIRED FOR THIS

2.2 Special Considerations: There are NO special project considerations other than requirements on the drawings and as required in specification sections of the project documents.

2.3 Government Furnished Property: There is NO Government furnished property provided for this project.
