Combined Synopsis & Solicitation

Optimizing the Human Weapon System

(i) This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued.

(ii) The solicitation number is W50S81-25-Q-B003, is hereby issued as a Request for Quote (RFQ) for Optimizing the Human Weapon System services located at Barnes Air National Guard Base, Westfield, MA 01085. A contract will be awarded without discussions as a single, Firm- Fixed Price (FFP) award utilizing FAR Part 13, Simplified Acquisition Procedures.

(iii) This solicitation document incorporates provisions and clauses are those in effect through Federal Acquisition Circular 2025-05, dated 08/07/2025 and the Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice 01/17/2025. It is the contractor's responsibility to become familiar with applicable clauses and provisions by visiting https://www.acquisition.gov/.

(iv) This requirement is a small business set-aside using Simplified Acquisition Procedures in accordance with (IAW) Federal Acquisition Regulation (FAR) Part 13 under NAICS code 621340 – Offices of Physical, Occupational and Speech Therapists, and Audiologists. The small business size standard is $12.5M. Interested vendors must certify to the applicable NAICS via their SAM Representations and Certifications. If an interested vendor is not registered to this NAICS, they are encouraged to add this NAICS to the SAM Reps and Certs prior to solicitation close date.

(vi) The Massachusetts Air National Guard has a requirement for Optimizing the Human Weapon System (OHWS), as defined in the Performance Work Statement (PWS) dated 08 August 2025.

(vii) The anticipated Period of Performance for this contract will start 30 September 2025 through 29 September 2026 at Barnes Air National Guard Base, Westfield, MA 01085.

(viii) The provision at 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition. See Provisions and Clauses document.

***Addendum to FAR 52.212-1***

The Offeror shall submit following items.

1. The Offeror shall provide documentation on the qualifications, certifications, and training of the proposed individual

2. The Offeror shall provide documentation to outline the past experience, years of experience, and expertise in the area. Neck, Spinal, and/or Lower Back.

3. The Offeror shall provide documentation on information included in individual training plans, possible examples of individual training plans or topics that will be included in training plans.

4. The Offeror shall provide proposed areas and items of metrics to be tracked, as well as details on how information will be tracked and presented as required by the performance work statement.

5. The Offeror shall provide documentation to outline the proposed method/system to schedule appointments, notifications of missed appointments, and reminder of upcoming appointments.

6. The Offeror shall submit at least two examples of within scope federal contract awards within the past 5 years by providing a reference contract number.

(ix) The provision at 52.212-2, Evaluation-Commercial Products and Commercial Services, applies to this acquisition. See Provisions and Clauses document.

***Addendum to FAR 52.212-2***

These factors are further described below:

1. Technical

a. The Offeror shall provide documentation on the qualifications, certifications, and training of the proposed individual that will be providing the services. Those that provide a licensed physical therapist that and holds a degree from an American Physical Therapist Association (APTA) accredited institution will be rated higher than those that provide other types of authorized licensed services.

b. The Offeror shall provide documentation to outline the past experience, years of experience, and expertise in the area. neck, spinal, and/or lower back. Those that provide expertise and experience that exceed the minimum requirement will be rated higher than those that provide the minimum standards. Those that provide expertise in all areas required will be rated higher than those that provide expertise in only a couple of the areas. Those that provide experience that exceeds the minimum amount will be rated higher than those that meet the minimum times outlined.

c. The Offeror shall provide documentation on information included in individual training plans, possible examples of individual training plans or topics that will be included in training plans. Those plans that are provided that are simple to understand, yet still meet all requirements outlined in the PWS will be rated higher than those that are more complicated and not as easy to understand. Those that provide a general outline will be rated lower than those that provide a detailed example.

d. The Offeror shall provide proposed areas and items of metrics to be tracked, as well as details on how information will be tracked and presented as required by the performance work statement. Those that provide details that cover all aspects of the requirements will be rated higher than those that just provide a general overview. Those that provide details on how the information will be presented will be rated higher than those that just state the information will be provided.

e. Provide documentation to outline the proposed method/system to schedule appointments, notifications of missed appointments, and reminder of upcoming appointments. Those that have a working system that can be reviewed and worked will be rated higher than those that just outlined a general system. Those that provide an easy operating system to book appointments with reminder for appointments will be rated higher than those that are more complicated to navigate.

2. Past Performance:

Submit at least two examples of within scope federal contract awards within the past 5 years by providing a reference contract number.

3. Price:

The Government will evaluate the pricing proposed by the Offeror for fair and reasonableness based upon pricing received from other Offerors. The Total Evaluated Price (TEP) will be the sum of the base year.

(x) The provision at 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services. (Alternate I) (Feb 2024), applies to this acquisition. Offeror shall complete the annual representations and certifications electronically via the System for Award Management (SAM) website. See Provisions and Clauses document.

(xi) The clause at 52.212-4, Contract Terms and Conditions-Commercial Products and Commercial Services, applies to this acquisition. See Provisions and Clauses document.

(xii) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Products and Commercial Services, applies to this acquisition.

(xiii) IAW FAR 52.204-16, prospective awardees shall have an active Cage Code prior to award of a government contract. To register for a Cage Code and to complete Online Representations and Certifications, go to the System for Award Management (SAM) at https://www.sam.gov/SAM/. All vendors interested and capable of obtaining contract award must be registered with the Wide Area Work Flow (WAWF) located at https://piee.eb.mil/. Vendors are required to create and submit invoices electronically through WAWF and receive payment via electronic funds transfer (EFT) for supplies or services rendered.

(xiv) The Defense Priorities and Allocations System (DPAS) and assigned rating, is not applicable to this acquisition.

(xv) Quotes must be submitted via email no later than (NLT) 2:00 P.M. EST 05 September 2025 and IAW Addendum to FAR 52.212-1, to 104.fw.msc@us.af.mil with solicitation number W50S8125QB003 in the subject line. Requests for Information (RFI) concerning this requirement must be submitted in writing to the email listed below NLT 2:00 P.M. EST 25 August 2025.

Service Contract Act Wage Determination #2015-4095 Rev. #31, dated July 8, 2025 applies to this solicitation.

(xvii) National Guard Bureau, Agency Protest Program

National Guard Agency Protest Program is intended to encourage interested parties to seek resolution of their concerns with the National Guard (NG) as an Alternative Dispute Resolution forum, rather than filing a protest with the Government Accountability Office (GAO) or other external forum.

Contract award or performance is suspended during the protest to the same extent, and within the same time periods as if filed at the GAO. The NG protest decision goal is to resolve protests within 35 calendar days from filing. An agency protest may be filed with either the Contracting Officer or the NG Agency’s Protest Decision Authority, but not both, in accordance with NG protest procedures.

To be timely, the protests must be filed with the periods specified in FAR 33.103.
