About this file

This is a Disclosure of Lobbying Activities form (Standard Form LLL) submitted by the State of Florida Department of Transportation in connection with the Escambia County Construction Engineering Inspection (CEI) services solicitation for the O.C. Phillips Road Over Brushy Creek Bridge Replacement project. The form is used to disclose any lobbying activities undertaken to influence federal or state actions related to covered federal actions, including contracts, grants, cooperative agreements, and loans. The disclosure requires reporting entities to identify the type and status of the federal action, provide details about the reporting entity and any prime contractor or subawardee relationships, and list the names and addresses of any lobbying registrants or individuals performing lobbying services. The form must be completed at the initiation of a covered federal action or when material changes occur to previous filings.

The form serves as a material representation of fact upon which reliance is placed by the tier above when a transaction is made or entered into, as required by 31 U.S.C. section 1352. Failure to file the required disclosure subjects the reporting entity to civil penalties ranging from $10,000 to $100,000 for each failure. The document includes detailed instructions for completion, referencing the Office of Management and Budget's implementing guidance, and notes that the valid OMB Control Number is 0348-0046 with an estimated public reporting burden of 10 minutes per response. All information collected through this form is available for public inspection.
