A plan for protecting personnel and property during the transport, storage, and use of any hazardous materials, C. Procedures for spill response and disposal, D. Material Safety Data Sheets (MSDS) for all materials listed in paragraph a.

10.1.1. The Contractor shall erect a board On Site for the purpose of posting all permits, MSDS, wage determinations, safety plans etc.

10.2.1. Preparation: Make a survey to locate and identify all isolating devices to be certain which switches, valves, or other energy isolating devices apply to the equipment to be locked or tagged out. More than one energy source such as electrical, mechanical or others may be involved.

10.2.2. Sequence of Lockout/Tagout Procedure:

A. Notify all affected personnel that a lockout/tagout system is going to be used and the reason therefor. The authorized personnel shall know the type and magnitude of energy that the machine or equipment uses and shall understand the hazards thereof.

B. If the machine or equipment is operating shut it down by the normal stopping procedure such as depressing stop button, open toggle switch, etc.

C. Operate the switch, valve, or other energy isolating devices so that the equipment is isolated from its energy source. Stored energy such as in charged capacitors, springs, elevated machine members, rotating flywheels, hydraulic systems and other pressurized air, gas, steam, or water systems etc. must be dissipated or restrained by methods such as repositioning, blocking, or bleeding down the stored energy.

D. Lockout and/or tagout the energy isolating devices with assigned individual locks, tags, or additional safety measures as may be required.

E. After ensuring that no personnel are exposed, and as a check on having disconnected the energy sources, operate the push button or other normal operating controls to make certain the equipment will not operate. CAUTION: Make sure the operating controls are returned to the "NEUTRAL" or "OFF" position after the test.

F. The equipment is now locked out or tagged out.

10.2.3. Restoring Machines or Equipment to Normal Production Operations:

Operate the energy isolating devices to restore energy to the machine or equipment.

10.2.4. Procedure Involving More Than One Person: In the preceding steps, if more than one individual is required to lockout or tagout equipment, each shall place his/her own personal lockout or tagout device on the energy isolating devices. When an energy-isolating device cannot accept multiple locks or tags, a multiple lockout or tagout hasp shall be used. If a single lock is used to lockout the machine or equipment, the single key shall be placed in a lockout box or cabinet, which allows the use of multiple locks to secure it. Each employee shall then use his/her own lock to secure the box or cabinet. As each person no longer needs to maintain his/her lockout protection, only that person shall remove his/her lock from the box or cabinet.

10.3 Fall Protection: In regard to OSHA FALL PROTECTION requirements, the following shall be noted. Personnel performing any type of construction (29 CFR 1926) or general industrial work on elevated surfaces or platforms, over 6 feet, must adhere strictly to

Occupational Safety and Health Standards, specifically 29 Code of Federal Regulations, Part

1926, Sections 500, 501 and 502. Currently there are no certified anchorage points, nor permanent guardrail systems in place on any of TYAD's roofs. Attention is drawn to the requirement for Warning Line Systems, described in CFR Part 1926.502-f, and a competent person to monitor the safety of other employees per CFR Part 1926.502-h, Safety Monitoring

Content for Environmental Protection Agency (EPA) Designated Products, to the

Contracting Office. Compliance with this program does not relieve the contractor from meeting all other specification requirements.

Section 13280, Removal and Disposal of Asbestos Materials, and as otherwise required in the contract. Asbestos abatement not required under the contract as originally awarded, can be incorporated into the existing contract via contract modification, or with the government taking responsibility for the asbestos abatement. The method of acquiring the abatement is at the discretion of the Government.

f. If a project involves the demolition of any load-bearing structural members, whether or not asbestos is present, twenty (20) days prior to the demolition, the Commonwealth of

Pennsylvania, Asbestos Abatement and Demolition/Renovation Notification, Form 2700-

FM-AQ0021, revised 8/2005 (or the most recent version), shall be submitted to the proper authorities as specified in Section 13280, Removal and Disposal of Asbestos

NORTHING and EASTING. While vertical surveys will use the North American

Vertical Datum (NAVD) 1988 coordinate system for an ELEVATION as detailed in paragraphs

1 through 9 below. Providing survey data is not a substitute for other marking methods. Contractors are responsible for implementing other markings such as tracing wire or tape as required.

1) The actual location, materials and sizes within the construction area shall be measured and provided with a Northing, Easting and Elevation with respect to the proper datum as previously mentioned. These coordinates shall be shown for all change in direction for points such as T’s, Y’s, Elbows, etc. and all surface or underground components such as valves, manholes, drop inlets, clean outs, meters, etc. in order to provide the location of these lines and appurtenances which may be determined in the event the surface openings or indicators become covered over or obscured.

2) The descriptions of exterior utilities shall include the actual quantity, size, and material of the utility lines. The AS-BUILT drawings shall show dimensions to two permanently fixed surface features at the end of each run, including each change in direction. Valves, splice boxes and similar appurtenances shall be located by dimensioning along the utility run from a Northing, Easting, and

Elevation point based upon the Pennsylvania (North) State Plane coordinate system (North American Datum 1983 (NAD83)).

3) There will be an allowable tolerance of one foot (1’-0”) in either direction

(Vertical or Horizontal) for any and all utilities aforementioned.

4) The location and dimensions of any changes within the building structure.

5) Layout and schematic drawings of electrical circuits and piping.

6) Correct dimensions and details transferred from shop drawings.

7) Correct grade, elevations, cross section, or alignment of roads, earthwork, structures or utilities if any changes were made from contract plans with a

Northing, Easting and Elevation based upon the Pennsylvania (North) State

Plane coordinate system (North American Datum 1983 (NAD83)).

8) Actual location of anchors, construction and control joints, etc., in concrete.

9) Changes in location of equipment and architectural features.
