i. Critical Safety Item: A part, assembly, installation or production system with one or more critical characteristics that, if not conforming to the TDP requirements, would result in hazardous unsafe conditions. Unsafe conditions include failure modes, which cause loss or serious injury to personnel using, maintaining or depending upon the item.

ii. Critical Safety Characteristic: Any feature (i.e. tolerance, finish, material composition, material hardness, manufacturing, assembly or inspection process) of product, material, or process, which if not conforming or missing would cause the safety failure or malfunction of the critical item.

iii. The contractor shall implement 100 percent automatic inspection or test for all CSI characteristics that can be nondestructively inspected or tested.

iv. All technical quality requirements relating to critical safety characteristics must be traceable to the time and place of production. The contractor's records on file should provide the degree of traceability required to enable after the fact verification of all aspects of material, manufacture, special processing, assembly and inspection of critical safety characteristics.

v. The contractor's vendor quality program shall address controls critical safety item characteristics produced by vendors and contain a requirement for the periodic audit of the implementation of these controls. Purchase orders to subcontractors producing CSIs shall contain a requirement for 100 percent inspection of all critical safety characteristics.

vi. Nonconformance to critical safety characteristics shall not be accepted through contractor Material Review Board actions.

vii. Whereas the emphasis of the contractor's quality program procedures pertaining to CSIs is to assure nonconforming critical safety characteristics do not occur, in the event that a critical defect is found whether by the contractor, subcontractor, or the Government, the following specific actions shall be taken:

viii. Production of the affected component, subassembly, or assembly, shall cease immediately; operations involved in the creation of the defect must stop, including "feeder" operations, and the Government Quality Assurance Representative (QAR) notified at once with concurrent notification to the Procurement Contracting Officer (PCO) and Administrative Contracting

ix. All suspect material must be segregated and shall not be used for further production. Suspect product must be "fenced" both ahead and behind the point of detection. The contractor shall conduct a detailed investigation and analysis to assess the extent of product affected and take all actions necessary to assure the conformance of all suspect material. A request for the use of any fenced material associated with the critical defect, along with the analysis supporting the request, shall be forwarded to the contracting officer for approval, with copies furnished to the Government QAR and ACO (DI-SAFT-80970A)(A010).

x. A detailed investigation and analysis with Government participation shall be conducted of the process, equipment, handling, quality control, etc. to determine the cause of the defect. Corrective action to prevent future manufacture of the critical defect shall be instituted. Positive corrective action is required. Screening of the affected product alone is insufficient to meet this requirement without other appropriate action intended to eliminate the cause of the defect.

xi. A request to restart production operations along with supporting documentation (i.e., investigation results, screening information, corrective action, etc.) shall be submitted to the contracting officer for approval with copies furnished to the

Government QAR and ACO. Restart shall begin only after approval to restart has been issued by the contracting officer and any conditions or restrictions associated with the approval are satisfied. Contracting officer response to restart requests will be provided within five (5) working days from the time of receipt.
