About this file

This is a Price Proposal Forms document for the Fort Valley Utility Commission's design-build procurement for an effluent pipe installation project from Highway 49 Water Reclamation Facility to Mossy Creek in Georgia. The project involves the design and construction of approximately 4,000 linear feet of 20-inch fusible PVC effluent pipe installed approximately 6 feet below grade, including approximately 185 linear feet of jack and bore work under a Norfolk Southern railroad line within a 30-inch steel casing pipe, along with associated excavation pits, manholes, and erosion control measures. Proposals were due July 3, 2026, at 2:00 PM EST, with award anticipated within 60 days of selection. The Price Proposal must be submitted in a separately sealed envelope and remain valid for 120 calendar days following submission.

The Price Proposal shall include all costs necessary for project completion and must be accompanied by thirteen required forms: Cover Sheet, Total Proposed Price, Design Fee Breakdown, Direct Construction Cost Breakdown, General Conditions Breakdown, Overhead and Profit, Contingency, Guaranteed Maximum Price Certification, Unit Price Schedule, Material Escalation Certification, Subcontractor Pricing, Self-Performed Work identification, and ARPA Cost Certification and Price Proposal Affidavit. The unit price schedule covers items including pipeline installation, rock excavation, unsuitable soil removal, imported fill, and restoration work. The project is funded through federal ARPA funding and requires certification that all costs are allowable, reasonable, necessary, and supported by pricing assumptions, with no prohibited costs included. Failure to submit all required forms may result in the proposal being deemed non-responsive.
