About this file
This is a Vendor Responsibility Questionnaire (VRQ) document issued by the State of Georgia for use in formal solicitations exceeding $100,000 and involving public health, infrastructure, or operational service delivery. The VRQ is specifically applicable to the City of Sandy Springs' FY26 LMIG Resurfacing Project (27-008), which involves milling and resurfacing approximately 22.35 miles of roadway throughout the city. The scope of work includes milling to a minimum depth of 1.5 inches, patching with 12.5 mm or 19 mm asphalt where required, and paving with 12.5 mm asphalt at 165 lbs./SY. The City reserves the right to add or delete roadway segments during the contract term, with traffic control and mobilization costs adjusted accordingly. Award shall be made to the lowest responsive and responsible bidder meeting all minimum technical requirements.
The VRQ requires vendors to complete comprehensive disclosure sections addressing general business identification, legal and regulatory compliance, financial and operational stability, and tax compliance. Vendors must truthfully disclose any debarments, environmental violations, OSHA citations, procurement fraud convictions, scrutinized company status, contract failures, ongoing investigations, and tax liabilities within specified timeframes, typically five years or three years for tax matters. All "Yes" responses require detailed written explanations and supporting documentation. The questionnaire mandates that vendors certify their financial capacity, maintain required insurance, obtain necessary bonds, and be in good standing with the Georgia Department of Revenue. The final page must be notarized with a valid commission expiration date. Compliance with O.C.G.A. statutes 50-5-50, 50-5-82, 50-5-84 et seq., and 13-10-91 is legally required, including E-Verify utilization for public works projects.
