The affidavit is mandated under Section 787.06(13) of the Florida Statutes and must be completed and signed by an authorized officer or representative of any nongovernmental contractor entering into, renewing, or extending a contract with the School Board of Hillsborough County, Florida. The affidavit certifies under penalty of perjury that the contractor does not engage in human trafficking or labor coercion practices, including the use or threat of physical force, unlawful restraint or confinement, exploitative debt practices, confiscation of identification documents, financial harm threats, fraud or deception, or provision of controlled substances for exploitation purposes.

The vendor executing this affidavit must declare that all statements are true and accurate. This document serves as a compliance requirement rather than addressing pricing, set-asides, funding, or other contractual terms. The Benefits Administration RFP seeks a U.S.-based benefits administration outsourcing partner to deliver comprehensive employee and employer service support aligned with HCPS's strategic vision and objectives. All prospective vendors responding to this opportunity must complete and submit this affidavit as part of their proposal package.
