About this file

This is a Vendor Affidavit document for Hillsborough County Public Schools (HCPS) in Florida related to the Roof Drain Services solicitation (25494-RFP-MST). The affidavit requires vendors to certify compliance with regulations prohibiting the use of coercion for labor and services. The solicitation seeks qualified vendors to provide comprehensive roof drain maintenance and repair services, including unclogging, cleaning, repairs, and maintenance across HCPS school facilities in three geographical areas (West, Central, and East). Vendors must supply all necessary labor, materials, equipment, and incidentals to complete service requests. HCPS intends to award contracts to three primary vendors, one for each maintenance area. The question deadline is October 9, 2025, with final proposals due October 23, 2025. The initial contract term begins upon Board approval with potential automatic renewals for up to four additional one-year periods.

Pricing will be submitted using a provided Excel spreadsheet with rates for standard and non-standard hours for both Journeyman and Apprentice technicians, based on estimated annual usage of 2,500 standard hours and 200 non-standard hours, utilizing a firm, fixed price per unit model that includes delivery, trip, fuel surcharges, and service call charges. The contract includes a maximum 15% markup on materials for miscellaneous items and requires a one-year warranty on all goods and services. The solicitation incorporates set-aside provisions through the Small Business Encouragement Program (SBEP), awarding evaluation points based on the percentage of work allocated to HCPS-registered small business enterprises. Vendors must complete compliance documents including the Vendor Affidavit Regarding the Use of Coercion for Labor and Services, a Contracting with Entities of Foreign Countries of Concern Prohibited Affidavit, and a Vendor Registration Form. Participation is restricted from entities owned, controlled, or principally located in designated foreign countries of concern, including China, Russia, Iran, North Korea, Cuba, Venezuela, and Syria.
