About this file

This is a Taxpayer Identification Number (TIN) form used by the State of Illinois for contractor registration and compliance purposes. The form requires vendors to provide their correct taxpayer identification information and certify that they are not subject to backup withholding by the Internal Revenue Service. The form accommodates various business entity types, including individuals, sole proprietors, partnerships, corporations, limited liability companies, and tax-exempt organizations, with specific instructions for each category regarding name entry and identification number submission.

The form requires vendors to select their legal status from the provided options and, for corporations, to attach IRS acceptance letters (CP261 or CP277) if applicable. Vendors must provide either a Social Security Number for individuals and sole proprietors or an Employer Identification Number for business entities. An authorized representative must sign and date the form to complete the certification process. This documentation is essential for state contracting purposes and ensures compliance with federal withholding requirements and tax identification standards.
