About this file
This is a Technical Response document submitted by respondent PMT-59 in accordance with Florida Statutes Chapter 287 for the Department of Children and Families' solicitation DCF 2526 058 regarding Elevator Maintenance and Repair services at the Roberts Building located at 5920 Arlington Expressway, Jacksonville, Florida. The technical response template requires respondents to demonstrate capability to perform the work by providing organizational information, years of experience in similar services, references for three entities receiving comparable services, and statements addressing the respondent's and any subcontractors' ability to successfully complete the elevator maintenance and repair work as described in the solicitation and its supporting documentation.
The document establishes disclosure and qualification requirements for the respondent, including mandatory reporting of related parties, affiliates, partnerships, associations, conflicts of interest involving State employees, and any terminated State or Federal government contracts within the five preceding years. Respondents must complete tables identifying proposed subcontractors and their contracted services, provide organizational details including years in business and available locations, and substantiate their experience and qualifications through reference documentation. The form does not contain specific pricing terms, award dates, contract duration, renewal options, or funding information within the template itself, as these elements are addressed separately in the formal solicitation documents referenced throughout.
