EXHIBIT A-2 – SURCHARGE CERTIFICATION
SURCHARGES FOR CONTRACTED ITEMS. Bidder certifies as follows (must check one):
· BIDDER ASSESS SURCHARGES AT TIME ORDER. Washington State Department of Transportation (WSDOT) customers will obtain the surcharge from the bidder before the purchase order is placed. This surcharge can be a dollar amount or percentage. This surcharge will not change once the purchase order is placed. The surcharge can decrease but will not increase once the order is submitted. Surcharges are used to produce goods (raw material, labor rates, etc.) and shall be used on a pass-through basis not to increase profits. The bidder may be asked to show proof of the surcharge of the order in question.
OR
· BIDDER ASSESS SURCHARGES AT TIME OF SHIPMENT. Washington State Department of Transportation (WSDOT) customers will obtain the surcharge from the bidder at the time of shipment. The bidder shall give an estimate of the surcharge at the time of the order. This surcharge can be a dollar amount or percentage. This surcharge will not change once the order has shipped. The surcharge shall not increase more than 2% of the estimated surcharge. Surcharges are used to produce goods (raw material, labor rates, etc.) and shall be used on a pass-through basis not to increase profits. The bidder may be asked to show proof of the surcharge of the order in question.
OR
· BIDDER WILL NOT ASSESS SURCHARGES DURING THE LIFE OF THE CONTRACT.
· SURCHARGES CHANGE ON A SCHEDULE LISTED BELOW: (must check one):
· Weekly
· Monthly
· Quarterly
· Case by Case basis (As the item is ordered)
