About this file

This document is a Tax Compliance form provided by the State of Georgia for suppliers interested in state and local contract opportunities. The form requires suppliers to provide their company name, physical address, federal identification number, and details on any existing tax registration in the state. Suppliers must indicate the type of service they will perform and whether they will sell any tangible goods. The form also requires information on any affiliate companies, including their tax identification details. The state entity will submit the completed form to the Georgia Department of Revenue to verify the supplier's eligibility and check for any outstanding tax issues prior to contract award. Missing, incomplete, or erroneous data may delay or prohibit the supplier's verification and eligibility for the contract.

The form emphasizes that no prohibited source, as defined by Georgia law, may receive a contract award. Suppliers are strongly encouraged to check their tax status and resolve any outstanding liabilities or missing tax returns before submitting the form.
