About this file

This Tax Compliance Form is a document prepared by the State of Georgia for suppliers bidding on state and local contracts, specifically in connection with the Jones County School District's solicitation for Construction Professional services for Jones County High School Renovations in Gray, Georgia. The form requires suppliers to provide comprehensive tax and identification information, including the supplier's name, physical address, Federal Identification Number, and relevant Georgia state tax registration details such as State Taxpayer Identification Number, Sales and Use Tax Number, and Withholding Tax Number. Suppliers must also disclose whether they will sell tangible personal property or goods, identify any affiliated entities with their corresponding tax information, and designate a person responsible for handling the supplier's tax issues, including contact details for that individual.

The Georgia Department of Revenue will use the information submitted on this form to verify that suppliers are not "prohibited sources" as defined by O.C.G.A. 50-5-82 and to identify any outstanding tax liabilities or compliance issues before contract award. The state entity must submit completed forms to the Georgia Department of Revenue at tsd-state-contractors@dor.ga.gov for processing in accordance with the Georgia Procurement Manual. Suppliers are cautioned that missing, incomplete, or erroneous data may delay or prohibit verification of eligibility for contract award, and no prohibited source may receive a contract award. Suppliers are strongly encouraged to verify their current tax status and resolve any outstanding tax liabilities or missing tax returns prior to submission.
