About this file
This is a Tax Compliance Form issued by the State of Georgia for suppliers bidding on state and local contracts, specifically related to RFQ #27-22 for HP Fortis G1m Rugged Chromebooks and Google Chrome OS Education Management Licenses with the Jones County School System. The form requires suppliers to provide comprehensive tax identification and compliance information, including their Federal Identification Number (FEI), State Taxpayer Identification Number (STI), Sales and Use Tax Number, and Withholding Tax Number. Suppliers must also disclose whether they have been previously registered in Georgia, identify any affiliates with their respective tax information, and designate a person responsible for handling the supplier's tax issues. The form applies to suppliers of all service types and those selling tangible personal property or goods.
Suppliers must complete this form accurately and thoroughly, as incomplete, missing, or erroneous data may delay or prohibit verification of eligibility for contract award. The State of Georgia will submit the completed form to the Georgia Department of Revenue for verification that the supplier is not a "prohibited source" as defined by O.C.G.A. 50-5-82 and to confirm there are no outstanding tax liabilities or missing tax returns. No prohibited source may receive a contract award. The form instructs that state entities must submit completed forms via email to the Georgia Department of Revenue at tsd-state-contractors@dor.ga.gov for processing in accordance with the Georgia Procurement Manual. Suppliers are strongly encouraged to verify their tax status and resolve any outstanding tax issues prior to submission.
