About this file

This document is a bid amendment for a state and local contract opportunity issued by the Department of the Treasury, Division of Purchase and Property Procurement Bureau for the State of New Jersey. The contract is for the secure printing, storage, sale, and delivery of Certified Copy of Vital Record forms (REG-42A and REG-42B) to over 500 municipal Offices of Vital Statistics in New Jersey, as well as the State Office of Vital Statistics and Registry. The estimated annual usage is 1,913,900 forms, with packaging quantities of 100 or 500 per package. The contract term is 3 years, with up to 3 one-year extensions. Bids are due on June 20, 2024, and an optional pre-bid conference was held on April 17, 2024. Vendors must demonstrate their qualifications and capability to perform the contract scope of work.

The bid amendment provides clarification on pricing, security requirements, packaging, and other aspects of the contract. Pricing must be all-inclusive, and vendors must provide benchmark pricing for each form size and packaging quantity to allow for paper price escalation during the contract term. There are no set-aside requirements. Current pricing and usage information for the prior contract is available, but vendors should not rely on that data when preparing their bids. Vendors must complete a Security Due Diligence Third-Party Information Security Questionnaire and comply with the State's Standard Terms and Conditions.
