Supplemental Bidder Attestation for MPA-534 Elevator Service and Repairs

This document is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases for MPA-534, a Master Price Agreement solicitation for Service, Repair and Maintenance of Elevators, Escalators and other Vertical Transportation Equipment. The form requires bidders to certify that their company and all parent corporations, subsidiaries, affiliates, and subcontractors do not have conflicts of interest with any state official, officer, or agency involved in the RFP, and that they have not materially participated in or been consulted regarding the requirements, technical aspects, or formation of the solicitation. The solicitation opened May 4, 2026 at 1:00 PM EDT with a proposal deadline of June 1, 2026 at 1:00 PM EDT, with vendor questions accepted through May 25, 2026 at 4:30 PM EDT. The contract is anticipated to commence in December 2026 for an initial three-year term with options for up to two additional 12-month renewal periods. Services encompass comprehensive maintenance, repair, and potential new installation of various vertical transportation equipment types, requiring monthly maintenance, emergency response capabilities with specified service level response times, and annual safety testing in compliance with Rhode Island General Law Chapter 23-33 and the National Elevator Code Section 8.6 ASME A17.1.

Bidders must hold current Rhode Island elevator licensing, maintain general liability insurance of $1,000,000 per occurrence, workers compensation insurance of $100,000, and automobile liability insurance of $1,000,000, and register in Ocean State Procures. Evaluation is based on a cost formula whereby the lowest cost bidder receives 100 percent of available cost points, with other vendors receiving points calculated proportionally. All vendors are eligible to respond with no set-asides or restrictions based on organizational type or disadvantaged enterprise status. Prevailing wage requirements apply pursuant to Rhode Island General Laws 37-13-1 et seq., requiring submission of a Prevailing Wage Contract Addendum form and compliance with mandatory wage obligations. Pricing structure requires submission of hourly billing rates for mechanic and helper labor at standard and overtime rates across fiscal years 2027 through 2030, with monthly per-unit pricing for equipment and repair services on a cost-plus basis for parts with FOB delivery and no shipping charges to the state. A contract administrative fee of 0.33 percent of annual contract spend applies effective January 1, 2026. No incumbent vendors or current contractors are identified in the solicitation documents, and political subdivisions may participate in the resulting Master Price Agreement.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
